BACKGROUND: Rituximab (RTX) represents an established therapeutic option for myasthenia gravis refractory to conventional immunotherapy. However, its heterogeneous efficacy across populations creates a challenge in clinical application, necessitating the development of robust predictive models to better identify patients likely to derive meaningful benefit from this treatment.
METHODS: This study retrospectively reviewed the patients who visited the research center and received RTX treatment between August 2019 and January 2025. The primary outcome measure was minimal symptom expression (MSE)…

Juvenile autoimmune myasthenia gravis (JAMG) is uncommon in children and extremely rare in infants. The condition is frequently recognised late since it mimics congenital and neuromuscular disorders. We report a case of a toddler presenting with fluctuating bilateral ptosis that worsened in the evening. The bedside ice pack test was positive, and serum acetylcholine receptor antibodies confirmed the diagnosis. A chest CT scan revealed no abnormalities in the thymus. The child showed a good response to anticholinesterase treatment. This case emphasises the significance of recognising fatigable …

Increased disease awareness and advanced serological tests have improved myasthenia gravis (MG) diagnosis. However, the risk of misdiagnosis remains. The aim of this study was to evaluate the rate and causes of MG misdiagnosis in a tertiary MG clinic. We analyzed medical records from patients diagnosed with MG who were referred to our tertiary clinic for a second opinion between 2019 and 2024. Of the 144 patients originally included in the analysis, 15 were excluded due to insufficient data. MG diagnosis was not confirmed in 26/129 patients (20.2%). None of these 26 patients had clinical sympt…

BACKGROUND: Myasthenia gravis (MG) is an immune-mediated disease with a bimodal incidence: early-onset (< 50 years, predominantly female) and late-onset (> 50 years, predominantly male). Recently, a subgroup with very late-onset (≥ 65 years) has been described; however, its clinical relevance concerning disease course and prognosis remains uncertain.
OBJECTIVE: To evaluate the potential clinical implications of very late presentations of MG.
METHODS: The present is a retrospective cohort study comprising all patients diagnosed with MG currently followed up in the Neuromuscular Disorders Unit…

BACKGROUND: Treatment value is traditionally assessed through clinical benefits, health-related quality of life, and healthcare utilization. However, rare diseases such as generalized myasthenia gravis (gMG) that heavily affect families and/or caregivers require a more holistic approach that centers around impacted individuals across all areas of daily life. Here, key domains impacted by gMG were identified through individual interviews with both patients and caregivers and these impacts were further assessed through a survey in an independent sample.
METHODS: Interviews with patients with gM…

BACKGROUND: The management of patients with generalized myasthenia gravis (gMG) is complex and often involves substantial health care expenses. As the treatment paradigm continues to evolve, understanding the economic burden of gMG must also reflect the availability and increasing use of newly approved and advanced therapies in a diverse patient population.
OBJECTIVE: To estimate incremental health care resource utilization (HRU) and costs associated with gMG compared with non-MG controls and identify predictors of high health care costs in gMG in an insured US population cohort.
METHODS: Ad…

Myasthenia gravis is an antibody-mediated disorder of the neuromuscular junction. Fatigability and fluctuations are hallmarks of the disease, posing a challenge: evaluation through a single physical examination may not reflect disease control. A shift towards patient-reported Outcomes is a new paradigm in the current practice, and these outcomes are usually assessed through validated scales. In the present paper, we discuss some of the most used scales in the clinical practice, such as the Myasthenia Gravis Foundation of America (MGFA) scale, the Myasthenia Gravis Activities of Daily Living (M…

Muscle-specific tyrosine kinase antibody-positive myasthenia gravis (MuSK-MG), a subtype of myasthenia gravis (MG), follows an acute and severe disease course with suboptimal responses to standard treatments. Ofatumumab is a fully humanized anti-CD20 monoclonal antibody that induces potent and sustained B-cell depletion. Nonetheless, there are few reports on its use in sequential therapy failure after the failure of conventional drug treatment in MuSK-MG patients, especially in the Asian population. We report a 32-year-old female patient with anti-MuSK antibody-positive myasthenia gravis who w…

BACKGROUND: While immune checkpoint inhibitors (ICIs) have revolutionized cancer treatment, they can trigger diverse immune-related adverse events (irAEs). Among these, ICI-related myocarditis, myositis and myasthenia gravis (MMM) overlap syndrome (ICI-MMM) is a rare but potentially fatal complication. Conventional immunotherapy often exhibits limited efficacy against ICI-MMM, which is associated with high mortality rates. Thus, there is an urgent need for novel and effective strategies to mitigate its life-threatening outcomes.
METHOD: We conducted a retrospective analysis of the successful …

BACKGROUND: Generalized myasthenia gravis (gMG) is an autoimmune disorder that is marked by fluctuating muscle weakness and poor health-related quality of life (HRQoL). This pre-specified analysis of the phase 3 Vivacity-MG3 study (NCT04951622) assessed the effect of nipocalimab (fully human anti-FcRn monoclonal antibody) on HRQoL in patients with gMG.
METHODS: Seropositive adults (anti-AChR/anti-MuSK/anti-LRP4) with gMG were randomized (1:1) to intravenous nipocalimab (30 mg/kg loading, followed by 15 mg/kg every 2 weeks) or placebo, in addition to standard-of-care (SOC) for 24 weeks. The My…

Immune checkpoint inhibitor-associated myocarditis, myositis, and myasthenia gravis overlap syndrome (IM3OS) is an uncommon immune-related adverse event (irAE) associated with the use of immune checkpoint inhibitors for the treatment of malignancies. We present a case of a 77-year-old woman who received anti-PD1 treatment for high-grade invasive urothelial carcinoma, and subsequently presented with weakness, ptosis, and dysarthria concerning for myositis, myocarditis, and myasthenia gravis overlap syndrome. She was successfully treated with high-dose glucocorticoids, mycophenolate mofetil, and…

INTRODUCTION/AIMS: The burden of myasthenia gravis (MG) is often underestimated, and studies usually focus on the symptom burden. However, treatment-related adverse events also contribute to patients' burdens and affect their treatment decisions. Understanding the treatment preferences of people with MG will help inform patients, clinicians, and policy makers. We conducted a qualitative study to understand the most relevant factors that people with MG consider when making treatment decisions.
METHODS: Fifteen patients with a wide range of MG severity participated in semi-structured interviews…

A cure for myasthenia gravis is necessary to achieve "A World Without Myasthenia Gravis". Current treatments reduce symptom burden and broadly suppress immune activity, but generally need to be administered chronically and are associated with significant risks, incomplete remission rates, infrequent treatment-free remission, and potential disability. We define a cure for MG as complete remission of MG, without the need for ongoing treatment, plus desirably: a high probability of success; a low risk of later relapse; the possibility of treatment repetition if MG does relapse; low and proportion…

Mindfulness-based interventions (MBIs) are increasingly studied for clinical conditions, with evidence pointing toward their benefits for chronic and autoimmune disorders. Research shows the biological effect of MBIs on immunological pathways and benefits for fatigue, mood, and quality of life in chronic conditions. These raise the possibility of benefits in myasthenia gravis (MG). However, there is a gap in research on MBIs for MG. This paper makes the case for MG research to study the potential benefits, mechanisms, and special considerations when designing MBIs for people living with MG. Su…

INTRODUCTION/AIMS: Muscle-specific kinase myasthenia gravis (MuSK-MG) is an autoimmune neuromuscular disorder predominantly mediated by IgG4 autoantibodies disrupting MuSK signaling. The contribution of other isotypes remains incompletely defined. We characterized the serological profile of a Greek cohort of MuSK-MG patients.
METHODS: Baseline (n = 140) and longitudinal (available n = 99) samples from 140 patients, positive for anti-MuSK by radioimmunoprecipitation (RIPA) were analyzed using a live cell-based assay (L-CBA) to detect total IgG, IgG1-4 subclasses, IgM, and IgA. Also, disease an…

INTRODUCTION: Generalized myasthenia gravis (gMG) is a chronic autoimmune disorder that causes muscle weakness. This human factors (HF) validation study aimed to demonstrate that use of a prefilled syringe with needle safety device (PFS-SD) or autoinjector (AI) is safe and effective for patients with gMG, caregivers, and healthcare professionals (HCPs) to administer injectable medication in the intended use environments without preventable use errors.
METHODS: Patients with gMG, caregivers, and HCPs were randomized to the PFS-SD or AI. No training on either device use was provided. The safety…

Fatigue is a common, often disabling symptom in myasthenia gravis (MG), distinct from muscle fatigability, and strongly associated with reduced quality of life. This narrative review examines current evidence on fatigue in MG, its patient impact, and future research directions. Earlier studies, mostly small and heterogeneous, reported a highly variable prevalence of fatigue. Recent large registry-based studies have now established a more consistent prevalence around 60% and confirmed associations with disease severity, depressive symptoms, and physical inactivity. Fatigue has also been assesse…
