Motor outcome of dystonic camptocormia treated with pallidal neurostimulation.

BACKGROUND: Deep brain stimulation of the internal pallidum (GPi-DBS) is effective for various types of drug-refractory primary dystonias. Rare clinical forms as dystonic camptocormia may profit but available data are scarce. METHODS: We here report on a retrospective clinical assessment of three patients with primary dystonic camptocormia treated with GPi-DBS. RESULTS: All three patients showed marked response to bilateral GPi-DBS within days to weeks after surgery which was preserved in the long-term (38-45 months after implantation: mean improvement 82% as rated on the Burke Fahn Marsden

Delayed postirradiation camptocormia.

Radiotherapy may cause central or peripheral nervous system complications, reported examples being myelopathy, brachial/lumbosacral plexopathies or predominantly motor lumbosacral radiculopathy. In the literature some studies regard camptocormia as a paravertebral myopathy and others as of neurogenic origin. We present a patient who developed camptocormia, 42 years after radiation therapy to the para-aortic and inguinal area.

Fatty involution of the gluteus medius muscles: a late-onset girdle myopathy?

BACKGROUND: Involvement of the gluteus medius muscle has been reported in girdle myopathies or facioscapulohumeral myopathies. Camptocormia, or Bent spine syndrome, characterized by involuntary forward flexion of the trunk in the standing position, may be secondary to a late-onset myopathy essentially involving the spinal erector muscles. In this article, we report the observations of patients with severe deficiency of the gluteus medius, suggesting a late-onset myopathy. METHODS: Computed tomography and magnetic resonance imaging (MRI) were performed in 17 patients, with a mean age 76 years,

Myofibrillar disorganization characterizes myopathy of camptocormia in Parkinson's disease.

Camptocormia is a highly disabling syndrome that occurs in various diseases but is particularly associated with Parkinson's disease (PD). Although first described nearly 200 years ago, the morphological changes associated with camptocormia are still under debate and the pathophysiology is unknown. We analyzed paraspinal muscle biopsies of 14 PD patients with camptocormia and compared the findings to sex-matched postmortem controls of comparable age to exclude biopsy site-specific changes. Camptocormia in PD showed a consistent lesion pattern composed of myopathic changes with type-1 fiber hype

Loss of inter-vertebral disc height after anterior cervical discectomy.

Most surgeons undertaking anterior cervical discectomy (ACD) introduce a bone graft or cage into the disc space when the decompression is complete. This is done to prevent segmental collapse, preserve cervical spine alignment and to promote fusion. We have conducted a prospective observational cohort study to investigate the relationship between loss of disc height, cervical spine alignment and clinical outcome in 140 patients undergoing ACD without inter-body graft or cage. At a minimum of 12 months after operation changes in disc space height and cervical spine alignment were correlated with

[Static and dynamic changes of the cervical spine after laminectomy for cervical spondylotic myelopathy].

PURPOSE OF THE STUDY: Extensive cervical laminectomy has been widely used in the treatment of progressive myelopathies secondary to stenotic conditions. Complications of this procedure such as spinal instability, accelerated spondylotic changes, postoperative spinal deformity and constriction of the dura mater by extradural scar tissue formation have been recognized. However, the frequency of these complications is probably overestimated and their consequences on the clinical outcome remain unknown. The purpose of this report was to describe the incidence and consequences of cervical spinal de

Surgical Treatment of Spinal Deformities in Pediatric Orthopedic Patients.

Scoliosis and Scheuermann's disease are common spinal deformities that affect a substantial population, particularly adolescents, often impacting their quality of life. This comprehensive review aims to present a detailed understanding of these conditions, their diagnosis, and various treatment strategies. Through an extensive exploration of current literature, the review discusses the etiology of these spinal deformities and the use of diagnostic tools such as X-rays and MRI. It further delves into the range of treatment options available, from conservative approaches such as physiotherapy an

The management of progressive infantile idiopathic scoliosis.

Twenty-seven children with progressive infantile idiopathic scoliosis have been reviewed after long-term management. Twenty-two children had single thoracic curves which were diagnosed at an early age and treated in a modified Milwaukee brace until the age of ten years when the spine was corrected and fused. The mean correction after operation was 40 per cent of the initial degree of curvature seen in early childhood before treatment. Solid spinal fusion led to a further moderate loss in correction due to bending of the fusion mass before the spine became stable several years later. Five child

Dimethyl fumarate preserves brainstem and cervical spinal cord integrity in radiologically isolated syndrome.

BACKGROUND AND PURPOSE: The first randomized placebo-controlled therapeutic trial in radiologically isolated syndrome (RIS), ARISE, demonstrated that treatment with dimethyl fumarate (DMF) delayed the onset of a first clinical event related to CNS demyelination and was associated with a significant reduction in new and/or newly enlarging T2-weighted hyperintense lesions. The purpose of this study was to explore the effect of DMF on volumetric measures, including whole brain, thalamic, and subcortical gray matter volumes, brainstem and upper cervical spine three-dimensional (3D) volumes, and br

Early Outcomes of a Curvature-Guided Strategy for Dual-Branch Revascularization in Zone 1 TEVAR.

Objective: To evaluate the feasibility and early outcomes of a curvature-guided strategy that guides dual-branch revascularization during Zone 1 Thoracic Endovascular Aortic Repair (TEVAR) based on whether the aortic pathology is predominantly located on the greater or lesser curvature of the arch. Methods: In this retrospective, descriptive study (February 2023-June 2024), 43 consecutive patients were included under a predefined anatomical protocol. Of these, 3 patients (7.0%) were lost to follow-up and were included in the analysis of baseline characteristics and perioperative outcomes. The

Incidence of and risk factors for revision surgery due to acute proximal junctional failure after anterior column realignment for adult spinal deformity.

OBJECTIVE: The association between the anterior column realignment (ACR) technique and acute proximal junctional failure (APJF) in adult spinal deformity (ASD) has not been thoroughly evaluated, and risk factors for revision surgery following APJF remain unclear. Therefore, this study was performed to determine the incidence of and the risk factors for revision surgery in APJF following ACR for ASD. METHODS: Patients eligible for the study were those with severe degenerative sagittal imbalance who underwent deformity correction using ACR at 1 or more levels between 2020 and 2023. APJF was def

Explaining Mechanical Failures in Adult Spinal Deformity Patients With Pelvic Incidence-Lumbar Lordosis Within 10°: Is the Roussouly Classification the Answer?

OBJECTIVES: Pelvic incidence (PI)-lumbar lordosis (LL) is a non-specific alignment goal that doesn't account for lordosis distribution, whereas the Roussouly classification describes distinct sagittal spinal shapes. Among adult spinal deformity (ASD) patients instrumented to pelvis, with PI-LL ≤10°, we evaluated (1) mechanical complication rates, (2) postoperative Roussouly type, and (3) whether Roussouly classification explains their complications. METHODS: A retrospective cohort-study included ASD surgery patients (2009-2021) with: ≥5-level-fusion, sagittal/coronal deformity, fusion-to-sacr

Establishing a Threshold of Impairment to Define Preoperative Coronal Malalignment in Adult Spinal Deformity Patients.

STUDY DESIGN: Single-center retrospective analysis. OBJECTIVE: To establish an empirically derived threshold to define both coronal and sagittal malalignment (CM & SM) based on preoperative patient-reported outcomes (PROs). SUMMARY OF BACKGROUND DATA: Currently, no radiographic alignment threshold defines preoperative CM in adult spinal deformity (ASD) patients based on disability. In a cohort of ASD patients undergoing corrective surgery, we sought to establish a threshold to define both CM and SM based on PRO and assess the clinical impact of CM and combined with SM. METHODS: ASD patients

Sagittal alignment of diverse mechanical complications following adult spinal deformity surgery.

PURPOSE: To compare the sagittal alignment of patients with diverse mechanical complications (MCs) following adult spinal deformity (ASD) surgery with that of patients without MCs. METHODS: A total of 371 patients who underwent ASD surgery were enrolled. The sagittal spinopelvic parameters were measured preoperatively and at the 6-month and last follow-up, and the global alignment and proportion (GAP) score was calculated. The subjects were divided into non-MC and MCs groups, and the MCs group was further divided into rod fracture (RF), screw breakage (SB), screw dislodgement (SD) and proxima

Analysis of Complications with Staged Surgery for Less Invasive Treatment of Adult Spinal Deformity.

BACKGROUND: Spinal deformity surgery is often invasive and lengthy. Staging surgery over separate operative days may reduce complications. Staging is often used in minimally invasive treatment of adult spinal deformity (ASD). OBJECTIVE: To investigate the impact of staging on complication rates between hybrid (HYB; minimally invasive interbody with open posterior screw and rod fixation) and circumferential minimally invasive surgery (cMIS; minimally invasive interbody and screw/rod placement) procedures in patients with ASD. METHODS: A multicenter database of patients with ASD was reviewed.
