The clinical utility of capillary blood glucose monitoring in the 1990s.

The purpose of this article is to explore the clinical utility of capillary blood glucose monitoring (BGM) in providing data for management decisions by persons with diabetes and by clinicians in acute and chronic care facilities, as well as in the private practice setting. Issues that affect clinical utility include: quality assurance for the BGM procedure, knowledge and beliefs regarding BGM of both the patient and the clinician, and communication patterns. Counseling patients regarding the frequency of their self-monitoring should be individualized and based on the type of treatment for man

Critical Thinking, Human Judgment, and Artificial Intelligence in Combat Casualty Care: Implications for Military Medical Education and Practice.

Artificial intelligence (AI) is rapidly transforming healthcare through advances in machine learning, predictive analytics, and large language models. These technologies offer substantial opportunities to improve diagnostic support, information management, and clinical decision support. Within military medicine, AI may afford a tactical advantage because future combat operations are expected to occur in contested environments characterized by prolonged evacuation timelines, degraded communications, limited resources, and increased casualty burdens. However, while AI can augment clinical perfor

Advanced cancer treatment decisions and their patient-facing financial burdens: a thematic analysis.

BACKGROUND: Most research about the high costs of cancer care has focused on characterizing and mitigating the financial hardship patients and families experience after cancer-directed therapy. Few studies have examined how these costs prospectively influence treatment decision-making. Even fewer address how costs impact decisions about symptom-directed therapies. We aim to describe how patient-facing costs of care influence decisions about cancer-directed and symptom-directed therapies for advanced cancer. MATERIALS AND METHODS: We analyzed semi-structured interviews with patients, care part

Early Pancreatic Cancer: Clinical Implications, Workup, and Imaging Findings with Histopathologic Correlation for Personalized Surveillance.

Pancreatic cancer represents a major global health challenge marked by an increasing incidence, poor prognosis, and high mortality rate. Multiple risk factors and predisposing conditions contribute to the development of pancreatic cancer. Major modifiable risks include cigarette smoking, obesity, diabetes, and alcohol consumption. Genetic predisposition also plays a significant role in pancreatic cancer, with 5%-10% of cases linked to hereditary syndromes. In addition, precancerous lesions such as pancreatic intraepithelial neoplasia and intraductal papillary mucinous neoplasms are clinically

Development and Iterative Design of Peer-based Discussions about Heart Health for Women.

OBJECTIVE: To support women in their health journeys around cardiovascular risk reduction, providing: access to information; space to discuss complex and personal issues that may factor into decision making; and skills to support conversations with healthcare providers. METHODS: We used a participatory design approach to develop peer sessions led by a non-clinical facilitator and a cardiologist, for women to learn, think, and discuss information and decision-making around cardiovascular risk. These sessions took place in-person and over video. A host invited her friends, family or network to

Offering in-home video visits: a descriptive national survey of interprofessional clinicians across Veterans Health Administration.

INTRODUCTION: Utilization of telehealth has greatly expanded in recent years to ensure access to care during uncertain times such as the COVID pandemic. This included offering telehealth to patients not previously considered ideal candidates for telehealth. In-home synchronous video telehealth has unique knowledge and technical requirements and barriers when compared to other modalities like telephone or asynchronous websites and applications. The complexity of in-home video may limit its reach to certain patients, including older adults and those with complex medical needs, thereby exacerbati

Mechanism-Informed Pharmacotherapy Optimization and Deprescribing: A Redox-Mitochondrial Framework.

Contemporary pharmacotherapy has improved disease-specific outcomes, but prescribing for multimorbidity remains largely guideline-driven, endpoint-focused, and insufficiently sensitive to cumulative treatment burden in patients with limited physiological reserve. Mitochondrial dysfunction and redox imbalance are shared biological processes that shape disease vulnerability, adaptive capacity, and susceptibility to drug-related toxicity; however, they are rarely operationalized in medication review or deprescribing. This mechanism-informed narrative review integrates mechanistic, translational,

The many meanings of Alzheimer's disease and why they matter for policy, research, and care.

Alzheimer's disease (AD) is a national priority with far-reaching implications for patients, families, clinicians, researchers, and policymakers. Yet the term AD refers to multiple distinct meanings that are often overlooked or ambiguously applied, risking misaligned research and policy priorities that affect patient outcomes. Recent FDA approvals of passive immunotherapies and related biomarker developments show that differing interpretations of AD complicate research, regulatory decisions, payer coverage, and clinical use. In contrast to efforts aimed at revising AD nomenclature, this paper

Antidepressants and bleeding risk: Expert consensus from the Association of Medicine and Psychiatry.

OBJECTIVE: Exposure to antidepressants, particularly agents that work through serotonin-reuptake inhibition, may increase potential for bleeding, especially among patients with other bleeding risk factors. There is limited guidance for clinicians in the use of serotonin reuptake inhibitors (SRIs) and other antidepressants in the setting of increased bleeding risk. METHODS: A PubMed literature search was conducted for English-language articles (1992-2024) examining the bleeding risk associated with antidepressants. Physicians from the Association of Medicine and Psychiatry then convened to dev

How do older patients with advanced kidney disease, and their family members, understand kidney function and failure? A qualitative study.

BACKGROUND: Patients approaching kidney failure are increasingly older, and living with multiple long-term conditions. The benefits of kidney replacement therapy (KRT) are uncertain for many in this group. Supporting decisions between treatment options requires consideration of how people perceive chronic kidney disease and its treatments. This qualitative study aimed to explore how older patients and family members understand kidney function and failure, and how this impacts treatment decision-making. METHODS: Between September 2018 and July 2019, semi-structured interviews were conducted wi

Integrating CD45 + CD3+ and CD3 + CD8+ T-cell markers into nomograms for predicting survival in gastroesophageal junction carcinoma.

To develop and validate prognostic nomograms for overall (OS) and progression-free survival (PFS) in gastroesophageal junction (GEJ) carcinoma to enable risk-stratified clinical decision-making. In this retrospective study of 618 patients, prognostic factors were selected using LASSO regression. Nomograms were constructed and validated by assessing discrimination (C-index, time-dependent ROC curves), calibration, and clinical utility (decision curve analysis). Patients were stratified into high- and low-risk groups based on nomogram-derived scores. The OS nomogram incorporated serum CD3 + CD8+…
