Surgical fixation of distal diaphyseal radial and ulnar fractures in a macaque (Macaca fascicularis) using dual titanium locking plates: a case report.

Antebrachial fractures in captive non-human primates are challenging because unpredictable forelimb use and limited tolerance of external coaptation increase torsional loading across the radius-ulna unit. A 10-year-old, 7.8-kg male long-tailed macaque (Macaca fascicularis) presented with a closed distal diaphyseal oblique radial fracture and a concurrent transverse distal diaphyseal ulnar fracture of the right forelimb. No radiographic evidence of underlying osseous pathology was found. After initial medical management, open reduction and internal fixation of both bones was performed. A low-co

Removal of bent intramedullary nail in diaphyseal segmental femoral fracture with intra-articular distal femoral fracture after below-knee amputation.

Refracture accompanied by nail bending in healed or unhealed femoral fractures is a rare complication, and its removal is challenging intraoperatively. A man in his mid-20s presented to the emergency department with a deformed thigh with a puncture wound in a below-knee amputee limb following a road traffic accident, and radiographs revealed a diaphyseal segmental femur fracture with an intra-articular distal femur fracture with a bent femur nail in situ. Attempts to remove a bent femoral nail frequently fail non-invasively and may cause iatrogenic damage. Cutting the bent femur nail using a c

Platelet-rich plasma enhanced therapy for aseptic atrophic tibial diaphyseal nonunion: a single-center retrospective controlled study of 90 cases.

BACKGROUND: Although successful outcomes of platelet-rich plasma (PRP) in treating nonunion fractures have been reported, there are widespread confounding factors and biases in the studies. This study aims to assess the biological benefits of PRP-enhanced therapy while controlling for the two core factors of nonunion fractures-biomechanics and biology. METHODS: Retrospective analysis of medical records of patients with aseptic atrophic tibial diaphyseal fracture nonunion treated with a single locking plate in a tertiary trauma center. Patients were divided into the PRP group and the non-PRP g

Management of an Adult Open Diaphyseal Both-Bone Forearm Fracture With Severe Bone Loss: A Case Report, Literature Review, and Surgical Techniques.

Open forearm fractures involving both the radius and ulna are challenging orthopedic injuries, typically resulting from high-energy impacts. These injuries require complex surgical interventions, such as open reduction and internal fixation (ORIF) or intramedullary nailing, and often necessitate multiple approaches due to the proximity of neurovascular structures. Severe cases can lead to complications such as infection, nonunion, and functional impairment of the limb. Here, we present the case of a 42-year-old male who sustained multiple injuries in a motor vehicle accident, including an open

Evaluation of a 3D-Printed Reduction Guide for Minimally Invasive Plate Osteosynthesis of Short Oblique Radial Diaphyseal Fracture in Dogs: A Cadaveric Study.

This study aims to evaluate the clinical application of three-dimensional (3D)-printed custom reduction guides (3DRG) for minimally invasive plate osteosynthesis (MIPO) of short oblique radial diaphyseal fractures. Canine forelimb specimens (n = 24) were prepared and a diaphyseal short oblique fracture was simulated in the distal radius and ulna. Bone fragments were stabilized with the MIPO technique using a 3DRG (Group A), open reduction (Group B), or closed reduction with circular external skeletal fixation (ESF) (Group C). The diaphyseal short oblique fractures were created in each radius a

Intramedullary nail fixation versus open reduction and internal fixation for treatment of adult diaphyseal forearm fractures: a systematic review and meta-analysis.

BACKGROUND: Diaphyseal radius and ulna fractures require surgical fixation in adults. Open reduction and internal fixation (ORIF) have been considered the gold standard of treatment. The recent development of an interlocking intramedullary nail (IMN) has provided an alternative treatment method for these fractures. The objective of this meta-analysis is to compare the outcomes and complications of IMN versus ORIF for diaphyseal forearm fractures in adults. METHODS: MEDLINE and Embase were searched from January 1, 2000, through January 7, 2024. All English-language studies were included compar

Lateral minimally invasive plate osteosynthesis (MIPO) with long PHILOS for proximal metaphyseal-diaphyseal humeral fracture: surgical techniques and a clinical series.

The minimally invasive plate osteosynthesis (MIPO) for proximal metaphyseal-diaphyseal humeral fracture is an effective alternative treatment with satisfactory outcomes. In this study, we described the surgical techniques and clinical results using MIPO via a lateral approach and long PHILOS plate fixation in 23 patients. All fractures were successfully united within a mean union time of 13.5 weeks (range 9-18). There was no iatrogenic radial nerve palsy. The deltoid power was grade 5 in all patients, except for 2 patients who had associated brachial plexus injury and gunshot injury at the del

New generation of superior single plating vs. low-profile dual minifragment plating in diaphyseal clavicle fractures: a biomechanical comparative study.

BACKGROUND: Recently, a new generation of superior clavicle plates was developed featuring the variable-angle locking technology for enhanced screw positioning and a less prominent and optimized plate-to-bone fit design. On the other hand, minifragment plates in dual plating mode have demonstrated promising clinical results. The aim of the current study was to compare the biomechanical competence of single superior plating using the new-generation plate vs. dual plating using low-profile minifragment plates. METHODS: Sixteen paired human cadaveric clavicles were pairwise assigned to 2 groups

Surgical outcomes of segmental diaphyseal forearm fractures in adults: a small case series on plate osteosynthesis, intramedullary nailing, and other surgical methods.

BACKGROUND: Segmental fractures often result from high-energy or indirect trauma that causes bending or torsional forces with axial loading. We evaluated surgical outcomes of patients with forearm segmental diaphyseal fractures. METHODS: We retrospectively analyzed data from patients with forearm segmental fractures for which they underwent surgery at the Pusan National University Trauma Center from March 2013 to March 2022. We also analyzed accompanying injuries, injury severity score (ISS), injury mechanism, occurrence of open fracture, surgical technique, and treatment results. RESULTS: F

Multicenter, retrospective comparison of implant survival, complications and cost between plate and screw and intramedullary nail fixation for metastatic lesions of the diaphyseal humerus.

BACKGROUND: The humerus is a common site of metastatic disease that can be fixated with either plate and screw or intramedullary nail (IMN) constructs. A multicenter retrospective comparison study was undertaken to compare implant survival, complication rate and cost between the two constructs. No prior studies have included a cost comparison. METHODS: Databases of two academic practices were queried retrospectively to identify patients with metastases of the humerus. Inclusion criteria were a lesion in the proximal metaphysis to distal diaphysis and amenable to both implant options with avai

Risk of complications following surgical fixation of femoral diaphyseal fractures in children aged 4 to 12 years: A systematic review and meta-analysis.

INTRODUCTION: There is debate regarding the optimal surgical technique for fixing femoral diaphyseal fractures in children aged 4 to 12 years. The National Institute for Health and Care Excellence (NICE) and the American Academy of Orthopaedic Surgeons (AAOS) have issued relevant guidelines, however, there is limited evidence to support these. The aim of this study was to conduct a systematic review and meta-analysis to compare the complication rate following flexible intramedullary nailing (FIN), plate fixation and external fixation (EF) for traumatic femoral diaphyseal fractures in children

Precontoured Plate Fixation for Incomplete Atypical Diaphyseal Fracture of Femur using Three-Dimensional Printing Rapid Prototyping: Two Cases Reports.

BACKGROUND: Reconstruction intramedullary nail spanning the whole length of the femur has been the gold standard treatment for complete atypical diaphyseal fractures of the femur (ADF). However, in cases of incomplete ADF combined with severe bowing, this approach might have complications and lead to iatrogenic complete fracture. We report two cases of incomplete ADF with severe bowing using a precontoured plate (PCP) after rapid prototyping (RP) of the deformed femurs with three-dimensional printing (3DP) technology. CASE PRESENTATION: Two patients presented with gradually worsening thigh pa

Reunion of post nail aseptic non-union of diaphyseal femoral fractures by augmentation plating, decortication and bone grafting - Replacement for exchange nailing.

INTRODUCTION: The aim is to re-unite post nail aseptic non-union of diaphyseal femoral fractures in an economical and safe way with almost certainty, reducing dependence on others with early rehabilitation. The cascade of fracture healing is dependent on mechanical stability and uninterrupted cellular and bio-chemical responses (biology). Intra-medullary nailing in fresh diaphyseal femoral fractures is gold standard treatment with high success rate, still non-union of these fractures is reported, though the incidence has declined considerably. METHOD: This prospective study conducted at level

Antero-lateral minimally invasive plate osteosynthesis (MIPO) with the radial nerve exploration for extra-articular distal-third diaphyseal fractures of the humerus.

INTRODUCTION: Traditional open reduction and internal fixation (ORIF) of extra-articular distal humerus fractures has a risk of iatrogenic radial nerve injury, extensive soft tissue stripping, and long incision scar. We performed an antero-lateral minimally invasive plate osteosynthesis (MIPO) technique with the radial nerve exploration for distal-third diaphyseal fractures of the humerus and evaluated clinical and radiographic outcomes through this respective study. METHODS: From April 2010 to June 2016, 28 cases of extra-articular distal-third diaphyseal fractures were treated with an anter

A new mini-invasive technique in treating pediatric diaphyseal forearm fractures by bioabsorbable elastic stable intramedullary nailing: a preliminary technical report.

BACKGROUND AND AIM: Operative treatment is often indicated in unstable pediatric diaphyseal forearm fractures. Recently minimally invasive reduction and elastic stable intramedullary nailing have been of increasing interest, instead of open reduction and internal fixation with plates. There are several disadvantages of metallic intramedullary implants, such as soft-tissue irritation and a risk of disturbing later imaging. Thus, they are generally removed in later operations. We aimed to develop a new technique to stabilize pediatric forearm fractures by the bioabsorbable intramedullary nailing

Marked phenotypic variability in progressive diaphyseal dysplasia (Camurati-Engelmann disease): report of a four-generation pedigree, identification of a mutation in TGFB1, and review.

Progressive diaphyseal dysplasia (PDD) (Camurati-Engelmann disease) is an autosomal dominant craniotubular dysplasia characterized by hyperostosis and sclerosis of the diaphyses of the long bones and the skull. Mutations in transforming growth factor beta-1 (TGFB1) were recently found in patients with PDD. We report on a four-generation pedigree with seven individuals affected by PDD, linkage and mutational analysis results, and review the literature. This pedigree demonstrates the autosomal dominant inheritance pattern, remarkable variation in expressivity, and reduced penetrance. The most se…

Documented Fracture-Healing Outcomes After Operative and Nonoperative Management of Closed Mid-Diaphyseal Humerus Fractures in Active-Duty Service Members.

INTRODUCTION: Closed diaphyseal humerus fracture treatment is nuanced with regard to the selection of surgical candidates. Fracture union is crucial, with historical data suggesting rivaling union rates between operative and nonoperative treatment; however, recent literature suggests increased union rates with operative treatment. Although nonoperative treatment of most fracture patterns is supported by the literature, there remains an argument for operative treatment for select patient populations, especially when considering union rates. Evidence regarding fracture-healing outcomes in young
