Pathological fracture complicating epidural local anaesthetic opioid infusion for cancer pain.

Two cases of severe bone pains from metastatic cancer, without clinical or radiological evidence of fractures on admission, are presented. Pain control and mobilization were achieved initially with local anaesthetic-opioid epidural infusion. Subsequent loss of analgesia prompted a re-examination, which revealed pathological fractures of the femur. Internal fixation of the fractures resulted in good control such that strong opioids were no longer required.

[The treatment concept and results of peri-/sub-prosthetic fractures].

The treatment of choice in ipsilateral femoral shaft fractures in patients with hip prosthesis depends on the location of the fracture lines and the observation of loosening of the prosthesis. Either a change of prosthesis, if necessary in combination with refixation of the greater trochanter or simple open reduction with internal fixation (ASIF) along with autologous and/or homologus spongiosa are the used techniques. The rate of complications in peri- or subprothetic fractures is high. Lack of motion in hip and knee joint occurs frequently.

Early versus delayed fixation of maxillofacial fractures in polytrauma: a systematic review.

BACKGROUND: Maxillofacial fractures are common in polytrauma, and these patients often require intensive care unit (ICU) admission until delayed definitive surgical management. This review aims to evaluate the scientific evidence on timing of definitive internal fracture fixation of maxillofacial fractures in polytrauma. We hypothesise that delayed definitive surgery is associated with better outcomes. METHODS: This systematic review was conducted in accordance with the PRISMA protocol. MEDLINE, EMBASE, Scopus, CINAHL and PubMed were searched from inception to July 2025 for articles in English

Tension Control UCL Reconstruction With Internal Brace Augmentation.

BACKGROUND: Ulnar collateral ligament (UCL) rupture is a common injury, particularly in throwing athletes. It occurs either in the acute setting or due to repetitive microtrauma. UCL reconstruction has been a successful treatment with proper patient selection. A number of different surgical techniques have been described in the literature. We developed a novel surgical technique using internal brace augmentation to address the 2 most common causes of failure: suture failure and bone tunnel fracture. INDICATIONS: To provide information about this novel UCL reconstruction technique. TECHNIQUE

Retrospective cohort trial protocol of screw fixation compared with hemiarthroplasty for displaced femoral neck fractures in elderly patients.

BACKGROUND: There is limited evidence for the evaluation of the efficacy and safety of the hemiarthroplasty versus screw fixation in elderly patients with the displaced femoral neck fractures. Our current investigation aimed at assessing the complications, functional outcome, and revision rate of the patients (over 65 years old) who received internal fixation or hemiarthroplasty via a same senior surgeon. METHODS: A retrospective study was conducted on elderly patients with displaced femoral neck fractures from May 2014 to February 2018. The current study was carried out at our hospital and i

[Open reduction and plate fixation (ORIF LCP) for treatment of bilateral calcaneal fractures].

PURPOSE OF THE STUDY: Calcaneal fractures are frequent injuries which may interfere with the patient's daily activities for a long time. They usually occur as the result of an axial load causing impaction of the talus into the calcaneus. They can also be bilateral. Falls and jumps from height are the most frequent causes. MATERIAL AND METHODS: Between August 2005 and December 2011, the method of open reduction and internal fixation (ORIF) with a calcaneal locking compression plate (LCP) was used to treat 98 patients with 114 calcaneal fractures. Of these, 16 were bilateral fractures found in

The "Bag of Bones" Treatment of Comminuted Intra-articular Distal Humerus Fractures in the Elderly.

BACKGROUND: Extensively comminuted intra-articular distal humerus fractures in the elderly present a challenging therapeutic dilemma. The purpose of this study was to investigate the results of nonoperative treatment of these fractures in a select subset of patients. METHODS: Patients treated with nonoperative management for a comminuted intra-articular distal humerus fracture between 2007 and 2018 were reviewed. Patients were administered 3 elbow-specific functional outcomes instruments. RESULTS: A total of 8 patients (2 men, 6 women) were treated with brief immobilization followed by early

Minimally-invasive treatment of three- and four-part fractures of the proximal humerus in elderly patients.

The surgical treatment of three- and four-part fractures of the proximal humerus in osteoporotic bone is difficult and there is no consensus as to which technique leads to the best outcome in elderly patients. Between 1998 and 2004 we treated 76 patients aged over 70 years with three- or four-part fractures by percutaneous reduction and internal fixation using the Humerusblock. A displacement of the tuberosity of > 5 mm and an angulation of > 30 degrees of the head fragment were the indications for surgery. Of the patients 50 (51 fractures) were available for follow-up after a mean of 33.8 mon

[Current trends of the treatment of the femoral neck fractures in seniors].

INTRODUCTION: Proximal femur fracture of the seniors is often "omen" for their life. These fractures are the most frequent cause of acute hospitalization of the aged and on the contrary to previous years the procedures are considered as emergent - life saving. OBJECTIVE: The aim of the study was to find out which of the contemporary used surgical procedures for the treatment of unstable femoral neck fractures at the Department of Trauma Surgery in Kosice yield in the lowest number of failures and postoperative complications. Subsequently we have analyzed changes in surgical treatment of these

Arthroscopic fixation of the fractures of the intercondylar eminence via transquadricipital tendinous portal.

We describe a new technique for arthroscopic reduction and internal fixation of fractures of the intercondylar eminence of the tibia. In this technique cannulated screws are placed through a new portal (transquadricipital tendinous) instead of anteromedial arthroscopic portal. Twelve patients who were treated with this technique for displaced types II and III fractures of the intercondylar eminence of the tibia were reviewed after a mean of 49 months. At follow-up all patients had excellent or good results, without any case of nonunion of the fracture or related complications, such as function

Four-part valgus impacted fractures of the proximal humerus.

There is a specific type of displaced four-part fracture of the proximal humerus which consists of valgus impaction of the head fragment; this deserves special consideration because the rate of avascular necrosis is lower than that of other displaced four-part fractures. Using either closed reduction or limited open reduction and minimal internal fixation, 74% satisfactory results can be achieved in this injury.

Angiographic assessment and control of potential operative hemorrhage with pathologic fractures secondary to metastasis.

Two cases are reported, which demonstrate the contribution of angiography to the operative management of neoplastic, pathologic fractures. The degree of fracture site neovascularity was assessed by pre-operative arteriography. Selective trans-catheter gelatin-foam embolization was then employed to markedly reduce or obliterate the arterial supply to lesions shown to be richly vascular. Open reduction and internal fixation was subsequently accomplished with minimal operative hemorrhage.

An assessment of outcomes with intramedullary fixation of fractured ribs.

BACKGROUND: Surgical management of fractured ribs with internal fixation is an increasingly accepted therapy. Concurrently, specific rib fixation prostheses are being developed which should improve results and minimise hardware and rib/splint construct failures. The Synthes titanium intramedullary splint lends itself to difficult to access areas such as posterior rib fractures and fractures under the scapula. We analyse a case series of patients in whom this rib fixation prosthesis has been used. METHODS: Fifteen patients received 35 intramedullary splints. Follow up at 3 and 6 months was per

External fixation in the treatment of tibial pilon fractures: comparison of two frames in torsion.

BACKGROUND: Ankle spanning external fixation has become the initial treatment of choice for complex tibial pilon fractures. Many fixator designs exist, but their biomechanical performance has not been studied extensively for this application. The goal of the present study was to compare the torsional performance of two commercially available frames, the Orthofix XCaliber and the Howmedica Hoffmann II. METHODS: The XCaliber and the Hoffmann II were each applied to six fresh cadaver lower extremities and were loaded in a materials testing machine. Strain gauges were attached to the anteromedial
