Stability of low-frequency residual hearing in patients who are candidates for combined acoustic plus electric hearing.

The purpose of this study was to investigate the stability over time of low-frequency auditory thresholds to better determine if the new technique of using a short-electrode cochlear implant that preserves residual low-frequency acoustic hearing can be a long-term solution for those with severe-to-profound hearing loss at high frequencies. The present study determined the long-term rate of decline in acoustic hearing in patients who have a preexisting hearing loss yet have not been implanted with a cochlear implant. A retrospective analysis of patients' audiograms that fit into the range for c

Temporal Bone Changes in the Angle of Insertion after Cochlear Implantation and Its Effects on Outcome: Unlocking the Controversy.

INTRODUCTION: A meaningful social life depends on our ability to perceive and respond to our environment, with hearing being crucial. Deafness impacts both individuals and families, making cochlear implantation (CI) vital for those with hearing loss. Success in CI relies on factors like proper patient selection, electrode placement, and postoperative monitoring. Studies show deeper electrode insertion and more active electrodes improve speech perception, while optimal angles and electrode lengths enhance outcomes. Despite advancements, the long-term effects of changes in electrode insertion an

Nanostructured gold electrodes promote neural maturation and network connectivity.

Progress in the clinical application of recording and stimulation devices for neural diseases is still limited, mainly because of suboptimal material engineering and unfavorable interactions with biological entities. Nanotechnology is providing upgraded designs of materials to better mimic the native extracellular environment and attain more intimate contacts with individual neurons, besides allowing for the miniaturization of the electrodes. However, little progress has been done to date on the understanding of the biological impact that such neural interfaces have on neural network maturatio

Spinal cord stimulator implantation with immediate post-operative paraplegia: Case report.

The most common complication of dorsal column spinal cord stimulator implantation is hardware migration. Spinal cord injury following paddle or percutaneous lead implant is rarely reported, with an overall incidence of 0.42%. This report describes a case of immediate post-operative incomplete paraplegia following implantation of one thoracic paddle electrode to address post-laminectomy pain syndrome. Despite emergent removal of the electrode, post-operative corticosteroids, and a course of inpatient rehabilitation, the patient discharged with persistent incomplete paraplegia. Although there is

Twiddler's syndrome in a patient with a deep brain stimulation device for generalized dystonia.

Deep brain stimulation (DBS) is the technique of neurostimulation of deep brain structures for the treatment of conditions such as essential tremor, dystonia, Parkinson's disease and chronic pain syndromes. The procedure uses implanted deep brain stimulation electrodes connected to extension leads and an implantable pulse generator (IPG). Hardware failure related to the DBS procedure is not infrequent, and includes electrode migration and disconnection. We describe a patient who received bilateral globus pallidus internus DBS for dystonia with initially good clinical response, but the device e

Effects of alprazolam on a model of human absences--rhythmic metrazol activity in rats.

Two doses of alprazolam (0.1 and 0.5 mg.kg-1) were tested against a model of human absences--rhythmic EEG activity elicited by low doses of pentylenetetrazol (35 mg.kg-1)--in 10 unrestrained rats with implanted cortical electrodes. Alprazolam delayed the onset of epileptic EEG activity, decreased the number of rhythmic episodes and shortened the total duration of rhythmic activity in a dose-dependent manner. The average duration of episodes of rhythmic activity remained unchanged; other benzodiazepines studied previously were able to influence this measure.

[Interstitial hyperthermia method in spontaneous tumors].

To induce hyperthermia of spontaneous tumors in dogs an interstitial method of heating was used: electrodes in the form of injection needles were implanted around tumors. Necessary temperature levels could not be attained in a dog with circumanal angiosarcoma because the tumor was well vascularized and had a considerable volume. No thermal damage of the animals' skin was observed during all sessions of hyperthermia. A conclusion was made that the interstitial method of heating could be used in multimodality therapy of malignant tumors of a complex shape.

Reduction of epileptiform activity through local valproate-implants in a rat neocortical epilepsy model.

PURPOSE: Pharmacotherapy of epilepsies is limited due to low concentrations at epileptogenic foci, side effects of high systemic doses and that some potentially efficient substances do not pass the blood-brain barrier. To overcome these limitations, we tested the efficacy of local valproate (VPA)-containing polymer implants in a model of necocortical injected tetanus toxin (TeT) in the rat. METHODS: Tetanus toxin was injected intracortically and cobalt (II) chloride (CoCl2) was applied on the cortical surface. Video-electrocorticography recordings with intracortical electrodes were performed.

Auditory nerve fiber excitability for alternative electrode placement in the obstructed human cochlea: electrode insertion in scala vestibuli versus scala tympani.

Objective. The cochlear implant (CI) belongs to the most successful neuro-prostheses. Traditionally, the stimulating electrode arrays are inserted into the scala tympani (ST), the lower cochlear cavity, which enables simple surgical access. However, often deep insertion is blocked, e.g. by ossification, and the auditory nerve fibers (ANFs) of lower frequency regions cannot be stimulated causing severe restrictions in speech understanding. As an alternative, the CI can be inserted into the scala vestibuli (SV), the other upper cochlear cavity.Approach. In this computational study, the excitabil

Intracranial electroencephalographic approaches in the intensive care unit: Safety, feasibility, and coverage.

OBJECTIVE: Nonconvulsive epileptic activity is common after acute brain injury and contributes to neuronal injury and poor outcomes. Although intracranial electroencephalography (iEEG) improves detection compared with surface EEG (suEEG), it currently relies on focal recordings of epileptic dynamics. We prospectively evaluated multielectrode iEEG strategies designed to achieve large-scale cortical and deep brain coverage in critically ill patients. METHODS: In a prospective cohort of adults with acute brain injury requiring invasive neuromonitoring, we implemented four depth electrode-based i

Implantable stimulator for selective stimulation of the common peroneal nerve: a preliminary report.

We present details of the modelling, design, and experimental testing of an implantable system with a monopolar half-cuff electrode for selective stimulation of fibres within certain superficial regions of the human common peroneal nerve which is capable of making a selective activation of muscles, thus contributing to strong dorsal flexion and moderate eversion of the hemiplegic foot. The development of the cuff electrode was based partly on data obtained from histological examination of human common peroneal nerves, and from previously described models of excitation of myelinated nerve fibre

Computer-assisted planning for the insertion of stereoelectroencephalography electrodes for the investigation of drug-resistant focal epilepsy: an external validation study.

OBJECTIVE: One-third of cases of focal epilepsy are drug refractory, and surgery might provide a cure. Seizure-free outcome after surgery depends on the correct identification and resection of the epileptogenic zone. In patients with no visible abnormality on MRI, or in cases in which presurgical evaluation yields discordant data, invasive stereoelectroencephalography (SEEG) recordings might be necessary. SEEG is a procedure in which multiple electrodes are placed stereotactically in key targets within the brain to record interictal and ictal electrophysiological activity. Correlating this act

The feasibility of endoscopic transcanal approach for insertion of various cochlear electrodes: a pilot study.

To determine the feasibility of inserting various types of electrode arrays using an endoscopic transcanal approach into the cochlea via the round window membrane (RWM). All the procedures were performed by the first author and started with a cortical mastoidectomy. A six o'clock vertical incision was made in the meatal skin, and a posterior tympano-meatal flap was elevated transmeatally to expose the middle ear cavity using a rigid 0° endoscope (diameter 3 mm, length 14 cm). The chorda tympani nerve (CTN) and body of the incus were exposed. The RWM was incised, and the electrodes were passed
