Randomized clinical trial of ventilator liberation with pressure support ventilation versus therapist-implement patient-specific weaning in prolonged weaning patients via tracheostomy.

BACKGROUND: Liberation from prolonged mechanical ventilation is challenging and its outcomes are poor. Patients who failed at least three spontaneous breathing trials, often referred to as prolonged weaning patients, are usually weaned with protocolized programs in specialized weaning units, but there are no standardized strategies to facilitate their ventilator liberation. The objective of this study was to compare the ventilator liberation rate of two common ventilator weaning programs. METHODS: Tracheostomized patients with ongoing invasive mechanical ventilation for at least 21 day who we

Effects of Protective Ventilation with Lung Expansion versus Permissive Atelectasis on Pulmonary Inflammation and Mechanical Power of Ventilation in an Experimental Model of Acute Lung Injury.

BACKGROUND: The energy transferred from the mechanical ventilator to the respiratory system over time, so-called mechanical power, may cause lung injury even with protective ventilation. This study hypothesized that protective ventilation aiming at moderate lung expansion results in less mechanical power and lung injury than aiming at permissive atelectasis or maximum lung expansion. METHODS: Twenty-four anesthetized pigs with acute lung injury induced by saline lung lavage were randomly assigned to ventilation according to either the Acute Respiratory Distress Syndrome Clinical Network's low

Detecting regional overdistention during mechanical ventilation using 4-dimensional computed tomography.

Mechanical ventilation (MV) is a critical intervention for managing respiratory compromise and acute respiratory conditions. However, MV can also cause ventilator-induced lung injury (VILI) through mechanisms such as volutrauma, atelectrauma, and biotrauma. Conventional global pressure-volume (PV) loop analysis cannot capture regional mechanical heterogeneity within the lungs, which is central to amplifying VILI, particularly in the context of pre-existing lung injury. In this study, we employed four-dimensional computed tomography (4DCT) combined with propagation-based phase-contrast X-ray im

MEGA: a computational framework to simulate the acute respiratory distress syndrome.

The acute respiratory distress syndrome (ARDS) is a critical condition that necessitates mechanical ventilation (MV) to ensure sufficient ventilation and oxygenation for patients. Intensivists employ various therapeutic tools such as adjusting positive end-expiratory pressure (PEEP) levels or positioning the patient prone. However, practitioners encounter several challenges when dealing with ARDS: high variability among patients and limited understanding of underlying mechanisms. As a result, decision-making by physicians largely relies on experience. Yet, having the ability to estimate the li

Effectiveness of diaphragmatic ultrasound as a predictor of successful weaning from mechanical ventilation.

Purpose: Weaning from mechanical ventilation (MV) is the transition from ventilator dependence to independent breathing. Optimal timing reduces complications. Traditional predictors like the Rapid Shallow Breathing Index (RSBI), diaphragmatic excursion (DE), and diaphragm thickening fraction (DTF) have limitations. This study evaluates the clinical utility of diaphragmatic excursion RSBI (DE-RSBI) and diaphragm thickening fraction RSBI (DTF-RSBI) alongside conventional RSBI in predicting weaning success. Methods: An observational study was conducted in the ICU of Menoufia University Hospitals

Utilizing Indirect Intrapleural Pressure to Guide Mechanical Ventilation in Burn Patients With ARDS.

Patients with severe burn injuries are at risk of developing acute respiratory distress syndrome (ARDS). Modern ARDS management does not typically factor in how burn injury influences lung dynamics during ventilator management. Obtaining indirect intrapleural pressures via esophageal pressure monitoring (Pes) may provide unique guidance on optimal positive end expiratory pressure (PEEP) titration to prevent alveolar collapse while ensuring safe plateau pressures but is not well described in burn injured patients. This work examined the use of Pes in burn injured patients with ARDS to determine

Convolutional long short-term memory neural network integrated with classifier in classifying type of asynchrony breathing in mechanically ventilated patients.

BACKGROUND AND OBJECTIVE: Asynchronous breathing (AB) occurs when a mechanically ventilated patient's breathing does not align with the mechanical ventilator (MV). Asynchrony can negatively impact recovery and outcome, and/or hinder MV management. A model-based method to accurately classify different AB types could automate detection and have a measurable clinical impact. METHODS: This study presents an approach using a 1-dimensional (1D) of airway pressure data as an input to the convolutional long short-term memory neural network (CNN-LSTM) with a classifier method to classify AB types into

Neurally Adjusted Ventilatory Assist Versus Pressure Support Ventilation: A Comprehensive Review.

Mechanical ventilation serves as crucial life support for critically ill patients. Although it is life-saving prolonged ventilation carries risks and complications like barotrauma, Ventilator-associated pneumonia, sepsis, and many others. Optimizing patient-ventilator interactions and facilitating early weaning is necessary for improved intensive care unit (ICU) outcomes. Traditionally Pressure support ventilation (PSV) mode is widely used for weaning patients who are intubated and mechanically ventilated. Neurally adjusted ventilatory assist (NAVA) mode of the ventilator is an emerging ventil

Tracheal pressure generated by high-flow nasal cannula in 3D-Printed pediatric airway models.

OBJECTIVE: Heated and humidified high flow nasal cannula (HFNC) is an increasingly used form of noninvasive respiratory support with the potential to generate significant tracheal pressure. The aim of this study was to quantify the pressure generated by HFNC within the trachea in anatomically correct, pediatric airway models. METHODS: 3D-printed upper airway models of a preterm neonate, term neonate, toddler, and small child were connected to a spontaneous breathing computerized lung model at age-appropriate ventilation settings. Two commercially available HFNC systems were applied to each ai

Care recommendations for the respiratory complications of esophageal atresia-tracheoesophageal fistula.

Tracheoesophageal fistula (TEF) with esophageal atresia (EA) is a common congenital anomaly that is associated with significant respiratory morbidity throughout life. The objective of this document is to provide a framework for the diagnosis and management of the respiratory complications that are associated with the condition. As there are no randomized controlled studies on the subject, a group of experts used a modification of the Rand Appropriateness Method to describe the various aspects of the condition in terms of their relative importance, and to rate the available diagnostic methods a

Attaining Low Tidal Volume Ventilation During Patient Triggered Ventilation in Sedated Subjects.

BACKGROUND: Low tidal volume (VT) ventilation has become the preferred approach in patients in the ICU. Sedation reduces VT by attenuating respiratory drive. Even in deep sedation, some patients exhibit high VT. We aimed to determine factors associated with low VT ventilation in deeply sedated subjects who exhibited an inspiratory effort by examination of the acid/base balance using the Stewart model. METHODS: The medical records of 630 consecutive subjects admitted to the ICU over 1 y were reviewed retrospectively, and daily data sets of patients with a persistent inspiratory effort, PaO2 /F

Cardiogenic oscillations to detect intratidal derecruitment and overdistension in a porcine model of healthy and atelectatic lungs.

BACKGROUND: Low positive end-expiratory pressure (PEEP) can result in alveolar derecruitment, and high PEEP or high tidal volume (VT) in lung overdistension. We investigated cardiogenic oscillations (COS) in the airway pressure signal to investigate whether these oscillations can assess unfavourable intratidal events. COS induce short instantaneous compliance increases within the pressure-volume curve, and consequently in the compliance-volume curve. We hypothesised that increases in COS-induced compliance reflect non-linear intratidal respiratory system mechanics. METHODS: In mechanically ve

Coupling of EIT with computational lung modeling for predicting patient-specific ventilatory responses.

Providing optimal personalized mechanical ventilation for patients with acute or chronic respiratory failure is still a challenge within a clinical setting for each case anew. In this article, we integrate electrical impedance tomography (EIT) monitoring into a powerful patient-specific computational lung model to create an approach for personalizing protective ventilatory treatment. The underlying computational lung model is based on a single computed tomography scan and able to predict global airflow quantities, as well as local tissue aeration and strains for any ventilation maneuver. For v

Esophageal and transpulmonary pressure in the clinical setting: meaning, usefulness and perspectives.

PURPOSE: Esophageal pressure (Pes) is a minimally invasive advanced respiratory monitoring method with the potential to guide management of ventilation support and enhance specific diagnoses in acute respiratory failure patients. To date, the use of Pes in the clinical setting is limited, and it is often seen as a research tool only. METHODS: This is a review of the relevant technical, physiological and clinical details that support the clinical utility of Pes. RESULTS: After appropriately positioning of the esophageal balloon, Pes monitoring allows titration of controlled and assisted mecha

Ventilation therapy for patients suffering from obstructive lung diseases.

Severe bronchial obstruction due to one of the major pulmonary diseases: asthma, COPD, or emphysema often requires mechanical ventilation support. Otherwise, patients are at risk of severe hypooxygenation with consecutive overloading and dilatation of the right cardiac ventricle with subsequent failure. This review focuses on how to manage a calculated ventilation therapy of patients suffering from bronchial obstruction and relevant patents. Options and pitfalls of invasive and non-invasive ventilation in the intensive care setting regarding clinical improvement and final outcome are discussed

Spontaneous effort causes occult pendelluft during mechanical ventilation.

RATIONALE: In normal lungs, local changes in pleural pressure (P(pl)) are generalized over the whole pleural surface. However, in a patient with injured lungs, we observed (using electrical impedance tomography) a pendelluft phenomenon (movement of air within the lung from nondependent to dependent regions without change in tidal volume) that was caused by spontaneous breathing during mechanical ventilation. OBJECTIVES: To test the hypotheses that in injured lungs negative P(pl) generated by diaphragm contraction has localized effects (in dependent regions) that are not uniformly transmitted,

Use of a laryngeal mask airway to stop a supraglottic air leak which prevented adequate ventilation via a tracheostomy in a patient with cerebral palsy and pneumonia.

We report an adolescent developing ventilation failure due to supraglottic air leakage with the use of an uncuffed hand-made tracheal tube fit to her tracheobronchial deformity. To eliminate the supraglottic air leakage, a size 2.5 laryngeal mask airway (LMA) was inserted into the oral pharynx. Most of air leakage arose from the LMA. Supraglottic air leakage was not detected under mandatory mechanical ventilation following sealing of the 15-mm connector of the LMA with a piece of tape, and the respiratory condition of the patient gradually improved. The combination of a hand-made Y-shaped tube

[Treatment of subcutaneous and mediastinal emphysema with cuirass ventilation in a patient with Pompe disease undergoing non-invasive positive pressure ventilation].

We report a case of subcutaneous and mediastinal emphysema in a 39-year-old woman with late-onset Pompe disease who was undergoing non-invasive positive pressure ventilation (NPPV). Although the patient had a history of pneumothorax, she did not present with pneumothorax at the time of admission. She had not undergone adequate respiratory rehabilitation, which resulted in decreased respiratory compliance. We speculated that the emphysema had been caused by an increase in the airway pressure due to NPPV. Decrease in inspiratory pressure of NPPV from 14 cm H2O to 9 cm H2O made the patient dyspne
