Early Clinical Results of Percutaneous Endoscopic Transforaminal Lumbar Interbody Fusion: A New Modified Technique for Treating Degenerative Lumbar Spondylolisthesis.

INTRODUCTION: Lumbar interbody fusion is used to treat degenerative lumbar spondylolisthesis with instability. We developed a device that safely expands a percutaneous path through Kambin's triangle and used it via a new technique: percutaneous endoscopic transforaminal lumbar interbody fusion (PETLIF). We report in this study the details and outcomes of this procedure after a one year follow-up. METHODS: Twenty-five patients requiring interbody fusion for degenerative spondylolisthesis of the L4 vertebra were enrolled in this study. The procedure involved percutaneous posterior pedicle screw

A Successful Outcome Despite Delayed Intervention for Cauda Equina Syndrome in a Young Patient with a Posterior Epidural Disc Extrusion.

Epidural disc extrusion is extremely rare and may cause cauda equina syndrome. This is a surgical emergency and needs rapid decompression. Although cauda equina is commonly caused by disc herniation, this is an unusual presentation with epidural disc extrusion. We present a very rare case of Cauda Equina syndrome, resulting from an epidural disc extrusion at L3-L4 level. Patient care and progress notes were reviewed along with pre-, post-, and intra-operative radiological imaging. Here, a 19-year-old male with a past medical history of type I diabetes mellitus, fell asleep on a chair at home i

Do Preoperative Corticosteroid Injections Increase the Risk for Infections or Wound Healing Problems After Spine Surgery?: A Swiss Prospective Multicenter Cohort Study.

STUDY DESIGN: A prospective multicenter cohort study. OBJECTIVES: This study evaluates the risk for surgical site infections (SSIs) or wound healing problems (WHPs) in patients who underwent corticosteroid injection before lumbar decompression surgery. SUMMARY OF BACKGROUND DATA: Corticosteroid injections are often used for the treatment of the degenerated spine. However, their well-known immunosuppressive effects could increase the risk for local infections, particularly if a surgical intervention follows the injection rapidly. METHODS: The Swiss Lumbar Stenosis Outcome Study (LSOS), which

Fibrolipomatous hamartomas of the median nerve in infancy and early childhood-imaging hallmarks, symptomatology, and treatment.

UNLABELLED: A fibrolipomatous hamartoma (FLH) is a rare lesion leading to an enlargement of the affected nerve and commonly manifests at the median nerve. Symptomatic patients are mostly adolescents or adults. In children below 10 years, this entity is rather unknown and likely to be misdiagnosed. We report three children with FLH, two severely and one mildly symptomatic, all below 4 years of age at the time of first presentation. Two of three children were initially misdiagnosed. We provide a review of the pertinent clinical and radiological findings of the entity. Two patients had a characte

Efficacy of Diffusion Tensor Imaging Indices in Assessing Postoperative Neural Recovery in Cervical Spondylotic Myelopathy.

STUDY DESIGN: Prospective observational cohort study. OBJECTIVE: The aim of this study was to analyze the efficacy of diffusion tensor imaging (DTI) anisotropy indices in predicting the postoperative recovery in cervical spondylotic myelopathy (CSM) patients and to describe postoperative changes in the DTI indices based on neurological recovery after surgery. SUMMARY OF BACKGROUND DATA: Surgical results of CSM are unpredictable and cannot be estimated based on preoperative MRI. DTI indices were found to have good sensitivity to detect changes in CSM, but their efficacy in predicting postoper

Microvascular decompression for hemifacial spasm: how can we protect auditory function?

OBJECTIVE: The nerve function of the VIIIth nerve is at risk during microvascular decompression (MVD) for hemifacial spasm (HFS). Intraoperative monitoring of brainstem auditory evoked potentials (BAEPs) can be useful in decreasing the danger of hearing loss. This study assessed the intraoperative BAEP findings related to the surgical process to clarify the optimum maneuvers of MVD surgery to avoid hearing impairment. METHODS: Hundred consecutive patients undergoing MVD for HFS with intraoperative BAEP monitoring were retrospectively reviewed. Patients were classified into four groups based o

Acute cauda equina syndrome secondary to a lumbar synovial cyst.

Cauda equina syndrome is a well described state of neurologic compromise due to lumbosacral root compression. In most cases, it is due to a herniated disc, tumor, infection, or hematoma. We report a case of rapid lumbar synovial cyst expansion leading to acute cauda equina syndrome and compare it to similar cases in the literature. The patient is a 49-year-old woman with a history of chronic low back pain who developed cauda equina syndrome. Serial lumbar magnetic resonance imaging studies demonstrated a significant increase in the size of a lumbar synovial cyst over a 2 week interval. After a

Minimally invasive tubular laminectomies in multilevel spine surgery-an illustrative case-based review of techniques and combined approaches.

Minimally invasive techniques have become part of the spine surgeons' armamentarium and are currently utilized to treat many conditions involving the cervical, thoracic, lumbar and sacral spine. Surgical treatment of severe degenerative conditions such as multilevel spinal stenosis, tandem stenosis, combination of stenosis or disk herniation and spondylolisthesis at adjacent spinal levels, as well as extensive infections or hematomas, may require a multilevel tailored approach with all the challenges that such surgical planning entails. Although the use of minimally invasive tubular decompress

Sublabral Foramen: An Anatomical Variation of Glenoid Labrum in a Patient with Superior Labrum Anteroposterior Lesion with Paralabral Cyst and Acromioclavicular Joint Arthritis.

Shoulder pathologies, particularly superior labral lesions, and may pose diagnostic and management challenges by anatomical variations such as sublabral foramen. We describe a case of a 36-year-old cricketer with acromioclavicular arthritis and a superior labrum anteroposterior lesion (SLAP) lesion accompanied by a paralabral cyst. Arthroscopic intervention revealed an incidental sublabral foramen. Surgical repair addressed the SLAP lesion with paralabral cyst decompression and acromioclavicular arthritis. The patient experienced significant postoperative improvement, resuming cricketing activ

A Case of Life-Threatening Contralateral Tension Pneumothorax during Video-Assisted Thoracic Surgery in a Patient with Retrosternal Goiter.

We report a case of a 74-year-old female with a retrosternal goiter undergoing video-assisted thoracic surgery (VATS) for a left lung lower lobectomy, necessitating one-lung ventilation (OLV). We encountered a highly unusual complication: contralateral tension pneumothorax. Forty-five minutes into the surgical procedure, a sudden cardiovascular collapse occurred. After confirming the correct positioning of the double-lumen tube and excluding mediastinal mass syndrome (MMS), tension pneumothorax was suspected. Immediate needle decompression followed by right chest tube thoracostomy resulted in

Recovery of Homonymous Hemianopsia Following the Clipping of a Large Thrombosed Anterior Cerebral Artery Aneurysm Compressing the Optic Tract: A Case Report.

Unruptured cerebral aneurysms can cause visual dysfunction by compressing the optic pathways. However, there are no reports of an aneurysm at the A1 segment of the anterior cerebral artery compressing the optic tract. A 69-year-old male presented with progressive right homonymous hemianopsia, and radiological examinations revealed a large, partially thrombosed left A1 aneurysm compressing the left optic tract. The patient underwent microsurgical clipping of the aneurysm, which significantly resolved the visual field defect. In cases of progressive visual dysfunction, aneurysms should be consid

Radiological and intraoperative findings of a rare case of vestibular paroxysmia.

Vestibular paroxysmia (VP) is a rare condition. The pathogenesis is linked to a neurovascular conflict (NVC) between an abnormal arterial loop and the VII/VIII cranial nerve complex in the cerebello-pontine angle. Due to its rarity, intraoperative findings are only anecdotally reported. Here we reported on a case of VP, showing the radiological images and the intraoperative surgical video of microvascular decompression (MVD). Further we discussed our findings considering the pertinent literature. We think that in case of VP the concordance between the side of tinnitus/hypoacusia and the side o

Arthroscopic Treatment of Avascular Necrosis of the Femoral Head.

Avascular necrosis of the femoral head arises due to inadequate blood supply to the femoral head, leading to cell death, fracture, and eventually collapse. The disease often begins as asymptomatic but can present with pain, stiffness, and limited range of motion in later phases. These symptoms cause disability, predominantly in young to middle-aged individuals. Both conservative and operative treatment modalities have been used depending on the progression of the disease. Timely surgical intervention is essential to enhance outcomes and avert stress fractures, subchondral collapse, and seconda
