Functional Results of Unstable (Type 2) Distal Clavicle Fractures Treated with Superior Anterior Locking Plate.

BACKGROUND: The treatment of distal clavicle fracture is always a challenge, as it is mostly unstable and has higher rate of delayed union, malunion, non-union and associated acromioclavicular arthritis. So the management of these fractures remains controversial. The purpose of this study is to evaluate the functional results of Type 2 distal end clavicle fractures treated with superior anterior locking plate. METHODS: From June 2011 to August 2015 a retrospective study of12 male patients (mean age of 41.3 years) 11 with unilateral and 1 with bilateral distal clavicle fractures treated with s

Arthroscopic tuberoplasty for subacromial impingement secondary to proximal humeral malunion.

Subacromial impingement secondary to malunion of proximally displaced greater tuberosity fractures may be treated with arthroscopic acromioplasty in patients with slight displacements, but open anatomical repositioning of the malunited fragment is recommended when more severe deformities exist. This deformity may also be addressed by abrading the protruding proximal portion of the greater tuberosity (tuberoplasty). However, this procedure would require full elevation of the rotator cuff insertion if performed with open surgery. This article reports a technique of arthroscopic tuberoplasty base

Preoperative anti-remodeling osteoporotic therapy is associated with increased periprosthetic humeral fracture risk.

BACKGROUND: Osteoporosis increases fracture risk and adversely affects outcomes after shoulder arthroplasty. Preoperative bisphosphonate use has been linked to higher rates of periprosthetic fracture after lower extremity total joint arthroplasty, but data in shoulder arthroplasty are limited. The purpose of this study was to evaluate whether anti-remodeling agents such as bisphosphonates or anabolic osteoporotic therapy influenced 1-year outcomes, including periprosthetic humeral fracture, acromial stress fracture, and all-cause revision, following anatomic total shoulder arthroplasty (aTSA)

A Case Report of an Unusual Case of Tuberculous Osteomyelitis Causing Spontaneous Pathological Fracture of Humerus in a Middle Aged Female.

INTRODUCTION: Tuberculosis is a major health problem worldwide. Extrapulmonary tuberculosis is often secondary to some primary foci in lungs. There are reports of tuberculous osteomyelitis involving maxilla, ulna, femur, and shoulder joint but none have reported pathological fracture in humeral diaphysis due to tuberculosis osteomyelitis without shoulder joint involvement. We report a case of pathological fracture of humerus diaphysis due to tuberculous osteomyelitis with normal articular space. We noticed favorable outcome following surgery and antitubercular drugs. CASE REPORT: A 62-year-ol

Agricultural Rotavator Induced Polytrauma in a 11-years-old Child - A Case Report.

INTRODUCTION: Bilateral scapular fractures are a very rare injury with only few cases reported in the literature. They are said to be consequence of a violent trauma to the upper part of shoulder or as a result of electrocution or seizures. Glenoid involvement or open scapular fractures are one of the indications for operative treatment in these injuries, in other cases, simple immobilization is sufficient to obtain good results. Here, we report a case of a bilateral scapular fracture following an epileptic seizure, discussing the mechanism and outcome of conservative treatment. CASE REPORT:

Short-term Clinical Outcome of Dual Plate Fixation in the Treatment of Proximal Humerus Fractures with Calcar Comminution.

OBJECTIVE: Calcar comminution has been considered to be the main cause of the failure of internal fixation and fracture nonunion in proximal humerus surgery. Anatomical reduction and increasing the strength of internal fixation is the key to success. The purpose of this study was to investigate the short-term clinical effect of dual plate fixation in the treatment of proximal humeral fractures with calcar comminution. METHODS: The data of 37 patients with proximal humeral fractures with calcar comminution, treated in our departments from July 2018 to April 2020, were retrospectively analyzed.

Shoulder injuries from birth to old age: A 1-year prospective study of 3031 shoulder injuries in an urban population.

INTRODUCTION: Shoulder injuries are commonly encountered in emergency departments. In spite of this, the epidemiology is only partly known, and soft tissue injuries in particular remain unclear. The aim of this study was to obtain an overview of shoulder injuries in a general population cohort, and to estimate the relative proportion of the main injury categories soft tissue injuries, fractures and dislocations, as well as their variation with age and gender. PATIENTS AND METHODS: We registered prospectively all patients admitted with a suspected shoulder injury at a combined casualty and pri

[Treatment of intra-articular fractures of the distal meta-epiphysis of the humerus].

Medical rehabilitation was fulfilled in 75 patients with intra-articular fractures of the distal metaepiphysis of the shoulder bone. The authors came to the conclusion that the main methods of treatment of fresh intra-articular fractures of the distal metaepiphysis of the shoulder bone consisted in conservative measures (a single reposition, skeletal extension and compression-distractional osteosynthesis). In cases of the open reposition followed by osteosynthesis a sparing technique of surgical intervention should be used.

Osteonecrosis of the Humeral Head Following Anterior Shoulder Dislocation in a Dialysis-Dependent Patient: A Case Report.

CASE: A 75-year-old woman on long-term hemodialysis sustained anterior shoulder dislocation without a fracture. Four months later, she presented with persistent shoulder pain and limited motion. Imaging revealed humeral head osteonecrosis (Cruess stage V) and a massive rotator cuff tear. Reverse shoulder arthroplasty was performed with favorable outcome. CONCLUSION: Osteonecrosis of the humeral head following anterior shoulder dislocation without a fracture is rare. To our knowledge, this is the first reported case in a dialysis-dependent patient. This case highlights the need for increased c

A Comprehensive Radiologic Review of Shoulder Girdle Trauma.

Radiologic knowledge of different fracture patterns involving the shoulder girdle is an important tool to generate clinically relevant reports, identify concomitant injuries, guide management decisions, and predict and minimize complications, such as nonunion, osteoarthritis, osteonecrosis, and hardware failure. Complex unstable injuries like scapulothoracic dissociation can also occur because of shoulder girdle trauma. Management options may vary from conservative to surgical, depending on the fracture type and patient factors. Injuries around the shoulder girdle can involve the glenohumeral

Long-term Results, Functional Outcomes and Complications after Open Reduction and Internal Fixation of Neglected and Displaced Greater Tuberosity of Humerus Fractures.

BACKGROUND: Humerus fractures include 5% to 8% of total fractures. Non-union and delayed union of GT (GT) fractures is uncommon; however they present a challenge to the orthopedic surgeons. Significant controversy surrounds optimal treatment of neglected fractures. The purpose of this article was to perform a comparative study to evaluate the outcomes of open reduction and internal fixation (ORIF) of neglected GT fractures. METHODS: We retrospectively evaluated the results of surgical intervention in 12 patients with displaced nonunion of GT fractures who were referred to our center. Before a

Isolated tuberosity fractures of the proximal humeral: current concepts.

Despite the relatively common occurrence of fractures of the proximal humeral amongst the elderly, the subgroup of isolated greater and lesser tuberosity fractures have remained less well understood. While the majority of two-part fractures result from a standing-height fall onto an outstretched hand, isolated tuberosity fractures are also commonly associated with glenohumeral dislocations or direct impact to the shoulder region. Inasmuch as isolated greater tuberosity fractures are considered uncommon, isolated lesser tuberosity fractures are generally considered exceedingly rare. Non-operati
