Arthroscopic core decompression of the lunate in early stage Kienbock disease of the lunate.

Since the first description of Kienbock disease in 1910, the etiology and treatment have remained controversial topics. Core decompression is an accepted treatment option in early-stage avascular necrosis of the femoral head, and more recently the humeral head. This paper reports the technique of arthroscopic fluoroscopically guided core decompression of the lunate with 6-year results.

Fixation stability of comminuted humeral shaft fractures: locked intramedullary nailing versus plate fixation.

BACKGROUND: This study compared the fixation stability of two treatments for humeral shaft fractures with segmental bone loss during cyclic, physiologic loading. METHODS: Six matched pairs of human humeri received either a 10-hole broad dynamic compression plate or a locked antegrade inserted humeral nail applied to a humeral diaphyseal osteotomy with a 1.5-cm gap defect. The bone-implant humeral constructs were axially loaded for 10,000 cycles at 250 N and 500 N, with measurements of gap displacement and calculation of construct stiffness. The specimens were then loaded to failure. RESULTS:

Subacromial Balloon Spacer Implantation for Massive Irreparable Rotator Cuff Tears Is Associated With Restoration of the Acromiohumeral Interval and Glenohumeral Center of Pressure: A Systematic Review and Meta-Analysis of Controlled Laboratory Studies.

BACKGROUND: Biodegradable subacromial balloon spacers (SBSs) have become increasingly used for the treatment of massive irreparable rotator … specimens were included. At 0° of shoulder abduction, SBS implantation resulted in a mean inferior humeral head translation of 4.80 mm (95% CI, 3.20-6.40; P < .001) relative to the irreparable rotator cuff tear state. This decreased to 4.39 mm and 4.35 mm at 30° and 60° of abduction, respectively. At 0° of abduction, implantation of an SBS was associated with …

Percutaneous fixation of unstable proximal humeral fractures with cannulated screws.

Proximal humeral fractures treated at level I trauma centers are typically displaced, unstable, high-energy injuries associated with injuries to other extremities and closed head injuries. One method that can be used to treat 2-part proximal humeral fractures, if the patient meets certain criteria, is closed reduction and percutaneous cannulated screw fixation. We conducted a retrospective review of 20 consecutive patients treated with this technique, and complete follow-up data were available for 14. Thirteen achieved union and functional range of motion at a mean of 10.5 weeks. Three experie

Current concepts in the surgical management of primary glenohumeral arthritis with a biconcave glenoid.

Glenoid morphology has an important impact on outcomes and complication rates after shoulder arthroplasty for primary glenohumeral arthritis. The B2 glenoid, or a biconcave glenoid with posterior humeral head subluxation, in particular has been associated with a poorer outcome with shoulder arthroplasty compared with other glenoid types. A variety of techniques may be used to address the bone deficiency and instability seen with this glenoid type. Studies suggest that total shoulder arthroplasty may have a reasonable result in the short term but be associated with a high complication rate in t…

Chronic locked posterior shoulder dislocation with severe head involvement.

Arthroplasty is the treatment of choice for severely damaged humeral heads in chronic locked posterior dislocation of the shoulder. We retrospectively reviewed the results of 12 shoulder arthroplasties (11 patients) between 1999 and 2005. Mean follow-up was 37.4 +/- 16.8 months. Mean postoperative Constant score (CS) was 59.4 +/- 21.6 (normative age and gender-related CS 67.1 +/- 24). There was a significant improvement in range of motion for flexion, abduction and external rotation. There was negative correlation (Pearson's coefficient) between the related CS and number of previous operations…

Articular arc length mismatch as a cause of failed bankart repair.

The purpose of this article is to recognize an often overlooked cause of failed anterior stabilization procedures of the shoulder. Hill-Sachs lesions are common occurrences after anterior dislocation. But despite the frequency of occurrence, they are commonly not addressed in the treatment of recurrent anterior dislocation. The Hill-Sachs lesion produces a shortened rotational arc length of the humeral head on the glenoid; any engagement of the defect with the glenoid produces a sensation in the patient of subluxation/dislocation. However, instead of subluxation/dislocation, the sensation may …

Open reduction and internal fixation of three- and four-part fractures of the proximal humerus.

Thirty-one patients, ranging in age from 19 to 62 years (average, 55 years), were evaluated an average of six years seven months (range, one to 12 years) after open reduction and internal fixation of a three- or four-part fracture of the proximal humerus. Six patients had an associated dislocation of the humeral head. A modified cloverleaf plate was used in 26 cases and an AO T-plate in five cases. All fractures healed. No avascular necrosis of the humeral head was observed at follow-up examination. Twenty-three patients had an excellent result, two had a good result, and six had a fair result…

The chondral print sign: what does it really mean?

BACKGROUND: The chondral print (CP) sign is a chondral change on the humeral head underneath the long head of the biceps (LHB) tendon. Several suggested causative links have been described, but the pathologic mechanism remains unclear. METHODS: We designed this prospective cohort association study of 102 consecutive shoulder arthroscopies to investigate proposed associations of CP with LHB, rotator cuff, labral pathology, and other chondral lesions. Data collection was by a specifically designed pro forma, and statistical analysis was performed. RESULTS: We identified 24 patients (23.5%) wit…

Open reduction and internal fixation of three and four-part fractures of the proximal part of the humerus.

BACKGROUND: Controversy persists concerning the preferred treatment of displaced fractures of the proximal part of the humerus. The present study was undertaken to evaluate the results of open reduction and internal fixation of three and four-part fractures of the proximal part of the humerus and the functional limitations of patients in whom avascular necrosis of the humeral head develops as a complication of this fracture. METHODS: We assessed the intermediate and long-term results for sixty patients with a three or four-part fracture of the proximal part of the humerus who had undergone op…

Superior Capsular Reconstruction Technique for Massive Rotator Cuff Tear With Arthroscopic Transfer and Augmentation of Long Head of Biceps Brachii Tendon-The Biobridge Technique.

Massive irreparable rotator cuff tears remain a major surgical challenge, often leading to pain, functional loss, and superior humeral migration. Superior capsular reconstruction has emerged as a technique to restore superior stability. Autologous augmentation using the long head of the biceps brachii tendon offers a cost-effective, biologically viable option, avoiding graft harvest and morbidity. We describe an all-arthroscopic technique of superior capsular reconstruction with long head of the biceps brachii tendon transposition and augmentation called Biobridge technique. The method involve

Tension band sutures using a washer for a proximal humerus fracture.

To prevent distraction and varus deformity between the humeral head and shaft, tension band sutures placed between the head of the interlocking screw and the rotator cuff, and we recommend using nonabsorbable sutures. We describe our simple procedure to overcome these difficulties in tension band suturing after reducing a proximal humerus fracture to maintain the reduction.
