With high-resolution MR imaging, the collateral ligament of the elbow can be directly evaluated; thus ligamentous injuries can be detected, localized, and graded. The functional anatomy of the ligaments of the elbow is reviewed. The normal appearance of the ligaments on MR images as well as the clinical and MR imaging findings of medial and lateral collateral ligament injury are presented.

MR imaging is a useful method for evaluating nerve disease. It can portray the normal anatomy and identify unsuspected space-occupying masses. Severe intrinsic nerve disease can be depicted. This article outlines the normal anatomy of the three major nerves that traverse the elbow joint: the ulnar nerve, the median nerve, and the radial nerve. Entrapment and compression neuropathies of each nerve are discussed in detail. Finally, the role of MR imaging in delineating each nerve abnormality is examined.

The most common elbow injuries appear to have a pathophysiology that is microtrauma based but with multifactorial origin. This pathophysiology is a process of gradual degeneration on a cellular and tissue level, with resulting alterations that predispose to more overt injury as the athlete responds to the inherent demands of the sport. Arthroscopy of the elbow has proven to have a diagnostic and therapeutic benefit. Removal of loose bodies and a thorough evaluation of associated intra-articular pathologies are possible without arthrotomy. The relative importance of hyperreflexia and paresis in disturbances of voluntary arm movement was studied in a group of patients (n = 25) with spasticity arising from a unilateral ischemic cerebral lesion. Twelve of 14 patients who underwent arthrolysis of the elbow for post-traumatic stiffness were re-examined after a mean of 62 months. Pain and subjective stiffness were improved. The mean function of the elbow improved from 73 degrees to 112 degrees. Those operated on within a year of their injury improved twice as much as those who had been operated on after a longer period. Arthrolysis may be very rewarding when conservative treatment fails, especially when it is carried out within a year of the injury.

The effectiveness of elbow arthroscopy was assessed in 35 consecutive patients (primarily weight lifters and baseball players) treated by one surgeon. The most commonly treated lesions were loose bodies and impinging spurs. Most elbow problems resulted from repetitive or acute athletic trauma. Repetitive stress injuries usually involved the athlete's dominant arm (91%). A standardized rating system demonstrated significant improvement at 24 months' average follow-up. Proximal radioulnar transposition in an elbow dislocation is rare. Only three cases have been recorded. Translocation can easily be missed unless radiographs are carefully examined. Proximal radioulnar translocation has been believed to be iatrogenic. The injury we report was not iatrogenic, and mechanical locking of radial head and neck fracture necessitated open reduction.

Granuloma annulare may be recurrent and various precipitating factors have been described but seasonal cases have not been previously reported. We describe two patients with granuloma annulare on the elbows which developed in spring and resolved spontaneously in October each year. We believe that this type of seasonally recurrent granuloma annulae has not been previously described.

Thirteen elbows affected by severe haemophilic arthropathy and treated by silastic interposition arthroplasty were followed up for at least five years. The severity of pain, the frequency and severity of spontaneous haemorrhage and the range of movement were assessed before operation and at review. All patients were much improved and needed less factor replacement. Three elbows were revised, one for infection and two because of fragmentation of the silastic sheet. They regained good function following revision.

Fracture of the forearm with ipsilateral injury of the elbow joint is a rare traumatic entity in children. Twenty-four such cases were treated in our clinic during the last 4 years (1981-1984). The treatment was surgical in six cases and related to the elbow injuries, while the forearm underwent closed treatment. The results of these cases, regardless of treatment and severity of the injury, did not differ appreciably from the results of common forearm or elbow injuries.
