Pulmonary arteriovenous malformations in hereditary hemorrhagic telangiectasia: a series of 126 patients.

Hereditary hemorrhagic telangiectasia (HHT) is a genetic disorder characterized by epistaxis, telangiectasia, and visceral vascular manifestations. Infectious and ischemic central nervous system (CNS) manifestations due to embolism through pulmonary arteriovenous malformations (PAVMs) represent the main causes of morbidity. To improve the phenotypic characterization of HHT with PAVM, we conducted a retrospective multicenter study of patients with HHT and at least 1 PAVM detected by chest computed tomography (CT) and/or pulmonary angiography, with particular attention to CNS and infectious mani

Presence of Collaterals from the Contralateral Hemisphere on Preoperative Angiogram Predicts Failure of Encephalo-Duro-Arterio-Synangiosis (EDAS) in Adult Patients with Moyamoya Disease.

BACKGROUND AND PURPOSE: Moyamoya disease (MMD) is characterized by chronic progressive stenosis of intracranial vessels and subsequent formation of abnormal collateral vessel networks. Indirect revascularization techniques, such as encephalo-duro-arterio-synangiosis (EDAS), promote angiogenesis to restore perfusion but have variable success rates. This study aimed to identify clinical and radiographic predictors of successful collateral vessel in-growth after EDAS, emphasizing the role of contralateral interhemispheric collaterals. MATERIALS AND METHODS: We conducted a single-center retrospec

Perfusion Quantification using Arterial Spin Labeling Magnetic Resonance Imaging after Revascularization for Moyamoya Disease.

The hemodynamics in the brains of individuals with Moyamoya disease are complex and variable. Cerebral revascularization is an important treatment when hemodynamics are severely damaged. It's of great value to accurately quantify blood perfusion of different functional brain regions for better postoperative prognosis. In this study, we developed methods to segment territory of middle cerebral arteries (MCA) and its functional brain regions based on T1 and arterial spin labeling (ASL) imaging, absolute and normalized cerebral blood perfusion (CBF) were calculated for target regions-of-interest

Higher Plaque Burden of Middle Cerebral Artery Is Associated With Recurrent Ischemic Stroke: A Quantitative Magnetic Resonance Imaging Study.

Background and Purpose- This study aims to investigate the association between the characteristics of atherosclerotic plaques of middle cerebral artery and recurrent ischemic stroke using magnetic resonance vessel wall imaging. Methods- One hundred and five patients with ischemic stroke attributed to middle cerebral artery plaque underwent high-resolution black-blood magnetic resonance vessel wall imaging. They were divided into group 1, with the first episode of acute stroke (imaging within 4 weeks of stroke, n=44); group 2, with recurrent acute stroke (n=29); and group 3, with chronic stroke

Cognitive Impairment Before Intracerebral Hemorrhage Is Associated With Cerebral Amyloid Angiopathy.

BACKGROUND AND PURPOSE: Although the association between cerebral amyloid angiopathy (CAA) and cognitive impairment is increasingly recognized, it is not clear whether this is because of the impact of recurrent intracerebral hemorrhage (ICH) events, disruptions caused by cerebral small vessel damage, or both. We investigated this by considering whether cognitive impairment before ICH was associated with neuroimaging features of CAA on magnetic resonance imaging. METHODS: We studied 166 patients with neuroimaging-confirmed ICH recruited to a prospective multicentre observational study. Preexis

A novel route of revascularization in basilar artery occlusion and review of the literature.

Ischemia of the basilar artery is one of the most devastating types of arterial occlusive disease. Despite treatment of basilar artery occlusions (BAO) with intravenous tissue plasminogen activator, antiplatelet agents, intra-arterial therapy or a combination, fatality rates remain high. Aggressive recanalization with mechanical thrombectomy is therefore often necessary to preserve life. When direct access to the basilar trunk is not possible, exploration of chronically occluded vessels through collaterals with angioplasty and stenting creates access for manual aspiration. We describe the firs

Using the snare system to cross the acute-angled vertebrobasilar junction in treating posterior inferior cerebellar artery aneurysm with the stent-assisted method via a retrograde approach. A technical note.

Retrograde stenting via the contralateral vertebral artery (VA) is a safe and effective treatment for posterior inferior cerebellar artery (PICA) aneurysm. Many methods, including tip shaping and the looping technique, have been attempted as ways to cross the vertebrobasilar (VB) junction. Here, we introduce an alternative method using a Snare system to overcome the acute-angled VB junction after repeated failures using other techniques. The Snare system was navigated to the proximal basilar artery via the ipsilateral VA. A guidewire was introduced in the contralateral VA and gently advanced t

Entrapment of a micro-guidewire during stenting of basilar stenosis.

Entrapment of a microwire related to intracranial endovascular therapy is an uncommon complication. A 64-year-old man with symptomatic basilar artery stenosis was treated with stenting. A 300 cm Synchro microwire became trapped in the P1 segment of the left posterior cerebral artery during the procedure. The decision was made to leave the microwire in place, considering that aggressive retrieval procedures could cause injury to the cerebral vasculature. The entrapped microwire was later inadvertently dislocated and removed while pulling the microwire back into the femoral artery with a looped

[Efficacy of fusion image for the preoperative assessment of anatomical variation of the anterior choroidal artery].

We report two cases with internal carotid artery(ICA)aneurysms, in which fusion image effectively indicated the anatomical variations of the anterior choroidal artery (AchoA). Fusion image was obtained using fusion application software (Integrated Registration, Advantage Workstation VS4, GE Healthcare). When the artery passed through the choroidal fissure, it was diagnosed as AchoA. Case 1 had an aneurysm at the left ICA. Left internal carotid angiography (ICAG) showed that an artery arising from the aneurysmal neck supplied the medial occipital lobe. Fusion image showed that this artery had a

Catastrophic intracranial hemorrhage as a presenting feature of juvenile polyarteritis nodosa.

Intracranial hemorrhage has been rarely reported during the course of polyarteritis nodosa. We describe a 6-year-old boy who presented with fever, altered sensorium, skin rash, hypertension, and catastrophic intracranial hemorrhage. After surgical evacuation of the intracranial hematoma, he underwent a computerized tomography angiogram that showed narrowing of the right anterior cerebral artery. Skin biopsy showed small vessel vasculitis. Nerve conduction studies were suggestive of mononeuritis multiplex. He was diagnosed as polyarteritis nodosa and managed with immunosuppressants, to which he

Intra-aneurysmal Onyx embolization for distal aneurysms of the cerebellar arteries.

BACKGROUND: Distal cerebellar artery aneurysms are difficult to treat both surgically and endovascularly. The purpose of this study is to assess the efficacy and safety of intra-aneurysmal Onyx embolization of these lesions with parent artery preservation. METHODS: Ten consecutive patients harboring 10 distal cerebellar aneurysms were treated with intra-aneurysmal Onyx embolization. Locations of the aneurysms were as follows: superior cerebellar artery in three patients, anterior inferior cerebellar artery in two, and posterior inferior cerebellar artery in five. The technical feasibility of

Persistent hypoglossal artery--an increased risk for intracranial aneurysms?

A 53-year-old hypertensive female smoker presented with a WFNS grade II subarachnoid haemorrhage. Angiography showed a left persistent hypoglossal artery with multiple intracranial aneurysms. She underwent successful coil embolisation of the ruptured aneurysm. We reviewed the literature on persistent hypoglossal artery and found 26% of them have associated intracranial aneurysms. This controversial topic is discussed.
