Clinical outcomes and static and dynamic assessment of foot posture after lateral column lengthening procedure.

BACKGROUND: Lateral column lengthening (LCL) has been shown to radiographically restore the medial longitudinal arch. However, the impact of LCL on foot function during gait has not been reported using validated clinical outcomes and gait analysis. METHODS: Thirteen patients with a stage II flatfoot who had undergone unilateral LCL surgery and 13 matched control subjects completed self-reported pain and functional scales as well as a clinical examination. A custom force transducer was used to establish the maximum passive range of motion of first metatarsal dorsiflexion at 40 N of force. Foot…

First-line ibrutinib for Bing-Neel syndrome.

The authors present a case of an elderly man with a history of Waldenstrom macroglobulinaemia in remission who presented with progressively worsening gait abnormalities and falls for several months. His examination was notable for bilateral lower extremity weakness and an unsteady gait. Brain and spinal MRI showed focal leptomeningeal enhancement in the brain and spinal column. Lumbar puncture was performed and cerebrospinal fluid flow cytometry demonstrated a monoclonal CD5/CD10-negative, CD20-positive B-cell lymphocyte population consistent with a diagnosis of Bing-Neel syndrome. He was star…

Evolution of walking ability after soft tissue surgery in cerebral palsy patients: what can we expect?

Eleven patients with spastic cerebral palsy were evaluated preoperatively, and 3 and 9 months postoperatively after soft tissue surgery. Evaluation included clinical examination, the Functional Mobility Scale questionnaire, and instrumented gait and center of mass trajectory analysis. A decrease in time-distance parameters after 3 months was followed by progress in all parameters at 9 months postoperatively. Push-off range of ankle motion decreased after surgery and was not restored to preoperative level until 9 months later. The center of mass vertical displacement improved significantly. The…

Differentiation of high-functioning autism and Asperger's disorder based on neuromotor behaviour.

Autism and Asperger's disorder (AD) are characterised by impairments in social interaction, stereotypic behaviours or restricted interests. Although currently listed as distinct clinical disorders, the validity of their distinction remains controversial. This study examined gait in children with autism and AD. Eleven children with high-functioning autism and eleven children with AD completed a series of walking tasks. Results indicated distinct movement disturbance; these findings are discussed in light of seminal papers in this field by Vilensky et al. (Arch Neurol 38:646-649, 1981) and Halle…

The hip adduction moment as a proxy for joint loading: a comparison between walking and running.

Hip adduction moment (HAM) is commonly used as a surrogate for hip joint loading; however, its validity across different gait tasks and modifications remains unclear. Understanding how gait modification influences this relationship is essential for developing effective load-management strategies for individuals with or at risk of hip osteoarthritis. This study examined the relationship between HAM and hip joint reaction force (HJRF) during walking and running under natural and foot progression modification conditions in healthy adults. Musculoskeletal modelling in OpenSim was used to estimate …

The neural correlates of walking fatigability in people with multiple sclerosis: An fNIRS study.

Walking fatigability (WF) is a common and disabling feature of people with multiple sclerosis (pwMS), yet its neural underpinnings are poorly understood. Here, we investigated real-time cortical activity during the 6-min walk test (6MWT) and its relationship with changes in gait speed and quality. PwMS with and without WF (n = 42), and healthy controls (HC, n = 25), completed a 6MWT. Using functional near-infrared spectroscopy, cortical activity was measured in the prefrontal, premotor and motor cortices. Concomitantly, inertial measurement units measured gait speed and quality on a minute-by-…

Relation of pain sensitization to knee loading during walking in people with knee osteoarthritis.

OBJECTIVE: To examine the association of pain sensitization with knee joint loading during walking in people with knee osteoarthritis (OA). METHOD: For this cross-sectional study, we used baseline data from participants with symptomatic knee OA (n=104) enrolled in two clinical trials. We used pressure pain threshold (PPT) at the knee and wrist to assess sensitization. Using gait analyses during walking, we derived peaks and impulse of knee adduction moment (KAM) and knee flexion moment, and frontal and sagittal plane range of motion during stance. We used an Analysis of Covariance (ANCOVA) ad…

The Cumulative Impacts of Fatigue during Overload Training Can Be Tracked Using Field-Based Monitoring of Running Stride Interval Correlations.

Integrating running gait coordination assessment into athlete monitoring systems could provide unique insight into training tolerance and fatigue-related gait alterations. This study investigated the impact of an overload training intervention and recovery on running gait coordination assessed by field-based self-testing. Fifteen trained distance runners were recruited to perform 1-week of light training (baseline), 2 weeks of heavy training (high intensity, duration, and frequency) designed to overload participants, and a 10-day light taper to allow recovery and adaptation. Field-based runnin…

The effect of mobility-related anxiety on walking across the lifespan: a virtual reality simulation study.

Older adults who report a fear of falling are more likely to subsequently fall, yet, some gait anxiety-related alterations may protect balance. We examined the effect of age on walking in anxiety-inducing virtual reality (VR) settings. We predicted a high elevation-related postural threat would impair gait in older age, and differences in cognitive and physical function would relate to the observed effects. Altogether, 24 adults (age (y) = 49.2 (18.7), 13 women) walked on a 2.2-m walkway at self-selected and fast speeds at low (ground) and high (15 m) VR elevation. Self-reported cognitive and …

Predictors of incident reversible and potentially reversible cognitive frailty among Taiwanese older adults.

BACKGROUND: Few studies emphasize on predictors of incident cognitive frailty (CF) and examine relationships between various gait characteristics and CF. Therefore, we conducted a 2-year prospective study to investigate potential predictors, including gait characteristics, of incident reversible CF (RCF) and potentially RCF (PRCF) among Taiwanese older adults. METHODS: Eligible participants were individuals aged ≥ 65 years, who could ambulate independently, and did not have RCF/PRCF at the baseline. The baseline assessment collected information on physical frailty and cognitive measures, in a…

A systematic review of total arthroplasty and arthrodesis for end-stage hallux rigidus: A biomechanical perspective.

BACKGROUND: Both arthrodesis and total arthroplasty are acceptable surgical options for end stage hallux rigidus without significant angular deformity. Total arthroplasty preserves first metatarsophalangeal joint (MTPJ) motion, which may help restore a more physiological gait pattern. RESEARCH QUESTION: Is there a difference in the findings of gait studies after 1st MTPJ total arthroplasty or arthrodesis for end-stage hallux rigidus? METHODS: PRISMA guidelines were followed to conduct a systematic review of literature for studies reporting gait analysis after the above procedures. Predetermi…
