Remodeling process and clinical outcomes following all-arthroscopic modified Eden-Hybinette procedure using iliac crest autograft and 1-tunnel double Endobutton fixation system.

BACKGROUND: Arthroscopically modified Eden-Hybinette procedures for glenohumeral stabilization have been used for a long time. With the advancement of arthroscopic techniques and the development of sophisticated instruments, a double Endobutton fixation system has been used clinically to secure bone graft to the glenoid rim placed through a specifically designed guide. The purpose of this report was to evaluate clinical outcomes and serial glenoid remodeling process following all-arthroscopic anatomical glenoid reconstruction using autologous iliac crest bone grafting technique through 1-tunne

Posterior shoulder instability managed by arthroscopic acromial pediculated bone-block. Technique.

UNLABELLED: In posterior shoulder instability (recurrent dislocation, involuntary posterior subluxation or voluntary subluxation that has become involuntary), surgery may be considered in case of failure of functional treatment if there are no psychological contraindications. Acromial bone-block with pediculated deltoid flap, as described by Kouvalchouk, is an alternative to iliac bone-block, enabling triple shoulder locking by the blocking effect, the retention hammock provided by the deltoid flap and posterior capsule repair. Arthroscopy allows shoulder joint exploration and diagnosis of ass…

Upper limb joint kinetic analysis during tennis serve: Assessment of competitive level on efficiency and injury risks.

The aim of this work was to compare the joint kinetics and stroke production efficiency for the shoulder, elbow, and wrist during the serve between professionals and advanced tennis players and to discuss their potential relationship with given overuse injuries. Eleven professional and seven advanced tennis players were studied with an optoelectronic motion analysis system while performing serves. Normalized peak kinetic values of the shoulder, elbow, and wrist joints were calculated using inverse dynamics. To measure serve efficiency, all normalized peak kinetic values were divided by ball ve

Shoulder instability. Diagnosis and management.

BACKGROUND: The shoulder is the most commonly dislocated joint in the body. The most frequent complication of shoulder dislocation is recurrence, occurring most often in young athletes. OBJECTIVE: To summarise the functional anatomy of the shoulder joint, the structures damaged following shoulder dislocation and the methods to evaluate and treat shoulder instability. DISCUSSION: Clinical history is effective for diagnosing obvious shoulder instability. The O'Brien's sign (for superior labral lesions), the sulcus sign (for capsular laxity) and the apprehension sign (for anterior instability) …

Role of joint interactions in upper limb joint movements: a disability simulation study using wearable inertial sensors for 3D motion capture.

BACKGROUND: Restriction of movement at a joint due to disease or dysfunction can alter the range of motion (ROM) at other joints due to joint interactions. In this paper, we quantify the extent to which joint restrictions impact upper limb joint movements by conducting a disability simulation study that used wearable inertial sensors for three-dimensional (3D) motion capture. METHODS: We employed the Wearable Inertial Sensors for Exergames (WISE) system for assessing the ROM at the shoulder (flexion-extension, abduction-adduction, and internal-external rotation), elbow (flexion-extension), an

Human cadaveric study of subscapularis muscle innervation and guidelines to prevent denervation.

The upper and lower subscapular nerves provide innervation to the subscapularis muscle. However, the axillary nerve may provide a significant innervation to the lower portion of the muscle. The prevalence and patterns of anomalous innervation of the subscapularis muscle were studied to determine if these variations increased the risk of muscle denervation during open shoulder surgery. Twenty human cadaveric shoulders were dissected, and the innervation to the subscapularis was defined. The distance from the nerve insertion to the shoulder joint was measured in neutral and maximal external rota…

A Case of Axillary Web Syndrome Caused by Venous Blood Sampling.

Axillary web syndrome (AWS) occurs after breast cancer surgery, sentinel lymph node dissection, or sentinel lymph node biopsy. Here, cord-like structures from the axilla to the forearm limit the range of motion of the shoulder joint and cause pain. Although the etiology is unknown, AWS has been attributed to the blockage of normal lymphatic flow. Here, we report a novel case of AWS after venous blood sampling in a patient. A healthy, 31-year-old male patient experienced pain with a limited range of motion of the shoulder joint the day after venous blood was collected from the left upper extrem…

Evaluation and management of neuropathic arthropathy of the shoulder: narrative review.

Neuropathic arthropathy of the shoulder, or Charcot shoulder, is a rare and destructive neuroarthropathy resulting from impaired protective sensation and altered joint mechanics that is most commonly associated with syringomyelia. The pathogenesis is believed to involve both neurotraumatic and neurovascular mechanisms, resulting in progressive osseous destruction, joint instability, and soft tissue compromise. Clinical presentation varies, with painless swelling, joint effusion, crepitus, and progressive limitation in shoulder function commonly reported. Radiographic findings frequently demons

Current Approaches to Arthroscopic Management of Anterior Shoulder Instability.

Operative management should be considered from the initial episode of anterior shoulder instability to prevent bone loss, minimize risk of recurrence, maximize the potential for return to sport at the preinjury level, and from a cost-effectiveness standpoint. The glenoid track concept will be examined as a crucial factor in determining whether bone loss is significant. The increasingly comprehensive understanding of the dynamic interactions between bone loss on the humeral head and glenoid rim will be further discussed as a key factor influencing both surgical management and outcomes. The indi

Posterior Shoulder Instability: A Clinical Review.

Posterior shoulder instability has emerged as an increasingly prominent concern for surgeons treating shoulder pathology. Previously estimated to account for only 5% to 10% of cases in the general population, it is now recognized that the true incidence is likely underestimated. This growing recognition highlights the need for tailored treatment strategies. Surgical patients without osseous involvement can achieve successful outcomes with arthroscopic labral repair, while those with glenoid bone loss exceeding 13.5% may benefit from procedures designed to restore glenoid bone width.

Morphometric and Biomechanical Comparison of the Scapular Spine and Coracoid Graft in Anterior Shoulder Instability.

BACKGROUND: The scapular spine has been described as a relatively new bone graft alternative used in glenoid augmentation. The classic Latarjet procedure, which transfers the coracoid as a graft, is regarded as the gold standard. The comparison of these 2 techniques has not been fully reported. PURPOSE: To compare the anatomic and biomechanical properties of glenoid augmentation using scapular spine graft or coracoid graft. STUDY DESIGN: Controlled laboratory study. METHODS: The study used 20 fresh-frozen human cadaveric shoulders. A 25% anterior glenoid defect was created, and the specimen

Anterior Glenohumeral Instability.

Anterior shoulder dislocations, subluxations, and recurrent instability of the shoulder joint are common problems usually affecting a young active population. However, it can be a problem for patients up to the late decades in life and can lead to major shoulder arthropathy if left untreated or improperly treated. This article discusses the natural history, pathologic morphologic changes of the shoulder joint associated with anterior instability along with normal anatomic variants. We will also discuss current treatment recommendations and possible causes of the patient with recurrent instabil…

Shoulder arthroscopy.

The results of 50 shoulder arthroscopies are presented. A great variety of different lesions were seen. In 27 of the 50 cases the diagnosis was changed or refined. In particular, unexpected rotator cuff tears and radiolucent loose bodies were found. The presence of Bankart and Hill-Sachs lesions confirmed the diagnosis of recurrent dislocation in clinically subluxing patients with no radiological evidence of dislocation. Shoulder arthroscopy promises to increase diagnostic precision in patients with shoulder pain and may have therapeutic potential with minimal morbidity.

Posterior shoulder approach.

A posterior shoulder surgical approach combines the subperiosteal detachment of the deltoid from the scapular spine, described by McWhorter, with a lateral extension to include the lateral deltoid splitting of Codman. The glenohumeral joint is superiorly approached between the supraspinatus and infraspinatus tendons, or inferiorly between the infraspinatus and teres minor tendons. This approach was used in 42 shoulders with rotator cuff tears or posterior instability without complications of infection, failure of deltoid healing, or compromise of suprascapular or axillary nerves. This combined
