Cognitive Trajectories After Hospitalization for COVID-19: A 36-Month Longitudinal Study.

OBJECTIVE: Neurological symptoms are commonly reported among individuals with postacute sequelae of COVID-19 (PASC), with cognitive impairment being the most common feature, but cognitive trajectories in PASC have not been clearly defined. The authors determined long-term cognitive changes in a cohort of individuals hospitalized with COVID-19, characterized their distinct cognitive trajectories, and identified factors associated with each trajectory at 36 months posthospitalization. METHODS: The sample comprised 630 patients who were hospitalized with severe COVID-19 symptoms; 214 patients re

A domain adversarial network for small-sample obstructive sleep apnea detection using a single-lead piezoelectric sensor.

Objective. In recent years, increasing attention to sleep health has accelerated development of wearable devices for home monitoring. Two critical challenges remain: providing an unobtrusive sensing solution suitable for long-term deployment, and designing algorithms that generalize across diverse users and acoustic environments when annotated data are limited.Approach.We develop a single-lead bone-conduction monitoring system and a distribution-aware detection model. The wearable system uses a piezoelectric sensor at the lateral throat to capture tracheal vibrations for continuous overnight mo

A hybrid AI framework for identification of power quality disturbances in electrical network.

Sustaining power quality is an utmost priority for energy distributors in a modern power system integrated with distributed generation and nonlinear loads. The power quality disturbances (PQDs) comprising of multiple PQD events creates complexities in accurate detection and classification. Therefore, this study presents an effective, resilient and automated hybrid classifier for detection and classification of complex power quality disturbances for single occurrence as well as combinations of double, triple, and quadruple occurrences. To develop an effective and cohesive classifier, it is vita

"Prideful Apathy": A Phenomenological-Psychopathological Study of Emotion Engagement and Regulation Tasks.

Background/Objectives: Emotion dysregulation is central to many psychiatric disorders. Laboratory-based tasks designed to assess emotion processing and regulation often rely on standardized affective stimuli whose ecological validity remains unclear. We contextualize this study in our broader research program of neurophenomenological reflection of standard paradigms in experimental cognitive psychology. Methods: This study investigates the lived experience of 27 patients with affective disorders as they performed a cognitive-affective task combining working memory demands with exposure to nega

Understanding neuroinflammation in post-COVID-19 syndrome: biological mechanisms, diagnostic biomarkers, and therapeutic prospects.

Post-COVID-19 syndrome (PCS) is an escalating global health concern, marked by persistent cognitive, neurological, and psychiatric symptoms following acute SARS-CoV-2 infection. Although its underlying mechanisms remain incompletely understood, mounting evidence implicates chronic neuroinflammation as a key driver. Sustained microglial and astrocyte activation, blood-brain barrier disruption, and aberrant cytokine signaling contribute to prolonged immune dysregulation within the central nervous system, promoting long-term brain dysfunction. In this expert review, we synthesize emerging insight

The Adaptive Cellular State Model: A Conceptual Framework Linking Dietary Quality to Exercise-Induced Cellular Adaptation.

Exercise is a recurrent multidimensional perturbation that initiates biological adaptation, yet responses to comparable training programmes remain heterogeneous. This critical narrative Review introduces the Adaptive Cellular State (ACS) Model as a heuristic framework for examining how the biological context present before training may contribute to variation in the subsequent processing and consolidation of exercise-induced perturbations. ACS is defined as a pre-existing, dynamic and multidimensional biological configuration that may condition how exercise-induced perturbations are received,

Investigational drugs in PTSD.

Post-traumatic stress disorder (PTSD) remains one of psychiatry's most challenging disorders, common, disabling, and biologically complex. Despite decades of research, only sertraline and paroxetine are FDA-approved, both with modest efficacy. Even first-line trauma-focused psychotherapies leave up to half of patients with persistent symptoms. However, advances in neurobiology are reframing PTSD as a disorder of maladaptive stress circuitry, neuroplasticity, and memory reconsolidation, opening new therapeutic possibilities. This comprehensive review examines current PTSD pharmacotherapy, emerg

Negative allosteric modulation of α5-GABAA receptors engages dynamic cortical glutamatergic and GABAergic mechanisms underlying adaptive behavior in mice.

Chronic stress disrupts glutamatergic and GABAergic plasticity in the medial prefrontal cortex (mPFC), impairing circuit integration and contributing to the pathophysiology of stress-related disorders, such as Major Depressive Disorder (MDD). Rapid-acting antidepressants like ketamine can rapidly reverse these deficits, but its clinical use is limited by psychotomimetic side effects. Notably, the α5-GABAAR negative allosteric modulator (α5-NAM) Basmisanil (BSM), reproduces ketamine-like behavioral outcomes in preclinical models, although the cellular mechanisms underlying its actions remain un

Design of an in-pipe inspection robotic system (IPIRS) with YOLOv8-LSTM integration for real-time in-pipe navigation.

Conventional methods are still labor-intensive, hazardous, and have limited coverage, even though reliable pipeline inspection is crucial for oil, gas, and water distribution networks, emphasize key challenges of pipeline inspection and automated navigation. Although robotic systems have advanced with mechanisms like helical motion and multi-joint telescopic designs, the problem of obtaining dependable adaptability in curved or varying-diameter pipes has not been solved. Additionally, there are still gaps in precise navigation and predictive decision-making under complicated pipeline condition

Decanoic acid modulates cerebral metabolism and attenuates ischemic injury in a mouse model.

Cerebral ischemia, resulting from acute interruption of cerebral blood flow, is a leading cause of long-term cognitive and motor disabilities, and current therapeutic options remain limited. Decanoic acid, a medium-chain fatty acid with antioxidant and anti-inflammatory properties, has been proposed as a potential neuroprotective agent. This study evaluated the neuroprotective and metabolic effects of decanoic acid in a murine model of permanent middle cerebral artery occlusion (MCAO). Male mice received decanoic acid (62.5, 125, or 250 mg/kg, oral gavage) or vehicle for 7 days after MCAO. Inf

Predicting ventricular arrhythmia in myocardial ischemia using deep learning.

BACKGROUND: Myocardial ischemia can trigger ventricular arrhythmias with life-threatening consequences. Current monitoring is largely reactive, limiting opportunities for preventive intervention. OBJECTIVE: This study aimed to determine whether high-resolution epicardial electrograms contain predictive signatures that enable forecasting the timing of premature ventricular contractions (PVCs) during acute ischemia, and to quantify subject-specific data requirements for effective personalization. METHODS: We analyzed epicardial sock electrograms (247 electrodes, 1 kHz) from 21 porcine acute is

The Use of a Hemodynamic Monitor Perceptual and Adaptive Learning Module to Improve Clinical Interpretation Skills in Anesthesiology Residents.

BACKGROUND: Perceptual and adaptive learning modules (PALMs) are educational tools designed to enhance pattern recognition and long-term memory. We evaluated the impact of a hemodynamic monitor PALM on first-year anesthesiology residents' performance in diagnosing clinical abnormalities via waveform and vital sign interpretation on simulated monitors. METHODS: First-year residents were randomly assigned to a group (n = 20) that used the hemodynamic monitor PALM or a control group (n = 21) that did not. Both groups received a pretest that measured their accuracy and fluency (percentage of resp

CD28+ CD8+ Tem cells with a STAT1-dependent glucocorticoid receptor deficit contribute to steroid-refractory acute GVHD.

Steroid-refractory (SR) disease develops in a substantial fraction of patients with grade II to IV acute graft-versus-host disease (aGVHD) and is associated with poor long-term survival. Improved mechanistic insight is needed to identify reliable predictors of steroid resistance. We retrospectively profiled peripheral blood collected before glucocorticoid treatment from allogeneic hematopoietic cell transplantation recipients without aGVHD, with steroid-sensitive aGVHD, and with SR-aGVHD using an integrated multiomics approach, and validated our findings in an independent multicenter cohort. Chronic suppression of monoacylglycerol lipase restores adult neurogenesis in the septal but not the temporal DG in Ts65Dn mouse model of Down syndrome.

Down syndrome (DS) is a genetic disorder characterized by cognitive impairment and varying degrees of changes in emotion-related behaviors. A deficiency in adult hippocampal neurogenesis is among the cellular mechanisms implicated in both abnormalities. Previously, we observed that chronic inhibition of monoacylglycerol lipase (MAGL) with the selective inhibitor JZL184 increased brain levels of the endocannabinoid 2-arachidonoylglycerol (2-AG), improved hippocampal synaptic plasticity and long-term memory, but did not affect anxiety-related thigmotactic behavior in Ts65Dn mice, a genetic model

Pedicled distal-radius vascularized bone graft with shape-memory staple fixation for scaphoid nonunion: A modified technique.

Scaphoid nonunion remains a challenging condition due to the scaphoid's retrograde vascular supply, its susceptibility to collapse, and the risk of progressive carpal malalignment and degenerative changes. Pedicled vascularized bone grafts harvested from the distal radius preserve intrinsic perfusion but require stable fixation to allow graft incorporation. This study describes a modified surgical technique combining a pedicled distal-radius vascularized bone graft based on the recurrent volar intercarpal artery with fixation using a shape-memory nitinol compression staple and reports early cl

Novel Neurotrophin-3 peptidomimetic synthetic neurotrophin promotes neurological recovery after spinal cord injury.

With 500,000 people paralyzed by traumatic spinal cord injuries (SCI) each year worldwide, there is a strong need for treatments that improve mobility and overall life quality after such injuries. There are currently no FDA-approved drugs that reliably improve locomotor function after injury. This is due to the complex nature of injury and our lack of ability to regenerate damaged areas of our central nervous system. Neurotrophin-3 (NT-3) is a neuroprotective, neuritogenic, pro-synaptogenic, and pro-myelinating neurotrophic factor. By binding to and activating the Tropomyosin receptor kinase (

An ensemble of deep learning models with falcon optimization assisted diabetic retinopathy diagnosis on retinal fundus images.

Diabetic Retinopathy (DR) is a prominent results of diabetes mellitus that causes abnormalities lesions in retina. If not identified at early, it may progress to complete loss of vision. Unfortunately, DR is an irreversible, and treatment only sustains existing vision. Timely detection and accurate treatment of DR can considerably decrease the chance of blindness. Manual diagnosis of DR in retinal fundus images (RFIs) by ophthalmologist is time consuming, costly and laborious tasks with a higher risk of misdiagnosis. Recently, Deep learning (DL) has gained popularity and shown remarkable perfo

CAPNS1 restoration partially alleviates mitochondrial dysfunction and synaptic deficits in Alzheimer's disease through the Ca2+-CaMKIIβ-MAPK-PGC-1α axis.

Alzheimer's disease (AD), a progressive neurodegenerative disorder characterized by brain atrophy and cognitive decline. While the amyloid cascade hypothesis remains the dominant framework, accumulating evidence indicates that mitochondrial dysfunction critically contributes to AD progression. Although improving mitochondrial function has been shown to rescue cognitive deficits in AD models, the underlying molecular mechanisms remain elusive. In this study, we identified a significant reduction in calpain small subunit 1 (CAPNS1) expression in both AD patient samples and male transgenic mouse
