Percutaneous minimal osteosynthesis of fractures of the proximal humerus in elderly patients.

INTRODUCTION: The aim of this study was to evaluate the subjective and objective outcome of the percutaneous minimal osteosynthesis in elderly patients. Untreated, unstable and dislocated proximal humeral fractures show poor functional and subjective results. Reduction and fixation of the fragments is essential to achieve a good clinical outcome. Especially noted in elderly patients, the osteosynthesis is concurrent with the implantation of a prosthesis. PATIENTS AND METHODS: We examined 31 patients with a mean age of 72 years (51-87) after an average follow-up period of 15.8 months (9-31). T

Management of anteromedial coronoid fractures according to a protocol focused on instability assessment provides good outcomes with infrequent need for coronoid fixation.

BACKGROUND: Anteromedial coronoid fractures (AMCFs) are associated with persistent elbow instability and post-traumatic arthritis if managed incorrectly. It is unclear exactly which AMCFs require surgical intervention and how to make this decision. The aims of this study were to report outcomes of AMCFs managed using a protocol based on reproduction of instability using radiographic and clinical testing and to ascertain a threshold size of AMCF associated with instability. METHODS: Forty-three AMCFs were studied. Thirty-two patients formed the primary study group (group A). All were treated u

Management of upper cervical spine fractures in elderly patients: current trends and outcomes.

Upper cervical spine fractures in the elderly represent serious injuries. Their frequency is on the rise. Their early accurate diagnosis might be compromised by the existence of extensive degenerative changes and deformities. Adequate stabilisation allowing fracture healing is of paramount importance. However, the debate is ongoing as to the best protocol that can be applied taking into consideration the presence of comorbidities and the increase risk of mortality in this frail patient population. A literature review, based on PubMed, related to protocols reporting on fracture fixation of the

Trimalleolar fractures with impaction of the posteromedial tibial plafond: implications for talar stability.

BACKGROUND: Trimalleolar fractures usually include a lateral malleolar fracture, a triangular fracture of the posterolateral corner of the tibial plafond, and a horizontal or oblique fracture of the medial malleolus. A subtype of a trimalleolar fracture is reported, differing in the shape and extent of the medial malleolar fracture and the posterior lip fracture, with implications for treatment. METHODS: In a 3-year period, 10 patients were treated for a trimalleolar fracture with a multifragmentary transverse fracture of the entire posterior tibial lip, including the posterior colliculus of

[Long-term results of treatment of supra-condylar humerus fractures in children].

From 1982 to 1986, a total of 106 children with supracondylar fractures of the humerus were treated at the trauma surgery clinic in Braunschweig. Undisplaced or slightly displaced fractures were treated with plaster casts. More severely displaced fractures were initially treated by closed reduction under general anesthesia and percutaneous Kirschner wiring. In 72 cases (= 85.7%) a follow-up examination revealed a very good or good result. In 9 cases (= 10.7%) satisfactory results were found and in 3 cases (= 3.6%) unsatisfactory results. The most common complication prejudicing the result was

Ipsilateral injuries of the elbow and forearm in children.

Fracture of the forearm with ipsilateral injury of the elbow joint is a rare traumatic entity in children. Twenty-four such cases were treated in our clinic during the last 4 years (1981-1984). The treatment was surgical in six cases and related to the elbow injuries, while the forearm underwent closed treatment. The results of these cases, regardless of treatment and severity of the injury, did not differ appreciably from the results of common forearm or elbow injuries.

[Sequela of pelvic fractures (author's transl)].

Different kinds of pelvic fractures with the presumingly most frequent and serious complication of a socket fracture with luxation and the resulting danger of a hip-condyle necrosis are described concisely. Certain problems of therapy resulting from this are partially dealt with. Expert opinions of the accident sequences and the degree of body damage, which has to be assessed, are pointed out. As a progression must be faced in case of a posttraumatic coxarthrosis a re-examination over a longer period (10 years and more) is recommended.

A high-entropy gradient filler metal enables high-strength joints of Ti2AlNb and GH4169 alloys.

Vacuum brazing with an innovative filler alloy is critical for manufacturing high-precision aerospace components requiring exceptional comprehensive performance, yet achieving superior strength in Ti2AlNb/GH4169 brazed joints remains challenging. This study proposes a (TiZrHf)50(NiCu)45Al5/(TiZrHf)30(NiCu)65Al5 high-entropy gradient filler metal (HGFM) to join the Ti2AlNb alloy and the GH4169 superalloy, which relieved the stress concentration and elevated microstructure stability, achieving a maximum shear strength of 335 MPa. The lower thermodynamic inclination and high entropy characteristi

Robotic-assistance and computer-navigation have similar rates of intraoperative fracture and return to the operating room within 1 year to fluoroscopy-only direct anterior total hip arthroplasty.

INTRODUCTION: The use of technology in direct anterior approach (DAA) total hip arthroplasty (THA) is expanding. Although the use of computer-navigation (CN-THA) and robotics (RA-THA) has previously demonstrated improved component positioning, it is important to understand whether its use is associated with differences in intraoperative complications or early return to the operating room when compared to fluoroscopy-only (manual-THA) DAA THA. METHODS: 3433 DAA THAs (226 RA-THA, 1007 CN-THA, 2200 manual-THA) performed at a single institution were retrospectively reviewed. Cohorts were adjusted

The clinical outcomes and tuberosity healing after reverse total shoulder arthroplasty for acute proximal humeral fracture using the turned stem tension band technique.

BACKGROUND: Recent studies have reported the clinical effectiveness of tuberosity healing after reverse total shoulder arthroplasty in patients with proximal humeral fractures with respect to joint stability, long-term survival, and postoperative range of motion. However, it is challenging to achieve robust fixation of the fragile bone fragments in elderly patients. This study aimed to report on the radiographic and clinical outcomes of patients who underwent reverse total shoulder arthroplasty for acute proximal humeral fractures using a Turned stem Tension Band technique-a simple suture conf
