[Classification of local lead electromyograms].

The authors maintain that the classification of the electromyograms suggested by F. Buchthal in 1957 has become obsolete in view of an ever-growing number of electromyographic investigations that employ needle electrode leads. The main shortcoming of this classification lies in the fact that it does not allow the researchers to differentiate and evaluate the motor unit activity expressed as volley bursts of oscillations, which can be of special importance in diagnosing radiculitides, parkinsonism, neuromuscular apparatus lesions in thyrotoxicosis, and other disorders. In addition, electromyogr

A Comment About Methodological Limitations on the Study by An et al.

The study by An et al. provides valuable insights into rocuronium pharmacodynamics in patients with high body fat percentages (PBF). However, the lack of normalisation for acceleromyographic train-of-four (TOF) ratios raises concerns about the validity of endpoints. Due to the acceleromygraph's inherent "inverse fade" artefact, non-normalised TOF ratios may mask clinically significant residual paralysis. For example, a reported TOF of 0.9 could correspond to a true ratio of 0.75 (assuming a baseline of 1.2), overestimating recovery. This methodological gap weakens the study's conclusion on fat

Anesthesia for excision of vestibular schwannoma.

Anesthesia for the resection of vestibular schwannomas poses unique challenges; operative times are long, typically between 8 and 12h, during which there are varying levels of surgical stimulation. Close attention to positioning is essential to minimize skin damage, neuropraxia, and optimize surgical access. The delicate nature of microsurgical resection requires absolute patient immobility; this is made more demanding by the need to avoid neuromuscular blocking drugs to facilitate continuous electromyographic (EMG) monitoring of the facial nerve. The choice of anesthetic should enable this an

Basketball: Biomechanics and Imaging Findings of Common Injuries.

Basketball is a high-intensity sport associated with a wide range of musculoskeletal injuries, most commonly involving the lower extremities. These injuries frequently result from high-impact loading, rapid directional changes, and biomechanical inefficiencies such as poor neuromuscular control and abnormal movement patterns. Although acute injuries often occur through noncontact mechanisms during play, direct contact injuries have become more common over the years. Overuse injuries are also frequent in basketball and may develop silently and progress over time. Imaging plays a vital role in d

Independence in activities of daily living was achieved using aerobic exercise without overwork weakness during rehabilitation: a case report of Lambert-Eaton myasthenic syndrome.

[Purpose] Lambert-Eaton myasthenic syndrome (LEMS) is an autoimmune disease characterized by decreased transmitter secretion from neuromuscular junctions and nerve terminals. Such cases require physical therapy for independently performing daily activities; however, care must be taken to avoid overwork weakness. This study aimed to investigate the effects of aerobic exercise-based physical therapy in patients with LEMS. [Participants and Methods] We report a case of LEMS with decreased muscle endurance due to inactivity. The participant was subjected to physical therapy with an exercise modali

Proteomic identification of biomarkers of skeletal muscle disorders.

Disease-specific biomarkers play a central diagnostic and therapeutic role in muscle pathology. Serum levels of a variety of muscle-derived enzymes are routinely used for the detection of muscle damage in diagnostic procedures, as well as for the monitoring of physical training status in sports medicine. Over the last few years, the systematic application of mass spectrometry-based proteomics for studying skeletal muscle degeneration has greatly expanded the range of muscle biomarkers, including new fiber-associated proteins involved in muscle transformation, muscular atrophy, muscular dystrop

[Electrophysiological examination of the cranial nerves: technical aspects and practical applications].

Electrophysiological investigations of cranial nerves have long been limited to conventional electromyography, but recently have undergone new developments. Brainstem reflex testing is now well defined with correlations to imaging data. Brainstem reflexes can be tested in trigeminal and facial nerve diseases, but also in the case of intra-axial lesions. Surface neurography has developed to quantify troncular nerve responses, abnormal facial nerve responses or assessment of neuromuscular junction function through repetitive stimulation. Transcranial magnetic or electrical stimulation allows to

B Cell Immunophenotyping and Transcriptional Profiles of Memory B Cells in Patients with Myasthenia Gravis.

Myasthenia gravis (MG) is an autoimmune neuromuscular junction disorders mediated by various autoantibodies. Although most patients with MG require chronic immunosuppressive treatment to control disease activity, appropriate surveillance biomarkers that monitor disease activity or potential toxicity of immunosuppressants are yet to be developed. Herein, we investigated quantitative distribution of peripheral blood B cell subsets and transcriptional profiles of memory B cells (CD19+ CD27+) in several subgroups of MG patients classified according to the Myasthenia Gravis Foundation of America (M

Acute myopathy during treatment of status asthmaticus with corticosteroids and steroidal muscle relaxants.

Acute myopathy in patients being treated for severe asthma has been recognized with increasing frequency since first being described in 1977. We report three patients treated for status asthmaticus who developed severe generalized weakness. Electrophysiologic studies and muscle biopsy revealed evidence of muscle destruction. Each of these patients was treated with high-dose corticosteroids and underwent prolonged neuromuscular blockade with a steroidal muscle relaxant. A review of the literature revealed 15 similar cases. We postulate that the combined effects of corticosteroids and muscle rel

Time-of-Day Variation in Knee Extensor Strength and Fatigue Is Associated With Circadian Phenotype in Healthy Young Adults.

Time-of-day variation in neuromuscular performance is well established, but the contribution of circadian phenotype remains unclear. This study examined whether maximal knee-extensor strength and fatigue resistance differ between morning and evening and whether chronotype and actigraphy-derived sleep-timing are associated with diurnal performance variation. Forty-four healthy young adults (21 males, 23 females) completed a randomised-crossover protocol consisting of neuromuscular testing in the morning (0700-0900) and evening (1700-1900). Participants completed 7 days of actigraphy monitoring

An Upper Limb Muscle Fatigue Detection Approach for Overhead Work Using Interface Pressure Complexity Measurement.

The continuous static contraction of the upper limb during overhead work often leads to progressive muscle fatigue, which is the main cause of work-related musculoskeletal disorders (WMSDs) and reduced operational safety. Traditional fatigue detection methods cannot capture subtle early fatigue characteristics under dynamic working conditions, and sensors are difficult to conveniently deploy in harsh environments. This study constructs an upper limb fatigue detection model based on wearable interface pressure signals and proposes a spatiotemporal dual-entropy fusion strategy. Sample entropy ev

Biomarkers of Spinal and Bulbar Muscle Atrophy (SBMA): A Comprehensive Review.

Spinal and bulbar muscular atrophy (SBMA), also known as Kennedy's disease, is a rare, X-linked, late onset neuromuscular disorder. The disease is caused by a CAG trinucleotide repeat expansion in the first exon of the androgen receptor gene. It is characterized by slowly progressive lower motor neurons degeneration, primary myopathy and widespread multisystem involvement. Respiratory involvement is rare, and the condition is associated with a normal life expectancy. Despite a plethora of therapeutic studies in mouse models, no effective disease-modifying therapy has been licensed for clinical
