Cross-sectional study of neck pain and cervical sagittal alignment in air force pilots.

BACKGROUND: There is a high prevalence of neck pain in air force pilots; however, the causes are not clear and are considered work-related. Kyphotic changes in the cervical spine have been known to cause neck pain. In this study, we investigated the association between neck pain and cervical kyphosis in air force pilots. METHODS: This is a cross-sectional study of 63 Republic of South Korea Air Force pilots. We examined the C2-7 absolute rotation angle (ARA) using the posterior tangent method and other radiologic parameters on whole spine lateral radiographs. We divided the participants into …

Low Level Laser Therapy (LLLT) for Neck Pain: A Systematic Review and Meta-Regression.

PURPOSE: This systematic review update evaluated low level laser therapy (LLLT) for adults with neck pain. METHODS: Computerized searches (root up to Feb 2012) included pain, function/disability, quality of life (QoL) and global perceived effect (GPE). GRADE, effect-sizes, heterogeneity and meta-regression were assessed. RESULTS: Of 17 trials, 10 demonstrated high risk of bias. For chronic neck pain, there was moderate quality evidence (2 trials, 109 participants) supporting LLLT over placebo to improve pain/disability/QoL/GPE up to intermediate-term (IT). For acute radiculopathy, cervical o…

Continuous intracisternal and high cervical intrathecal bupivacaine analgesia in refractory head and neck pain.

BACKGROUND: The upper cervical component of the spinomesencephalic tract and cranial nerves V, VII (nervus intermedius), IX, and X are involved in mechanisms of acute and chronic pain from head and neck structures. To date there is no reliable method for relief of refractory pain (i.e., pain that cannot be relieved by conventional pharmacologic therapies) from these structures. Therefore, we explored continuous intracisternal infusion of bupivacaine for the treatment of refractory pain of the head and neck. METHODS: Intracisternal catheters were inserted in 13 adults with refractory nonmalign

When neck pain isn't muscular: A case report on TIPIC syndrome responding to NSAIDs.

Transient Perivascular Inflammation of the Carotid Artery (TIPIC) syndrome is an infrequent and often underdiagnosed etiology of acute unilateral neck pain. Characterized by localized inflammation of the carotid artery and its surrounding tissues, TIPIC syndrome typically presents with tenderness over the carotid bifurcation. The condition is self-limiting and generally responds well to nonsteroidal anti-inflammatory drugs. This report discusses a 24-year-old male with sudden-onset right-sided neck pain, diagnosed with TIPIC syndrome through imaging studies, and effectively managed with nonste…

Klippel-Feil syndrome revealed by post-traumatic neck pain: Case report and literature review.

Klippel-Feil syndrome (KFS) is a congenital anomaly involving the fusion of two or more cervical vertebrae. It is characterized by a low hairline at the nape of the neck, a short neck, and a limited range of motion. Here, we present the case of a 10-year-old girl who suffered brain trauma in a motor vehicle accident and subsequently experienced persistent occipital headaches and dizziness. A brain computed tomography scan performed to evaluate these symptoms revealed incomplete fusion of two cervical vertebrae (C2-C3), leading to a diagnosis of KFS. Magnetic resonance imaging of the spine was

Altered brain dynamics in chronic neck and shoulder pain revealed by hidden Markov model.

Chronic neck and shoulder pain (CNSP) is the most common clinical symptom of cervical spondylosis, which not only greatly affects individuals' quality of life but also places a significant burden on social healthcare systems. Existing analgesic treatments are often associated with significant adverse effects and limited efficacy. Recently, non-invasive neuromodulation techniques have shown promise, but the central mechanisms underlying chronic pain remain poorly understood. Recent advances in resting-state functional magnetic resonance imaging (rs-fMRI) have highlighted altered brain connectiv

Is sleep bruxism associated with smartphone use, neck pain, and sleep features among adolescents?

An association between bruxism and musculoskeletal disorders, such as neck pain, has been established. This study investigated the association of possible sleep bruxism (PSB) activities, including grinding, bracing, and thrusting, with smartphone use, smartphone-induced neck pain, and sleep features. This cross-sectional study involved 403 Brazilian adolescents aged 11 to 19 years. A self-administered questionnaire was used to assess the severity of PSB, smartphone use, smartphone-induced neck pain, and sleep features (sleep duration and quality and history of nightmares). Sociodemographic fac…

Stretching and muscle-performance exercises for chronic nonspecific neck pain: who may benefit most?

BACKGROUND: Although exercise is the mainstay of treatment for neck pain (NP), uncertainty remains over optimal decision-making concerning who may benefit most from such, particularly in the long term. OBJECTIVE: To identify the subgroup of patients with nonspecific NP most likely to benefit from stretching and muscle-performance exercises. METHODS: This was a secondary analysis of treatment outcomes of 70 patients (10 of whom dropped out) with a primary complaint of nonspecific NP in one treatment arm of a prospective, randomized, controlled trial. All patients performed the exercises, twic…

Intractable hiccups and neck pain due to left C4 radiculopathy decreased with posterior foraminotomy.

BACKGROUND: Intractable hiccups can last for more than 1-2 months and can occur, as shown in this case study, due to cervical nerve root compression. CASE DESCRIPTION: A 76-year-old male presented with intractable hiccups and neck pain of 7 months' duration. The patient underwent magnetic resonance imaging studies of the entire neuraxis. The only abnormality found was on the cervical magnetic resonance images that demonstrated left C4 nerve root compression due to the C3- C4 lateral/foraminal osteophyte. Following a left-sided C3-C4 laminoforaminotomy, the hiccups and the neck pain improved. …

Correlation between cervical lordosis and cervical disc herniation in young patients with neck pain.

Abnormal cervical curvature and cervical disc herniation are closely related to neck pain and should be taken into account before any treatment. However, studies have rarely reported on the correlation between cervical lordosis and cervical disc herniation in patients with neck pain. Therefore, in this study, we collect young neck pain patients with abnormal cervical curvature to evaluate the relationship between cervical lordosis and cervical disc herniation.Three hundred patients below 40 years old with neck pain were enrolled. Patient sex, age, apical vertebra, segment of intervertebral dis…

Is Scapular Stabilization Exercise Effective for Managing Nonspecific Chronic Neck Pain?: A Systematic Review.

Scapular stabilization is thought to have an important role in improving pain and dysfunction around the neck and shoulders, but evidence of this is lacking. We aim to systematically review the effect of a scapular stabilization exercise (SSE) on pain and dysfunction in patients with nonspecific chronic neck pain (NP). We searched the PubMed, EMBASE, CINAHL, and Cochrane Library databases using the terms (NP [MeSH] OR NP OR cervical pain OR neck ache OR cervicalgia) AND (scapular exercise OR periscapular exercise OR SSEs). We included suitable studies that met the study's inclusion criteria. A…
