Lymphocytic hypophysitis: differential diagnosis and effects of high-dose pulse steroids, followed by azathioprine, on the pituitary mass and endocrine abnormalities--report of a case and literature review.

We report on a man with a progressively increasing pituitary mass, as demonstrated by MRI. It produced neurological and ophthalmological symptoms, and, ultimately, hypopituitarism. MRI also showed enlargement of the pituitary stalk and a dural tail phenomenon. An increased titer of antipituitary antibodies (1:16) was detected in the serum. Pituitary biopsy showed autoimmune hypophysitis (AH). Neither methylprednisolone pulse therapy nor a subsequent treatment with azathioprine were successful in recovering pituitary function, or in inducing a significant reduction of the pituitary mass after a…

A prospective randomized trial comparing the 1-stage with the 2-stage implantation of a pulse generator in patients with pelvic floor dysfunction selected for sacral nerve stimulation.

OBJECTIVE: The aim of this study was to evaluate in a prospective, randomized setting if the 2-stage implant, compared to a 1-stage implant, leads to a superior subjective or objective outcome of sacral nerve stimulation after implantation of the pulse generator in patients with lower urinary tract symptoms. PATIENTS AND METHODS: We implanted a sacral (S3) foramen lead and a pulse generator (model 3023, Medtronic Inc, Minneapolis, MN, USA) in 42 patients. They were randomized in a 1-stage or a 2-stage implant if a more than 50% improvement in voided volume or reduction of residual urine was s…

Sphygmomanometrically determined pulse pressure is a powerful independent predictor of recurrent events after myocardial infarction in patients with impaired left ventricular function. SAVE investigators. Survival and Ventricular Enlargement.

BACKGROUND: There is increasing evidence of a link between conduit vessel stiffness and cardiovascular events, although the association has never been tested in a large post-myocardial infarction patient population. METHODS AND RESULTS: We evaluated the relationship between baseline pulse pressure, measured by sphygmomanometry 3 to 16 days after myocardial infarction, and subsequent adverse clinical events in the 2231 patients enrolled in the SAVE Trial. Increased pulse pressure was associated with increased age, left ventricular ejection fraction, female sex, history of prior infarction, dia…

The association of single ventricle morphology with long term quality of life among survivors: A report from congenital heart disease project to understand lifelong survivor experience (CHD PULSE).

BACKGROUND: Among individuals with single ventricle (SV) congenital heart disease (CHD), systemic right ventricle morphology is associated with decreased survival compared to systemic left ventricle morphology. We aimed to determine the association of ventricular morphology with quality of life and medical, neurocognitive, socioeconomic outcomes and compare these between SV CHD patients and siblings. METHODS: We performed a cross-sectional cohort study using data from congenital heart disease project to understand lifelong survivor experience, a survey conducted from 2021 to 2023 among adults

Spurious Fingertip Oxygen Desaturation During the 6-Minute Walk Test in a Patient With NSIP and Raynaud Phenomenon: The Value of Forehead Pulse Oximetry and Transcutaneous Oxygen Tension.

Reduced digital perfusion associated with Raynaud phenomenon may cause spuriously low fingertip pulse oximetry readings during the 6-minute walk test (6MWT), potentially leading to overestimation of exercise-induced hypoxaemia in patients with interstitial lung disease. We report a 39-year-old woman with non-specific interstitial pneumonia and Raynaud phenomenon who underwent repeated 6MWTs. Fingertip SpO2 progressively decreased from 96% to 82%, whereas forehead SpO2 remained stable (93%-98%). Simultaneously, transcutaneous oxygen tension (TcPO2) measured on the volar forearm remained unchang…

[Clinical study of Total Glucosides of Paeony Capsules for oral lichen planus with Yin-deficiency and internal-heat pattern: efficacy evaluation and analysis of tongue and pulse characteristics].

Oral lichen planus(OLP) is a chronic inflammatory oral mucosal disease with potential malignant transformation risk. Yin-deficiency and internal-heat is a common TCM subtype characterized by a red tongue with thin white coating and a thin and slippery pulse. Although western medical treatments, mainly glucocorticoids, can relieve symptoms to some extent, their efficacy is limited, and recurrence is frequent. This study aimed to investigate the clinical efficacy and safety of Total Glucosides of Paeony Capsules in treating OLP patients with the Yin-deficiency and internal-heat pattern, and to a…

Assessment of Pulpal Oxygen Saturation in Healthy and Diseased Primary Teeth Using a Customized Pulse Oximeter Holder in Children Aged Four to Nine Years: A Cross-Sectional Study.

Introduction Accurate assessment of pulp vitality is essential for diagnosis and treatment planning in pediatric dentistry. Conventional pulp sensibility tests, including thermal and electric pulp testing, assess neural response rather than pulpal vascularity and often demonstrate limited reliability in primary teeth. Pulse oximetry provides a non-invasive, objective assessment of pulpal oxygen saturation; however, evidence regarding its use in primary dentition remains limited. This study evaluated pulpal oxygen saturation in healthy and diseased primary teeth using a customized pulse oximete…

The Influence of Cognitive Heuristics and Biases on Palliative Social Workers' Support of Patient and Caregiver Decision Making: The Pulse Oximeter Buying Trend during the COVID-19 Pandemic.

A phenomenon occurring early in the pandemic involving media-based recommendations of pulse oximeters, devices purported to detect a dangerous Covid-19 symptom, invites attention to effects of decision-making shortcuts, or cognitive heuristics, and associated cognitive biases or errors, on patient/caregiver healthcare decisions. Heuristics also affect palliative/medical social workers' recommendations to patients/caregivers. This article looks at availability, confirmation, affect, false consensus and framing biases, and suggestions for debiasing decision making. Implications for other healthc…

Assessment of vascular stiffness in the internal carotid artery proximal to the carotid canal in Alzheimer's disease using pulse wave velocity from low rank reconstructed 4D flow MRI.

Clinical evidence shows vascular factors may co-occur and complicate the expression of Alzheimer's disease (AD); yet, the pathologic mechanisms and involvement of different compartments of the vascular network are not well understood. Diseases such as arteriosclerosis diminish vascular compliance and will lead to arterial stiffness, a well-established risk factor for cardiovascular morbidity. Arterial stiffness can be assessed using pulse wave velocity (PWV); however, this is usually done from carotid-to-femoral artery ratios. To probe the brain vasculature, intracranial PWV measures would be …
