Need for frequent dilations after magnetic sphincter augmentation: an assessment of associated factors and outcomes.

BACKGROUND: Dysphagia is the most common complaint after magnetic sphincter augmentation (MSA), with nearly one-third of patients requiring … ophagia (OR 7.18; p = 0.042), LES resting pressure > 45 mmHg (OR 28.5; p = 0.005), and ≥ 10% failed swallows (OR 23.5; p < 0.001). CONCLUSIONS: The need for frequent dilations after MSA is a marker for poor symptom control, dissatisfaction, and device removal. Patients with preoperative odynophagia, high LES pressures, and poor esophageal motility should …

Relationship between distension-contraction waveforms during esophageal peristalsis: effect of bolus volume, viscosity, and posture.

High-resolution esophageal manometry (HRM) in its current form assesses only the contraction phase of peristalsis. Degree of esophageal dist … impedance recordings of 50 healthy subjects with no esophageal symptoms were analyzed. Eight to ten swallows of 5 and 10 mL of 0.5 N saline and a viscous bolus were recorded in the supine and Trendelenburg positions. Custom-built computer software generated the distension-contraction plots and numerical data of the amplitudes of distension (cross-sectional …

A description of an 'obesogenic' eating style that promotes higher energy intake and is associated with greater adiposity in 4.5year-old children: Results from the GUSTO cohort.

Recent findings confirm that faster eating rates support higher energy intakes within a meal and are associated with increased body weight a … age to measure acute energy intake. Videos were coded for three eating behaviours (bites, chews and swallows) to derive a measure of eating rate (g/min) and measures of eating microstructure: eating rate (g/min), total oral exposure (min), average bite size (g/bite), chews per gram, oral exposure per bite (s), total bites and proportion of active to total …

Secondary oesophageal peristalsis in patients with non-obstructive dysphagia.

Secondary peristalsis was investigated in 30 patients with non-obstructive dysphagia and 20 age matched controls. Oesophageal motility was recorded at 3 cm intervals along the oesophageal body. Primary peristalsis was tested with 5 ml water swallows. Secondary peristalsis was stimulated with 10 ml boluses of air and water injected in the mid-oesophagus and by distensions (5 seconds duration) with a 3 cm balloon at the same level. Primary peristalsis was normal in 19 of the 20 control subjects and in nine of the 30 patients with dysphagia; 11 patients had diffuse spasm and 10 had non-specific a…

Heat shock protein gene expression varies among populations but does not strongly track recent environmental conditions: implications for biomarker development.

INTRODUCTION: Global temperatures are rising, and scientists are mobilizing to uncover which birds are most affected by the problem of heat. Heat shock proteins (HSPs), for example, can shed light on this issue because they prevent damage and promote recovery from heat. However, few studies have investigated the relationship between HSPs and heat outside of experimental contexts. Here, we ask whether natural variation in HSP gene expression can serve as a biomarker of recent ambient conditions in wild nestling tree swallows (Tachycineta bicolor). METHODS: We focused on HSP90AA1 because this H…

Laryngeal Manifestations of Rheumatic Disease.

Rheumatologic diseases are systemic, immune-mediated disorders that frequently involve the larynx, producing manifestations that range from subtle dysphonia to life-threatening airway obstruction. Laryngeal involvement may precede systemic disease, progress despite adequate systemic control, or be the sole presenting feature of active disease. Diagnosis relies on flexible laryngoscopy with disease-specific serology and biopsy when indicated. Corticosteroids, immunosuppressants, and biologics such as rituximab form the foundation of medical therapy, while surgery ranges from serial endoscopic d

"Heart Sign" with Double Aortic Arch.

Double aortic arches are rare congenital malformations resulting from persistence of the right and left aortic arches, as opposed to normal regression of the left aortic arch. These vascular malformations can cause vascular rings since they encompass the trachea and esophagus, and lead to swallowing difficulties or respiratory difficulties from extrinsic compression. Typically, these are diagnosed in childhood but rarely may present in adults. In the presence of symptoms, which require formal gastrointestinal and pulmonary evaluation, surgical resection of the nondominant arch is typically rec

[Case of multi-dimensional cervical disorder treated with acupuncture based on jingjin theory].

One case of multi-dimensional cervical disorder was diagnosed and treated using jingjin (sinew/muscle) theory. According to the patient's symptoms, guided by jingjin theory, this case was diagnosed as the jingjin (muscle region) disorder of foot-taiyang. On the distribution of the muscle region of foot-taiyang, the distal junctions of the muscle region, Kunlun (BL60) and Feiyang (BL58), as well as the knotted sites, Wangu (GB12), Tianzhu (BL10) and Cuanzhu (BL2) were the keys in the distal acupuncture technique along meridian. After three treatments, the movement of neck region was recovered,

[Pre-orthodontic orofacial myofunctional therapy].

Pre-orthodontic orofacial myofunctional therapy is regularly prescribed in young children to correct the function of the tongue as a causative factor of an anterior open bite. Tongue thrusting is thereby identified as a harmful habit that needs to be unlearned. However, the concept that soft tissue dysfunction is the primary cause of orthodontic abnormalities does not appear to be valid. The tongue adapts its function to the abnormal anatomy, which has arisen for other reasons. If the tongue would not do this, food and drink would be squeezed out rather than transported to the oesophagus. Teac

Consequences of hyposalivation in relation to cancer treatment and early management of radiation-induced caries: case reports.

Given the prevalence of head and neck carcinoma and the salivary changes induced by the oral side effects of radiotherapy, such patients are at higher risk/susceptibility of developing dental caries. Radiation-caries is often under-treated in patients undergoing cervicofacial radiotherapy, yet these lesions can increase the risk of osteoradionecrosis due to necessary subsequent dental extractions. Moreover, xerostomia is accompanied by difficulties with speech, chewing and swallowing. The prevention and/or early management of radiation-induced carious lesions is essential in preventing oral co

Clinical Evaluation of the Child with Eosinophilic Esophagitis.

The diagnosis of eosinophilic esophagitis (EoE) is based on clinical symptoms of esophageal dysfunction and eosinophil predominant esophageal inflammation. Clinical symptoms in children with EoE vary based on age and may be nonspecific. EoE has a male predominance with the majority having comorbid atopic disorders. At present, treatment options include medications (proton pump inhibition, swallowed topical steroids), dietary therapy or biologic therapy (dupilumab, approved for those ≥12 years of age). Outside of EoE in the context of oral immunotherapy, EoE is typically chronic requiring lifel

A multidisciplinary collaborative diagnosis and rehabilitation program for dysphagia in general hospitals.

Dysphagia is a common complication of various clinical conditions, with an increased incidence as age advances. Complications such as aspiration, malnutrition, and aspiration pneumonia caused by dysphagia significantly affect the overall treatment outcomes of patients. Scholars both domestically and internationally are increasingly focusing on early rehabilitation for dysphagia. This article summarizes common conditions causing dysphagia, clinical manifestations, complications, screening assessment, diagnosis, rehabilitation, and nutritional support related to dysphagia. It emphasizes the arri

Intestinal Obstruction Due to Ingested Olive Pits.

Intestinal obstruction after ingestion of foreign bodies is a rare condition. A wide variety of ingested foreign bodies has been reported as a cause of mechanical small bowel obstruction, with ingested fruit pits being rarely reported as causes of intestinal obstruction. Here, we report the case of a 70-year-old female diagnosed with intestinal obstruction due to swallowed olive pits tightly impacted in the distal ileum. A conclusive diagnosis was made preoperatively by computed tomography of the abdomen. After initial conservative treatment, the patient underwent laparotomy because of persist

Repair of type II paraesophageal hernia with Nissen fundoplication in a patient with von Willebrand disease and spondylolisthesis - a clinical case report.

Hiatal hernias continue to be fairly common in clinical practice. However, the variety of different symptoms presented by patients may hinder establishing the ultimate diagnosis. Nevertheless, currently, the diagnosis of hiatal hernia can be easily established, based on barium swallow radiography. We would like to present a clinical case report of a patient with complex medical history, including von Willebrand disease, degenerative spinal disease, and chronic sinusitis, who was finally diagnosed with hiatal hernia and treated with a standard laparoscopic Nissen fundoplication. Our case focuse

Achalasia: investigation and management.

Achalasia, characterised by the absence of peristalsis and failure of relaxation of the lower oesophageal sphincter, is an uncommon degenerative condition that results in dysphagia. If left untreated it can lead to aspiration, oesophageal perforation, oesophagitis and malnutrition. It has a range of immune, allergic, viral and genetic aetiological causes. Successful diagnosis relies on the use of oesophagogastroduodenoscopy, barium swallow and oesophageal manometry to characterise the severity of the disease and to rule out underlying malignancy. Although no treatment can reverse the degenerat

Isolated Posterior Mediastinal Thyroid Nodule Simulating Esophageal Pathology: A Multi-Modality Diagnosis.

Posterior mediastinal goiter is not a common cause of dysphagia, and symptoms can simulate esophageal malignancy. This case report highlights two critical clinical aspects. First, the patient's symptoms of gradually worsening dysphagia to solids and liquids, odynophagia, and hoarseness of voice secondary to retrosternal thyroid nodule extension can simulate esophageal malignancy. Second, a barium swallow study can effectively rule out esophageal pathology even though more advanced studies, like High-resolution computed tomography (HRCT), are inconclusive. We present a unique case of isolated p

Interdisciplinary care of children with diffuse midline glioma.

Diffuse Midline Glioma (DMG) which includes Diffuse Intrinsic Pontine Glioma (DIPG) is an infiltrative tumor of the midline structures of the central nervous system that demonstrates an aggressive pattern of growth and has no known curative treatment. As these tumors progress, children experience ongoing neurological decline including inability to ambulate, swallow and communicate effectively. We propose that optimal care for patients with DMG should involve a specialized team experienced in caring for the multifaceted needs of these patients and their families. Herein we review the roles and

Colon injury and gastrocolocutaneous fistula complication of percutaneous endoscopic gastrostomy.

Percutaneous endoscopic gastrostomy (PEG) insertion is an effective endoscopic procedure for enteral feeding in patients with difficulty swallowing. Many postprocedural complications have been reported after the PEG procedure. The displacement of the transverse colon over the anterior gastric wall can predispose the patient to colonic injury and fistulae during PEG placement. Gastrocolonic fistulas represent a serious but rare complication post PEG placement. We report a 90 year old man with a background of multiple comorbidities and high preoperative risk who developed a gastrocolocutaneous f

Massive upper gastrointestinal bleeding due to a gastric ulcer in a newborn.

Usually, swallowing of maternal blood is the cause of bloody vomiting in healthy term newborns. Other reported causes include gastritis, esophagitis, gastric ulcer, and duodenal ulcer. We report a newborn girl born by cesarean at 372/7  weeks who had hematemesis on postnatal day 1 and hematochezia on day 2. An erythrocyte transfusion was given on day 3. Gastroscopy performed on day 4 showed multiple gastric ulcers. Antacid treatment was given. On day 12, the newborn had a good general condition and stable vital signs and was discharged. Gastric ulcer needs to be considered as the etiology of h
