[Fabry-Anderson disease: current state of knowledge].

Fabry-Anderson disease is a lysosomal storage disease caused by deficiency of the enzyme alpha-galactosidase. This enzymatic defect results in the accumulation of glycosphingolipid into different lines cells. Usually the deficiency is complete, resulting in a multisystem disorder, with injury in different organs, predominantly heart, kidney and nervous system. However, in some patients the enzymatic deficit is partial and causes diverse clinical variants of the disease (renal or cardiac variety), this cause a difficult diagnostic and the absence of real epidemiology data. This review is about

Ischemia-reperfusion and neonatal intestinal injury.

We review research relating ischemia/reperfusion to injury in the neonatal intestine. Epidemiologic evidence suggests that the most common form of necrotizing enterocolitis is not triggered by a primary hypoxic-ischemic event. Its late occurrence, lack of preceding ischemic events, and evidence for microbial and inflammatory processes preclude a major role for primary hypoxic ischemia as the sentinel pathogenic event. However, term infants, especially those with congenital heart disease who have development of intestinal necrosis, and those preterm infants with spontaneous intestinal perforati

Ranolazine attenuates behavioral signs of neuropathic pain.

Ranolazine modulates the cardiac voltage-gated sodium channel (NaV 1.5) and is approved by the FDA in the treatment of ischemic heart disease. Ranolazine also targets neuronal (NaV 1.7, 1.8) isoforms that are implicated in neuropathic pain. Therefore, we determined the analgesic efficacy of ranolazine in a preclinical animal model of neuropathic pain. Both intraperitoneal and oral administration of ranolazine dose-dependently inhibited the mechanical and cold allodynia associated with spared nerve injury, without producing ataxia or other behavioral side effects. These data warrant clinical in

The pharmacology of nitroxyl (HNO) and its therapeutic potential: not just the Janus face of NO.

Nitroxyl (HNO), the 1-electron reduced and protonated congener of nitric oxide (NO), has received recent attention as a potential pharmacological agent for the treatment of heart failure and as a preconditioning agent for the mitigation of ischemia-reperfusion injury. Interest in the pharmacology and biology of HNO has prompted examination, or in some instances reexamination, of many of its chemical properties. Such studies have provided insight into the chemical basis for the biological effects of HNO, although the biochemical mechanisms for many of these effects remain to be established. In

Coenzyme Q10.

Coenzyme Q10 is a vitamin-like substance used in the treatment of a variety of disorders primarily related to suboptimal cellular energy metabolism and oxidative injury. Studies supporting the efficacy of coenzyme Q10 appear most promising for neurodegenerative disorders such as Parkinson's disease and certain encephalomyopathies for which coenzyme Q10 has gained orphan drug status. Results in other areas of research, induding treatment of congestive heart failure and diabetes, appear to be contradictory or need further clarification before proceeding with recommendations. Coenzyme Q10 appears

[Stimulation of the delta 1-receptors help to decrease reperfusion induced myocardial stunning].

In vivo or in vitro administration of a omega 1 receptor agonist d-SKF 10,047 (1 mg/kg intravenously or 10 mg/l in vitro) promoted an increase in the resistance of isolated perfused rat heart to ischemia/reperfusion injury. Both in vivo and in vitro stimulation of omega receptors prevents development of reperfusion contracture and release of creatine kinase and increases the developed pressure, double product, +dP/dt, and -dP/dt in the left ventricle. Activation of omega receptors has no significant effect on the occurrence of reperfusion arrhythmias ex vivo. Stimulation of cardiac sigma recep

The pathophysiology of hypertension: the importance of angiotensin II in cardiovascular remodeling.

Emerging data on the role of the tissue renin-angiotensin system are redefining our understanding of the pathophysiology of hypertension and other vascular disorders. Although the tissue renin-angiotensin system is quiescent after birth, it can be reactivated in response to tissue injury. Recent evidence suggests that this system is involved in maladaptive changes such as ventricular and vascular remodeling, alterations in renal function, and atherosclerosis. With increased recognition of the role of angiotensin II in these processes, new therapeutic opportunities are emerging. Blockade of ang

Sarns centrifugal pump for repair of thoracic aortic injury: case reports.

A new centrifugal pump (Sarns), originally designed for ventricular assist, was successfully used in two patients during repair of traumatic pseudoaneurysm of the descending thoracic aorta. The distal thoracic aorta was perfused without heparinization to avoid spinal cord and visceral ischemia, reduce afterload on the heart, and avoid clamp injury to the aorta. Distal mean aortic pressure was maintained above 50 mm Hg with a mean pump flow of 1.75 liter/minute. Proposed structural advantages of the Sarns centrifugal pump for perfusion of the distal thoracic aorta without heparin are resistance

Neonatal rheumatic disorders. View of the pediatrician.

Neonatal lupus is a unique clinical entity characterized primarily by cutaneous or cardiac injury. Dermatitis usually resolves without significant residual effects but heart block may be irreversible and life threatening during the neonatal period. SS-A/Ro and/or SS-B/La antibodies of maternal origin are present in the serum of the mother and affected infant and are markers for this syndrome. For many mothers breast feeding is the preferred choice for infant nutrition. With proper guidance, lactating mothers may safely use several antirheumatic medications such as ibuprofen, piroxicam, flurbip

Placental laceration and fetal death as a result of blunt abdominal trauma.

Placental laceration as a result of blunt maternal trauma has rarely been reported. Herein, we report a case of a 17-year-old, 37-week-pregnant woman who presented after a motor vehicle accident. Initial examination showed no fetal heart sounds or movements. Evaluation included enhanced CT scan of the abdomen. Laparotomy and Caesarian section were precipitated by the development of a coagulopathy, at which time the uterus was found to be filled with blood. Although there were no signs of external uterine injury, examination of the placenta showed a large radial laceration. The fetus had no sig

Pulmonary thromboembolism. 1. Current concepts in pathogenesis and diagnosis.

Thrombosis results from the dynamic interaction of venous stasis, hypercoagulability, and endothelial injury. Protection against thrombosis may be lost if there is deficiency of any of the factors that mediate platelet deaggregation, block fibrin deposition, or initiate fibrinolysis. A thrombus lodged in the pulmonary arterial circulation may remain hemodynamically and clinically silent or produce hemodynamic, clinical, and radiographic alterations. Although no signs and symptoms are specific to the condition, pulmonary thromboembolism can be diagnosed clinically if predisposing factors are ta

BRASH Syndrome.

BRASH syndrome is a life-threatening clinical entity characterized by Bradycardia, Renal failure, Atrioventricular (AV) blockade, Shock, and Hyperkalemia. This syndrome is often triggered in patients on AV nodal blocking agents who develop acute kidney injury, leading to a "vicious cycle" of worsening bradycardia and renal dysfunction. We present the case of an 83-year-old female with a history of chronic kidney disease and heart failure, who was admitted with severe bradycardia (31 bpm), hyperkalemia (6.9 mg/dL), and shock. Her condition was refractory to standard bradycardia treatment but im

Right aortic arch with cervical origin of left subclavian artery: Unusual arch anomaly associated with tetralogy of Fallot.

A 2-year-old girl with Tetralogy of Fallot and pulmonary atresia with a prior history of central shunt surgery was found to have an abnormal pulsation on the left side of her neck. Evaluation by computed tomography and invasive catheterization study revealed an unusual arch anomaly. There was a right aortic arch with a high-riding brachiocephalic artery and a cervical origin of the left subclavian artery, forming double hairpin loops in combination with a dilated left vertebral artery, previously unreported in the literature. Recognition of arch vessel anomalies in children with congenital hea

Runx1 in Postn-Expressing Fibroblasts But Not Cardiomyocytes Exacerbates Adverse Cardiac Remodeling Post-Myocardial Infarction.

Runx1 is strongly induced in the heart after acute injury and in chronic disease settings. Its expression negatively correlates with outcomes, and past work demonstrates a protective effect when Runx1 is inhibited through a variety of experimental means. We sought to establish which Runx1-expressing cell type(s) are responsible for the adverse remodeling phenotypes observed upon its induction. Using distinct Cre drivers, we ablated Runx1 from cardiomyocytes and fibroblasts. Following experimental myocardial infarction, we established that loss of Runx1 in fibroblasts, but not cardiomyocytes, i

Post-infarct lateral extension-Brody effect or stent thrombosis?

It is a common dilemma in determining whether to restudy a patient who has just received a stent in the left anterior descending artery for an anterior ST-elevation myocardial infarction, particularly when the ST-segments rise again and extend to lateral precordial leads. As a precaution, such patients frequently undergo repeat coronary angiography, which usually confirms stent patency. It appears that a dilated post-infarct left ventricle, affected by reperfusion injury, produces a Brody effect that mimics lateral infarction. The increase in intracavity blood volume renders the heart more ele

Case of Ischemic Hepatitis in the Setting of HFpEF Exacerbated by Persistent Atrial Fibrillation.

Congestive hepatopathy in the setting of chronic heart failure is predominantly cholestatic. Severe hepatocellular injury can be seen in cardiogenic shock, usually in an acute setting with severe reduction in ejection fraction and with significant hypotension. Hepatic ischemia with preserved ejection fraction in the setting of atrial fibrillation has not been widely recognized, although mild elevations of liver enzymes have been seen in such patients in the chronic state. We present a patient with preserved ejection fraction, rapid atrial fibrillation with hypotension who had ischemic hepatiti

Roles of Ferroptosis in Cardiovascular Diseases.

Ferroptosis is an iron-dependent regulated cell death characterized by lipid peroxidation and iron overload, which is different from other types of programmed cell death, including apoptosis, necroptosis, autophagy, and pyroptosis. Over the past years, emerging studies have shown a close relation between ferroptosis and various cardiovascular diseases such as atherosclerosis, acute myocardial infarction, ischemia/reperfusion injury, cardiomyopathy, and heart failure. Herein, we will review the contributions of ferroptosis to multiple cardiovascular diseases and the related targets. Further, we

The potential roles of Von Willebrand factor and neutrophil extracellular traps in the natural history of hypertrophic and hypertensive cardiomyopathy.

Inflammation is often applied broadly to human disease. Despite its general familiarity, inflammation is highly complex. There are numerous injurious, immune and infectious determinants, functional elements and signaling pathways, ranging from genetic to epigenetic, environmental, racial, molecular and cellular that participate in disease onset and progression, phenotypic heterogeneity, and treatment selection and response. In addition, inflammation can be tissue and organ specific, adding a layer of complexity to achieving a detailed and translatable understanding of its role in health and di

Neurovascular Bundle Entrapment and Through Knee Amputation After Cerclage Cabling of Distal Femoral Peri-implant Fracture: A Case Report.

CASE: We report the case of an 82-year-old woman with diabetes, arteriosclerosis, chronic heart failure, and hypertension treated with an anatomical locking plate and multiple cables for a spiral-wedged periprosthetic fracture of the distal femur which was complicated by direct occlusion of the femoral artery and crush of the sciatic nerve, resulting in leg necrosis and, ultimately, through-knee disarticulation despite early recognition and arterial repair. Getting to the Heart of the Matter: Review of Treatment of Cardiorenal Syndrome.

Acute decompensated heart failure is a common cause of hospitalization with worsening kidney function or acute kidney injury often complicating the admission, which can result in further dysfunction of both systems in the form of a cardiorenal syndrome. Therapy in this arena has been largely empiric as rigorous clinical trial data to inform therapeutic choices are lacking. Here we review and discuss the available clinical evidence for common approaches to the management of this condition. A multidisciplinary approach to the care of patients with cardiorenal syndrome that relies on the experien
