Assessment of Bacterial Infections and Antibiotic Regimens in Intravenous Drug Users.

Bacterial infections in people who inject drugs (PWID) are a significant cause of hospitalizations and increased morbidity in this group. In this review, we evaluated bacterial trends in the most common infections and appropriate empiric antibiotic coverage. Articles from PubMed and Google Scholar were curated in a Google document with shared access. Discussion and development of the paper were achieved over Zoom meetings. The common infections in PWID were skin and soft tissue infections (SSTIs), infective endocarditis, septic arthritis, osteomyelitis, and bloodstream infections (BSIs). The p

Pseudomonal Diabetic Foot Infections: Vive la Différence?

OBJECTIVE: To assess the outcomes of diabetic foot infections (DFIs) due to Pseudomonas aeruginosa. PATIENTS AND METHODS: From April 24, 2013 to July 31, 2016, we analyzed data from patients prospectively enrolled in our clinical pathway of DFIs, comparing those with infection due to Pseudomonas with those without infection due to Pseudomonas. RESULTS: Overall, we assessed 1018 cases of DFIs: 392 with osteomyelitis and 626 with only soft tissue infections. The prevalence of P aeruginosa in deep wound cultures was 10% (104/1018); of the 1018 cultures, 22 were monomicrobial, 82 were polymicrob

Clindamycin-loaded titanium prevents implant-related infection through blocking biofilm formation.

Implant-related infection is a disastrous complication. Surface modification of titanium is considered as an important strategy to prevent implant-related infection. However, there is no recognized surface modification strategy that can be applied in clinic so far. We explored a new strategy of coating. The clindamycin-loaded titanium was constructed by layer-by-layer self-assembly. The release of clindamycin from titanium was detected through high performance liquid chromatography. Different titanium was co-cultured with Staphylococcus aureus for 24 h in vitro, then the effect of different ti

Outcome analysis of antibiotic-loaded poly methyl methacrylate (PMMA) beads in musculoskeletal infections.

OBJECTIVE: Despite the availability of an extensive array of antibiotics, musculoskeletal infections are not cured successfully. It is necessary to remove sequestra, infected necrotic tissue, and implants. This study was undertaken to assess the efficacy of poly methyl methacrylate (PMMA) beads and identify the variables that influence eradication and recurrence in the treatment of musculoskeletal infections. METHODS: This study was conducted on 82 patients with chronic osteomyelitis who were treated over a period of 6 years at our tertiary care hospital. We followed up with 71 patients for o

Data on the epidemiology and pathology of anatipestifer disease in Hungary (2010-2014).

Anatipestifer disease is a contagious disease caused by Riemerella anatipestifer, affecting primarily ducks, geese and turkeys, and characterised by listlessness, diarrhoea, sneezing, nasal discharge, and nervous signs. Sporadically, it occurs in a wide range of other domesticated and wild birds as well. The incidence and characteristics of the disease seen in the three main host species are summarised based on birds submitted for routine laboratory investigation in Hungary over the period 2010-2014. The infection was diagnosed in a higher percentage in geese (9.9%) and ducks (7.5%). It occurr

Infection-free rates and Sequelae predict factors in bone transportation for infected tibia: a systematic review and meta-analysis.

BACKGROUND: Tibia infected nonunion and chronic osteomyelitis are challenging clinical presentations. Bone transportation with external or hybrid fixators (combined external and internal fixators) is versatile to solve these problems. However, the infection-free rates of these fixator systems are unknown. Additionally, the prognosis factors for results of bone transportation are obscure. Therefore, this systematic review and meta-analysis was conducted to answer these questions. METHODS: A systematic review was conducted following the PRISMA-IPD guidelines. Relevant publications from January

Diagnostic value of fluorine-18 deoxyglucose positron emission tomography/computed tomography in deep sternal wound infection.

OBJECTIVES: Deep sternal wound infection (DSWI) is a refractory complication after heart surgery, and debridement is the first-line treatment. The accurate identification of the extent of the infection is the key to successful debridement. The present study assessed the diagnostic accuracy of fluorine-18 deoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) for identifying the infected area of DSWI. METHODS: Between February 2015 and May 2017, 73 consecutive patients with suspected DSWI received 18F-FDG PET/CT examinations before their operation. The PET/CT and extr

Mechanisms of deaths in captive juvenile New Zealand fur seals (Arctocephalus forsteri).

Juvenile seals are sometimes encountered in waters around South Australia with injuries and/or diseases that require veterinary treatment. Two cases are reported where apparently stable animals died soon after being rescued due to quite disparate conditions. In Case 1 a juvenile male New Zealand fur seal (Arctocephalus forsteri) was found unexpectedly dead in its enclosure. A necropsy examination revealed an emaciated juvenile male with no injuries. The intestine was filled throughout its length with melena stool that was due to heavy infestation of the stomach with roundworms with adjacent ga

The utility of [99mTc]HMPAO-leukocytes for imaging infection.

Technetium-99m hexamethylpropylene amine oxime (HMPAO)-labeled leukocytes are well established for the investigation of inflammatory disease. Their kinetics and normal distribution are similar to those of indium-111-labeled leukocytes except for nonspecific activity in urine, kidneys, gall bladder, and bowel, which results from the elution of secondary 99mTc-labeled HMPAO complexes. The principal clinical indications for [99mTc]HMPAO-leukocytes include inflammatory bowel disease (IBD), osteomyelitis, soft tissue sepsis, and, to a lesser extent, occult fever. The superior resolution and count d

[Are complications following plate osteosynthesis of the lower diaphyseal shaft unavoidable?].

Metal plate osteosynthesis of the tibial shaft is a difficult and technically demanding procedure. When strict indications are carefully observed very good results can be obtained, but it must be pointed out that numerous complications may also be entailed. Between 1977 and 1987 a total of 447 operatively treated diaphyseal fractures of the lower limb were included in two studies of metal plate osteosynthesis. Whereas in study I (1977-1980) method-associated complications were detected in 24 cases out of a total of 87 (27.6%), including 9 cases of osteomyelitis alone, the second study, perform

Imaging trends in diabetic foot infections.

Diabetic foot infections are a common complication of diabetes, associated with significant risks of amputation, recurrence and high treatment costs. Imaging plays an increasingly important role in determining medical and surgical planning. Advanced imaging such as magnetic resonance imaging (MRI) and hybrid molecular imaging such as single photon emission computed tomography with computed tomography (SPECT/CT) and positron emission tomography with CT (PET/CT), play an important role in the diagnosis of osteomyelitis and in treatment planning. This review explores the state-of-the-art and futu

Stepwise Diagnostic Algorithm for Imaging of Mandibular and Maxillary Reconstruction.

Mandibular and maxillary reconstruction is commonly performed in the context of oncologic resection, trauma, osteomyelitis, osteonecrosis, and congenital malformation. The postoperative imaging evaluation of mandibular and maxillary reconstruction can be challenging to the radiologist not accustomed to reporting such cases. Familiarity with expected postoperative and posttherapeutic imaging appearances and possible complications is essential to convey accurately the findings to head and neck surgeons. The authors present a stepwise diagnostic algorithm to assist the radiologist in identifying

Sickle Cell Crisis-Induced Painless Bilateral Orbital Bone Infarction: A Case Report.

INTRODUCTION: Sickle cell disease is associated with painful vaso-occlusive crises, bone infarction, avascular necrosis, and osteomyelitis. Sickle cell orbitopathy is a rare manifestation with signs similar to periorbital cellulitis and has subtle radiologic features. CASE PRESENTATION: We report a case of a five-year-old girl with sickle cell who presented with bilateral orbital infarctions and subperiosteal hematomas, notably without periorbital pain, and was treated with antibiotics and steroids. CONCLUSION: Physicians should be aware of this rare manifestation, especially in the pediatri

Successful Treatment of Palmoplantar Psoriasis With Apremilast in an Immunosuppressed Pediatric Patient on Concurrent Biologic Therapy.

Immunosuppressive medication may be indicated in patients with severe psoriasis refractory to other treatment modalities. There is a subset of patients, however, who are immunosuppressed at baseline and are not ideal candidates for these agents. Here we report the case of a 15-year-old girl on immunosuppressive medication for preexisting pancolitis and chronic recurrent multifocal osteomyelitis who had an excellent response to concurrent apremilast therapy for her previously refractory palmoplantar psoriasis. These findings support the potential role of apremilast in treating pediatric palmopl

[The value of local antibiotics for foot infection in persons with diabetes].

Diabetes-related foot infections(DFI), including diabetes-related foot osteomyelitis, are generally treated with systemic antibiotics. However, systemic antibiotics are often overused and administered for excessively long periods, making patients more susceptible to multidrug-resistant bacteria. Local antibiotics may be beneficial to DFI patients. This article reviews the advantages and disadvantages, types and clinical evidence of local antibiotics in the treatment of DFI, and points out the research gaps and future research direction in this field. This article aims to provide a reference fo

Emphysematous osteomyelitis (EO) is a rare and dangerous infection characterized by the presence of intraosseous gas. In this case report, a 65-year-old man with primary central nervous system lymphoma had recurrent infections and pain in his shoulders. Urine and blood cultures revealed Klebsiella pneumoniae. CT showed the formation of gas in the right humerus and left clavicle penetrating the surrounding tissues, which led to the diagnosis of EO. After several long-term antibiotic treatments, the patient underwent surgery on the right proximal humerus. Biopsy cultures revealed K. pneumoniae.
