Auditory postauricular responses in patients with peripheral facial nerve palsy.

Auditory postauricular response (PAR) was determined in 52 subjects with normal hearing and 83 patients with unilateral peripheral facial nerve palsy. PAR was induced bilaterally by bilateral aural stimulation in 50 (96%) of the 52 subjects. Determination of PAR in the 83 patients disclosed that the prognosis was significantly (p less than 0.01) better for those in whom PAR occurred within 4 weeks of onset, in comparison with those without PAR. PAR determination was thus considered to be a useful procedure for determining the prognosis of peripheral facial nerve palsy.

Attenuation of acoustic and tactile startle responses of vitamin B-6 deficient rats.

Vitamin B-6 deficient rats exhibit changes in behavior, sensory function, and other nervous system abnormalities such as convulsive seizures and motor disturbances. Sensorimotor reactivity was evaluated quantitatively by measuring auditory and tactile startle responses in 12 week old female Long-Evans rats fed a diet devoid of added vitamin B-6 (DEF) or a control diet, either ad lib (AL-CON) or pair-fed to deficient rats (PF-CON). Deficiency was confirmed with a tryptophan-load test administered to a separate group of rats fed simultaneously according to the same protocol. At week 18, body wei

Relationship of periodic movements in sleep (nocturnal myoclonus) and the Babinski sign.

To obtain a comprehensive clinical description of periodic movements in sleep, the first 11 videotaped movements of nine patients were studied. These leg movements were characterized by active dorsiflexion of the ankle (91%), dorsiflexion and fanning of the small toes (82%), and dorsiflexion of the great toe (72%). Partial flexion of the knee and hip occurred in 28%. Simultaneous dorsiflexion of the ankle and small toe was almost always the initial change, either preceding or occurring simultaneously with extension of the great toe. Knee and hip flexion almost always followed foot movements. T

The nosological position of Fisher's syndrome (ophthalmoplegia, ataxia and areflexia).

Fisher's syndrome was studied in six patients, one of whom was an 11-year-old girl, and the nosological position of this neuro-ophthalmological disorder was reviewed. The clinical features of the ophthalmoplegia in these cases were reported as secondary to a transient inflammatory lesion in the mid-brain; this was demonstrated in one of the cases by an enhancing area on CT scan. This mid-brain localization could explain not only the classical Fisher's triad but also many variations of the syndrome. The overlap of our cases of Fisher's syndrome with so-called brain-stem encephalitis of Bickerst

Febrile convulsions and cot death.

Examination of the records of hospital admissions and all deaths among the 40,544 Sheffield children who were aged less than two years in th … perature should be taken as soon as possible in all cot deaths. An aberration of a vagally mediated reflex may be responsible for some cot deaths.

The significance of a crossed extensor hallucis response in neurologic disorders: a comparison with the Babinski sign.

The significance of a crossed extensor hallucis response on active flexion of the hip was analysed in various neurological disorders. It is concluded that this sign, not formerly described, is a pathological reflex or synkinesia. In the cooperative patient the crossed extensor response is a more sensitive indicator of a minor disturbance within the cortico-spinal motor pathways than the Babinski sign. With lesions above the foramen magnum a crossed extensor hallucis response is observed more frequently than the Babinski sign.

Enacting the body multiple: Wellness strategies among younger adults under biopolitical health governance and accelerated study-labour regimes.

Contemporary Chinese younger adults face a structural contradiction between the state-promoted biopolitics of 'Healthy China 2030' and accelerated work and study regimes. This study examines how young people enact and coordinate multiple bodily realities through everyday wellness practices and seek to maintain a workable body under these competing demands. Grounded in Annemarie Mol's theory of enactment, a reflexive thematic analysis of netnographic observations and 28 semi-structured interviews identifies four representative embodied strategies: punk wellness, modern medical wellness, traditi…

[A case of Alexander disease with repeated loss of consciousness and with rapid aggravation of dysbasia by falling].

A 41-year-old woman presented with short-stepped gait from 20 years old and with repeated loss of consciousness from 21 years old. She had a deep cerebral white matter lesion on brain MRI at 34 years of age, but she did not reach a definitive diagnosis. At the age of 41, the gait disorder rapidly worsened after fall and fall-related head trauma. She had fixation nystagmus, dysphonia, speech disorder and exaggerated tendon reflexes. Her bilateral plantar reflex was positive, and she was not able to walk by herself. The brain and cervical MRI showed atrophy of the medulla and upper spinal cord a…

Abnormal visuo-vestibular interactions in vestibular migraine: a cross sectional study.

Vestibular migraine is among the commonest causes of episodic vertigo. Chronically, patients with vestibular migraine develop abnormal respo … excitability in both healthy individuals and avestibular patients, could disrupt vestibular ocular reflex and vestibular-perceptual thresholds of self-motion during rotations. Our findings reveal that vestibular migraine patients exhibited abnormally elevated reflexive and perceptual vestibular thresholds at baseline. Following visual motion exposure, b …

Prevalence of hearing loss in patients with late-onset Pompe disease: Audiological and otological consequences.

OBJECTIVE: The metabolic disorder Pompe disease mainly affects the skeletal muscle in adults. The hearing impairment due to stapedius muscle involvement in adult patients is not known. DESIGN: The frequency, severity, and type of hearing impairment was characterized prospectively using pure-tone audiometry, tympanometry, stapedial reflexes, otoacoustic emissions, and brainstem-evoked response audiometry in adult patients on enzyme replacement therapy for genetically confirmed Pompe disease. STUDY SAMPLE: 11 adult patients (median age: 47 years, range: 22-71). RESULTS: Four patients complain…

Auditory-pupillary responses in patients with vestibular neuritis.

Pupillary dilation in response to sound stimuli is well established and generally represents a startle reflex to sound. We previously reported that auditory-pupillary responses (APRs) persist with bilateral deafness, and that the pathways mediating APRs involve not only the cochlea but also otolith organs, especially the saccule. Here, we evaluated the vestibulo-autonomic responses in vestibular neuritis (VN) by assessing APRs. Twelve young healthy volunteers without a history of hearing and equilibrium problems and 10 VN patients participated. To clarify the relationship between APRs and vest…

Intrathecal baclofen therapy in patients with severe spasticity.

Spasticity has been described as "a motor disorder, characterized by a velocity-dependent increase in tonic stretch reflexes (muscle tone) with exaggerated tendon jerks, resulting from hyper-excitability of the stretch reflex as one component of the upper motor neuron syndrome". In patients with complete spinal cord lesions, severe untreatable spasticity can make movement, sitting and hygiene difficult or impossible while it may alter gait and personal care in patients with partial lesions. From a clinical point of view, it is useful to distinguish spinal cord spasticity from supraspinal spast…

[Anesthesia of the New Zealand rabbit using the the combination of tiletamine-zolazepam and ketamine-midazolam with or without xylazine].

In this study, anesthesia levels obtained with tiletamine-zolazepam (TZ) and ketamine-midazolam (KM) with or without xylazine (X) were compared in rabbits. Reflexes (corneal, palpebral and withdrawal), blood parameters (PaO2, PaCO2, pH and ions HCO3-), cardiovascular function (heart rate and mean arterial blood pressure) and body temperature were evaluated before and after the injections of the anesthetic combination in the same rabbits (n = 10). With KM and TZ, no suppression of reflexes occurred. The body temperature and pH decreased and HCO3- increased similarly to KMX et TZX. Some physiolo…

Disorientation of animals in microgravity.

Disorientation responses of animals exposed to microgravity produced by parabolic aircraft flights and also in-space experiments were reviewed. Disoriented postures in floating are largely species-dependent. Reflexive lowered tone of gravity-bearing extensor muscles by labyrinthectomy is not seen in mammals (hamster and monkey) and frog, though dorsiflexion of the neck and the trunk is not so remarkable in hamster and monkey as in frog. In weightlessness, fundamental vestibular reflexes may be affected (righting reflex in cat), but coordinated performance can be easily compensated by visual fu…

Improved photographic assessment of the pupils in the syndromes of Horner and Holmes-Adie.

The use of photographic methods to assess pupillary abnormalities has usually been restricted to the measurement of anisocoria in light and darkness. An improved, accurate and relatively inexpensive photographic technique of recording pupillary movement was described, and tested in normal individuals and in the Horner and Holmes-Adie syndromes. A modern SLR camera, preprogrammable for exposure frequency and interval, was used for serial flash-lit photographs of the eyes during the course of darkness and near vision/accommodation reflexes. Measurement of pupillary diameter from these photograph…
