BACKGROUND: Cardiogenic shock due to severe aortic regurgitation in patients with left ventricle assist devices is a life threatening condition. Here, we consider transcatheter aortic valve implantation as a treatment option.
METHODS AND RESULTS: A patient with a left ventricle assist device was presented to us with cardiogenic shock due to severe aortic regurgitation. We successfully implanted a transcatheter aortic valve in emergency setting. The patient recovered and underwent cardiac transplantation three months afterwards. We performed a systematic literature review and identified 10 cas

The authors describe the case of a simultaneous mitral bioprosthesis hypertrophic scaring and native aortic valve fibrosis during benfluorex therapy in a 40-year-old woman. Four years before, she underwent a mitral valve replacement after the diagnosis of mitral regurgitation during benfluorex treatment (150 mg/day). This drug was reintroduced postoperatively. She presented with exercise and sometimes resting dyspnoea. The bioprosthesis and aortic valves exhibited similar histopathological lesions. Thickening and plaque deposits made by smooth muscle alpha actin- and vimentin-positive cells in

AIM OF THE STUDY: We retrospectively analyzed long-term outcome of concomitant mitral valve repair and aortic valve replacement.
METHODS: From 1996 to 2009 we performed mitral valve plasty with aortic valve replacement in 50 patients. Clinical and echocardiographic data were obtained from computer database and hospital records. Missing data were obtained through mailed questionnaire. We evaluated hospital mortality, long-term survival, thromboembolic and hemorrhagic complications and TR of 3+ on follow up echocardiography.
RESULTS: Four patients who had previously undergone aortic valve surg

With the increasing survival of heart transplant recipients, the risk for acquired allograft disease will be augmented. Severe mitral valve insufficiency after heart transplantation is rarely described. Similar to the general population, a degenerative process may affect the allograft mitral valve. Herein are reported two cases of severe mitral valve degeneration necessitating mitral valve replacement long term after heart transplantation. The pathophysiology and operative strategy for these patients are discussed.

The dimensions of the aortic valve components condition its ability to prevent blood from flowing back into the heart. While the theoretical parameters for best trileaflet valve performance have already been established, an effective approach to describe other less optimal, but functional models has been lacking. Our goal was to establish a method to determine by how much the dimensions of the aortic valve components can vary while still maintaining proper function. Measurements were made on silicone rubber casts of human aortic valves to document the range of dimensional variability encounter

Valvular heart disease (predominantly aortic and mitral valve disease) has been well documented in patients with osteogenesis imperfecta. Twenty-two prior reports of valve replacement and/or repair have been published. Mortality from cardiac surgery in these patients has been high, particularly in those receiving double valve prostheses. Here, a patient is described with advanced cardiovascular disease who underwent successful aortic and mitral valve replacement. This represents the second reported survival of double valve replacement in osteogenesis imperfecta and a reduced mortality rate for

The pathophysiology of mitral and valvular heart disease is presented with an emphasis on the relationship of these conditions to pregnancy. Management options are discussed. Special attention is directed to patients who have prosthetic valves in place and who become pregnant. The care of this group of patients may be difficult, and treatment strategies are presented.

Between 1961 and 1984, 91 patients underwent simultaneous triple valve replacement at the Mayo Clinic. Of the 273 prosthetic valves used, 77% were Starr-Edwards. Perioperative (30 day) mortality was 24% to 27% between 1962 and 1974 and 7% between 1975 and 1983 (p = .17). In patients with NYHA class IV symptoms, perioperative mortality was 44%, and in those with milder symptoms, it was 8% (p less than .0001). The median follow-up was 7.5 years (range, 6 weeks to 20 years). Cumulative survival, which was calculated taking into consideration perioperative mortality, was 64% at 1 year, 55% at 5 ye

Two hundred one patients over the age of 65 underwent valvular heart surgery with a hospital mortality of 5.5% and a late mortality of 18.4%. One hundred forty-one patients underwent isolated valve replacement (90 AVR, 51 MVR) with a hospital mortality of 1.5% (AVR 1%, MVR 2%). Multiple procedures carried a significantly higher hospital mortality (16%). Analysis of hospital and late deaths does not suggest that age alone should be accepted as a decisive factor in selection for surgery. There is a significantly higher late mortality in those who have had MVR (30%) compared with those having had

589 patients were followed up between 1972 and 1977 after aortic (285) or mitral valve (304) replacement. Three hundred and thirty patients had a ball valve (Starr-Edwards) and 259 a disc valve (Björk-Shiley). The average follow up was 37 months for the mitral patients and 40 months for the aortic. The results were analyzed by statistical methods based on multifactor analysis (cluster) in order to test the homogeneity of the different groups of patients. Survival and embolism were followed by actuarial rates. The survival rate between AVR patients at 72 months was 93 per cent for the Björk and

Eight patients have had thrombotic obstruction of a prosthetic valve since 1971, six mitral valves and two aortic. All eight patients had a Björk-Shiley valve. During the same period 159 Björk-Shiley valves were placed, 85 in the mitral and 74 in the aortic area. This represents a valve thrombotic occlusive incidence of 4.4 percent in our series, 5.9 percent of mitral and 2.7 percent of aortic prostheses. Among the six patients with mitral prostheses only one survived. The two patients with occluded aortic valves survived. The onset of symptoms was very abrupt in most patients and progressed v

BACKGROUND: There is a lack of comparative data on transcatheter aortic valve implantation (TAVI) in degenerated surgical prostheses (valve-in-valve [ViV]).
AIMS: We sought to compare outcomes of using two self-expanding transcatheter heart valve (THV) systems for ViV.
METHODS: In this retrospective multicentre registry, we included consecutive patients undergoing transfemoral ViV using either the ACURATE neo/neo2 (ACURATE group) or the Evolut R/PRO/PRO+ (EVOLUT group). The primary outcome measure was technical success according to Valve Academic Research Consortium (VARC)-3. Secondary outco

Transcatheter aortic valve replacement (TAVR) is increasingly considered as an alternative to surgical aortic valve replacement (SAVR) for low-risk patients with aortic stenosis. However, its long-term efficacy remains uncertain. This study aimed to compare clinical outcomes and procedural complications of TAVR versus SAVR in low-risk patients with aortic stenosis. We updated our 2019 systematic review by searching MEDLINE, EMBASE, and Cochrane Central (May 2019-April 2025) for randomized controlled trials (RCTs) comparing TAVR and SAVR in low-risk patients with aortic stenosis. We extracted o

Transcatheter aortic valve implantation (TAVI) has evolved from a last resort for inoperable aortic stenosis into the preferred bioprosthetic strategy across much of the surgical-risk spectrum for anatomically suitable patients. As it expands into younger and lower-risk populations, the central question is no longer whether TAVI can be performed, but how to choose the first valve intervention so that future options remain open. This clinical perspective focuses on practical decision-making: which patients are better served by TAVI or surgical aortic valve replacement (SAVR), how computed tomog
