Dorsally displaced fractures of the distal radius - a study of preferred treatment options among UK trauma and orthopaedic surgeons.

Distal radius fractures are common, and surgeons have in their armament a variety of ways of treating them. In this study, 50 orthopaedic surgeons in the UK were shown five clinical scenarios and radiographs from patients with various fracture patterns of the distal radius, and were asked for their preferred management. There was a wide variation in the preferred treatment for each scenario presented. Across all of the cases, 52% of surgeons preferred to use a volar locking-plate compared with 21% who chose fixation with Kirschner wires. There was very little consensus among surgeons with rega

Successful return to work after surgical repair of fracture of the medial condyle of the distal femur in two working farm dogs.

CASE HISTORY: A 5-year-old male Heading dog (working Collie) and a 2-year-old female Huntaway each presented with non-weight-bearing lameness of a hindlimb after jumping or falling with the leg trapped in a gate or motorbike carrier. CLINICAL FINDINGS: Pain and swelling were localised to the stifle, and radiographs revealed a fracture of the medial condyle of the distal femur in each case. DIAGNOSIS: Medial unicondylar, intra-articular fractures of the distal femur. CLINICAL RELEVANCE: Open reduction and internal fixation provided an excellent outcome in each case. Open reduction was achiev

Accuracy of corrective osteotomies in fibular malunion: a cadaver model.

BACKGROUND: While incorrect length of a fibular fracture reduction can be measured by plain radiographs, accurate imaging of rotational deformities requires computed tomography (CT). Operative correction of fibular malrotation has not been accurately measured. The aim of this study was to evaluate the accuracy of operative correction of fibular malrotation. MATERIALS AND METHODS: Six pairs of formalin-fixed, lower leg cadaver specimens had shortening with additional internal or external rotation induced by segmental fibular resection and plate fixation. The deformity was measured by CT. Two e

Arthrodesis of the first metatarsocuneiform joint: a comparative study of fixation methods.

BACKGROUND: The Lapidus bunionectomy is a popular procedure for severe bunion deformity where metatarsus primus varus is equal to or exceeds 15 degrees. We evaluated a new locking compression plate which may improve outcomes with the Lapidus procedure. METHODS: Ten matched pairs of cadaver feet were used to compare the standard crossed 4.0-mm compression screw method of fixation to the LPS Lapidus plate. After performing the matched operations the cadaver constructs were stressed to failure using the INSTRON and Wavemaker software. RESULTS: The LPS Lapidus plate load to failure was 108 Nm wi

A transparotid transcutaneous approach for internal rigid fixation in condylar fractures.

Closed versus open reduction in condyle fractures is a dilemma that may torment the plastic surgeon. Although at present it is accepted that there are fractures that must be open reduced as when the middle cranial fossa or temporal fossa are involucrated, foreign body are in the joint capsule, lateral extracapsular deviation of condylar deviation, and open fractures. Risdon or retromandibular approaches are used for the treatment of fractures in the condyle neck and superior third of the lower ramus. When both approaches are used the correct placement of screws is very difficult for the followi

The treatment of isolated ulnar fractures in adults: a systematic review.

The treatment of isolated ulnar fractures remains controversial, with different authors recommending both surgical and non-surgical management. We undertook a systematic review of the current literature in order to assess whether any conclusions can be drawn. A total of 33 series involving 1876 patients were identified as suitable for analysis. Data from these papers were combined by method of treatment. The results of the non-operative treatment of minimally displaced ulnar fractures with a stable configuration were uniformly good. Below elbow plaster cast, functional brace and early mobilisa

Subclavian artery injury caused by a screw in a clavicular compression plate.

A 22-year-old man presented with left-arm ischaemia, 22 months after placement of a clavicular compression plate for a fractured left clavicle. Digital subtraction angiography revealed a distal subclavian artery injury which, during surgery, was found to be caused by a screw in the compression plate. Ligation of the subclavian artery followed by carotid-axillary bypass resulted in a symptom-free left arm.

[Recent fractures of the humeral shaft in adults. Role of surgical treatment using screwed plates. Apropos of 35 cases surgically treated].

Chronic pain disorders are often associated with negative emotions, including anxiety and depression. The central nucleus of the amygdala (CeA) has emerged as an integrative hub for nociceptive and affective components during central pain development. Prior adverse injuries are precipitating factors thought to transform nociceptors into a primed state for chronic pain. However, the cellular basis underlying the primed state and the subsequent development of chronic pain remains unknown. Here, we investigated the cellular and synaptic alterations of the CeA in a mouse model of chronic muscle pa

Luque interpeduncular segmental fixation of the lumbosacral spine.

Interpeduncular segmental fixation of the lumbar and lumbosacral spine with Luque screws and plates is becoming increasingly popular. In this retrospective study, 52 consecutive patients treated with this technique were followed for 16 to 44 months (average, 21). Indications for surgical arthrodesis included lumbar instability from degenerative disease (16 cases), spondylolisthesis (25 cases), pseudarthrosis (eight cases), fracture (two cases), and tumor (one case). Fusion with instrumentation spanned one level in nine patients, two in 38 patients, three in four patients, and four in one patie

Preclinical and clinical care of extremity lesions in polytraumatized patients.

The first and principal aim of the preclinical care of polytraumatized patients is preserving life through the restoration of respiration and circulation. Following this, the external hemorrhages should be treated. Early reduction of closed and open fractures relieves the pressure on the skin and surrounding soft tissues and prevents secondary transport-damage. Immobilization of the fractures, together with the proximal and the distal joints in a pneumatic splint, is carried out before transport. During the clinical phase, the operative stabilization of the fractures follows reanimation and li

Open reduction and fixation of proximal humeral fractures and fracture-dislocations.

Open reduction and internal fixation was employed in the treatment of 25 severely displaced fractures and fracture-dislocations of the proximal humerus. Our aims were accurate reduction and stable fixation to allow early mobilisation and to achieve full functional recovery. In 15 fractures an AO T-plate was used and in 10 a bent semitubular plate was employed as a blade plate. Excellent or satisfactory results were obtained in all six patients with two-part fractures involving the surgical neck; in four of the five patients with three-part fractures involving the surgical neck and tuberosities

Chronic recurrent multifocal osteomyelitis in children. A roentgenologic and scintigraphic investigation.

Thirty cases of chronic recurrent multifocal osteomyelitis have been reported in the literature. A radiologic evaluation of thirty-one additional cases, 25 of whom also underwent bone scintigraphy, is presented. Bone biopsy specimens were obtained in 16 patients. Most lesions were located in the metaphyses of the long bones adjacent to the physis and had a characteristic, probably pathognomonic appearance. Extension into the epiphyses was rare. Lesions in the vertebral bodies, clavicle and pelvis had possibly a less specific radiographic appearance. Lesions in short tubular bones were non-spec

Ogden plate and other fixations for fractures complicating femoral endoprostheses.

Treatment of femoral shaft fractures complicating endoprostheses remains controversial. Nineteen such fractures were treated with open reduction and internal fixation using the Ogden plate. This modified plate allows for proximal fixation with heavy-duty Parham bands and distal fixation with screws. Sixteen of 19 patients healed their fractures in an average of 3.5 months. Two fractures developed delayed unions, one resulted in non-union. At final follow-up examination, results were rated as excellent (12), satisfactory (5), and poor (2). The procedure is not technically difficult. It can be a

Operative management of ipsilateral fractures of the hip and femur.

Thirteen patients with ipsilateral hip and femoral shaft fractures are discussed. In nine patients, the femoral shaft fracture was managed by a combination of dynamic compression plating with a medial cancellous bone graft and either multiple cancellous screws or a dynamic hip screw for the proximal fracture (Group I). Four patients had Ender pin fixation (Group II). In Group I, both fractures were united within 16 weeks in all patients, and at least 90% of normal hip and knee motion was regained. Nonunions developed in two of the four Group II patients. Immediate separate fixation of ipsilate

[Fractures of the central midface and frontal skull base--surgical management--postoperative care--possibility of complications].

Mini-plate osteosynthesis is an osteoplastic procedure that achieves secure stabilisation of the midface even in extensive fractures of the midface and the floor of the anterior cranial fossa. This procedure can be considered as a satisfactory operative treatment both from a cosmetic and from a functional point of view. Nevertheless the partial loss of previous mucosa, and the development of granulation tissue and scar tissue can impede ventilation and drainage of the paranasal sinuses and induce recurrent inflammation. In the last five years we have treated 73 patients with extensive fracture

Internal fixation for atlanto-axial instability in children.

A new and simple method for the operative treatment of atlanto-axial instability in children is described. The technique involves a metal rectangle wired in place between the occiput and cervical spine to act as an internal fixation device. Onlay bone grafts subsequently produce bony fusion. A synchronous decompression of the cervical canal can be performed and there is no need for complex or prolonged external fixation.

Preoperative skeletal traction in Legg-Perthes disease.

In four of 11 patients with Legg-Perthes disease for which a varus femoral osteotomy was planned, routine split Russell traction failed to produce complete preoperative containment. Preoperative skeletal traction was used to distract the joint, displace the extruded lateral physis below the acetabular margin, and then reintroduce the femoral head into the acetabulum with progressive abduction. The femoral heads were rated postoperatively as good (two cases), fair (two cases), and poor (one case). Preoperative skeletal traction is an adjunctive treatment method for patients with "hinge abductio

[The influence of primary closed reduction on the incidence of posttraumatic femoral head necrosis (author's transl)].

24 patients with fractures of the femoral neck were younger than 65. Eight of them were operated on the day of the accident. The fractures of the other 16 patients were immediately reduced, the leg placed in traction and the operation carried out between 2 and 8 days later. Osteosynthesis was performed by means of cancellous bone screws without opening the joint. If this internal fixation was stable, the incidence of femoral head necrosis was reduced to one tenth. For fractures of the group Pauwels III this method was not suitable. Early reductions seems to be of more importance for the final
