Adenosquamous Carcinoma of the Gallbladder: A Rare Surgical Entity.

Gallbladder cancer (GBC), while infrequent, is the predominant malignancy of the biliary tract. The absence of clinical symptoms in conjunction with aggressive behavior contributes to delayed diagnosis and unfavorable prognosis. This condition typically remains asymptomatic in its initial stages, making detection challenging. Herein, we report a case of adenosquamous carcinoma of the gallbladder in a 74-year-old Caucasian woman who was admitted due to anemia, gastrointestinal bleeding, and abdominal pain. Computed tomography (CT) of the abdomen demonstrated a formation adjacent to the neck of

[Immunotherapy in the treatment of aggressive pituitary tumors and carcinomas].

Since the therapeutic failure of temozolomide (TMZ), the first-line chemotherapy recommended against aggressive pituitary tumors (APTs) and pituitary carcinomas (PCs), there have been no solid evidence-based treatment recommendations. However, immune checkpoint inhibitors (ICIs) have emerged as a promising and recent therapeutic option, generating hope after the first successful case was published in 2018. APTs and PCs are rare tumors with poor prognosis due to their invasive behavior and high recurrence rate despite conventional treatments. These neoplasms pose a therapeutic challenge in endo

Recurrent desmoid tumors with ureteric fistula: a case report and literature review.

BACKGROUND: Desmoid tumors (DT) is a rare soft tissue tumor classified as a borderline neoplasm. It exhibits locally aggressive growth and is prone to postoperative recurrence. CASE PRESENTATION: We reported the first case of adolescent pelvic DT ureteral fistula, and report the patient's all progress of diagnosis and treatment. A recurrent pelvic DT in an adolescent female patient who initially underwent surgical resection but experienced recurrence, was effectively treated with sorafenib. Later, however, the patient's tumor changed from solid to cystic solid mixed, increasing in size, and t

Rare Emphysematous Osteomyelitis of the Femoral Head: A Case Report and Literature Review.

Emphysematous osteomyelitis (EO) is a perilous infection of the femoral head. In this case, we present a 66-year-old male patient of Han ethnicity, who had lumbar disc herniation and type 2 diabetes mellitus. The patient reported severe pain in his lower back and right lower limb, which had worsened over the past 20 days. Despite undergoing invasive procedures such as regional anesthetic blocks and other traditional Chinese medical methods such as acupuncture and acupotomy, his condition deteriorated. Pain in the lumbar region was not relieved, and there was a new onset of pain in the right hi

Factors affecting disease control after pituitary tumor resection in acromegaly: What is the current evidence?

Acromegaly, characterized by persistent hypersecretion of growth hormone (GH), is most often caused by a pituitary neuroendocrine tumor (PitNET), though, less often, ectopic GH or GH-releasing hormone secretion from various neoplasms outside the pituitary gland could cause it. Nearly 70% of somatotroph PitNETs are macroadenomas at diagnosis. Transsphenoidal surgery, the most effective treatment modality for acromegaly, could achieve remission in 73%. However, the remission rates could reach 87% if surgery is followed by medical therapy. Due to variable therapeutic responses to surgical and med

Differences in Endoscopic Features of Gastric Neuroendocrine Tumor and Neuroendocrine Carcinoma From a Clinicopathological Perspective.

Gastric neuroendocrine neoplasms are a rare type of stomach cancer, classified into well-differentiated neuroendocrine tumors (NETs) and poorly differentiated neuroendocrine carcinomas (NECs). Gastric NETs (G-NETs) originate from endocrine progenitor cells in the basal layer of the mucosa, primarily in the setting of chronic atrophic gastritis, such as autoimmune gastritis. They generally exhibit low malignancy and a favorable prognosis. By contrast, gastric NECs (G-NECs), a rare subtype of gastric cancers, arise from endocrine precursor cell clones that dedifferentiate in the deep portion of

An Unusual Case of Lynch Syndrome.

Lynch syndrome is the most common cause of hereditary colorectal cancer. It usually develops asymptomatically until symptoms related to colorectal carcinoma appear, such as gastrointestinal bleeding, abdominal pain, and changes in bowel habits and/or stool characteristics. Oftentime, when these clinical signs and symptoms are not present, the diagnosis becomes challenging. We present the clinical case of a 69-year-old woman, adopted, with no known previous history, who presented to the emergency department with low back pain, without irradiation, that had been going on for three days, associat

Effects of selective cyclooxygenase-2 inhibitor robenacoxib on primary cells derived from feline injection-site sarcoma.

Feline injection-site sarcomas (FISSs) are highly invasive malignant mesenchymal neoplasms that arise from injection sites in cats. Although the tumorigenesis of FISSs is still uncertain, there is a consensus that FISS is associated with chronic inflammation caused by irritation of injection-related trauma and foreign chemical substances. Chronic inflammation can provide a proper microenvironment for tumour development, which has been known as one of the risk factors of tumorigenesis in many tumours. To investigate the tumorigenesis of FISS and screen for its potential therapeutic targets, cyc

The Role of Radiotherapy in the Management of Vaginal Melanoma: A Literature Review with a Focus on the Potential Synergistic Role of Immunotherapy.

Among the mucosal melanomas, vaginal melanomas are very rare tumors, accounting for less than 20% of melanomas arising from the female genital tract. They occur most frequently in women in post-menopausal age, but younger patients may also experience this neoplasm, mainly located in the lower third of the vagina or the anterior wall. The optimal management of this tumor remains controversial, with surgery reported as the most frequently adopted approach. However, a clear benefit of surgical treatment in terms of survival has not yet been demonstrated. Conversely, radiotherapy may represent an

The meaning of R1 resection after endoscopic removal of gastric, duodenal and rectal neuroendocrine tumors.

INTRODUCTION: Well-differentiated gastric, duodenal, and rectal neuroendocrine neoplasms (NETs) are rare diseases usually managed by endoscopic treatment. Although several endoscopic techniques are available, the number of patients with incomplete (R1) resection is significant. AREAS COVERED: This review focuses on the meaning of incomplete R1 findings after endoscopic resection in type I gastric NETs; nonfunctioning, non-ampullary duodenal NETs; and small rectal NETs. Data were identified by MEDLINE database search without publication date limitation. EXPERT OPINION: An incomplete R1 findin

Adenosquamous cell carcinoma of the head and neck: a retrospective single institution series.

OBJECTIVES: Head and neck adenosquamous cell carcinoma (HN-ASCC) is a rare, aggressive neoplasm, with limited data reported in the literature. The aim of this study was to assess tumour behaviour and prognostic factors impacting overall survival (OS) in a retrospective, single institution series. METHODS: A retrospective study on patients affected by HN-ASCC who were treated surgically between 2002 and 2019 at the Department of Otorhinolaryngology - Head and Neck Surgery of the University of Brescia was conducted. Demographics, clinical data, OS, and relative prognostic factors were analysed.

Giant Soft Tissue Leiomyosarcoma of the Left Lower Extremity: Case Presentation With a Review of the Literature.

Leiomyosarcoma (LMS) accounts for approximately 5-10% of soft tissue sarcomas, with an estimated incidence in the United States (US) of less than one case/200,000 persons, more frequent in women than men. Approximately two-thirds of LMSs are retroperitoneal, abdominal, and mediastinal. Localized, soft tissue LMSs represent a lower percentage, with the lower limbs and trunk being the most frequently involved sites. LMSs larger than 5 cm (so-called giants) are even rarer, and to date have been little reported in the literature. In this paper, we present the case of a giant LMS of the left lower

Primary acinic cell carcinoma of the breast: A case report and literature review.

Acinic cell carcinoma (ACCA), a type of malignant epithelial neoplasm, tends to occur in the parotid gland, and is occasionally found within the breast. Published literature regarding primary ACCA of the breast is scarce, and the number of reports may be fewer than 100. At present, full clinical details have not been published. As an extremely rare disorder, ACCA cannot be definitively diagnosed depending on microscopic structure alone and often requires the assistance of immunohistochemistry. Currently, universal therapies are not available. Here, we present a 47-year-old patient with a histo

Purposeful use of multimodality imaging in the diagnosis of caseous mitral annular calcification: a case series report.

BACKGROUND: Caseous mitral annular calcification (CMAC) is a rare liquefactive variant of mitral annular calcification (MAC) and superficially mimics a cardiac vegetation or abscess. CMAC is viewed as a benign condition of MAC, while MAC has clinical implications for patients' lives. Correctly diagnosing CMAC is essential in order to avoid unnecessary interventions, cardiac surgery or even psychological suffering for the patient. CASE PRESENTATION: We report on 6 patients with suspected intra-cardiac masses of the mitral annulus that were referred to our institution for further clarification.

Circulating Noncoding RNAs in Pituitary Neuroendocrine Tumors-Two Sides of the Same Coin.

Pituitary neuroendocrine tumors (PitNET) are common intracranial neoplasms. While in case of hormone secreting tumors pituitary hormone measurements can be used for monitoring the disease, in non-functional tumors there is a need to discover non-invasive biomarkers. Non-coding RNAs (ncRNAs) are popular biomarker candidates due to their stability and tissue specificity. Among ncRNAs, miRNAs, lncRNAs and circRNAs have been investigated the most in pituitary tumor tissues and in circulation. However, it is still not known whether ncRNAs are originated from the pituitary, or whether they are casua

Eye movement abnormalities in neurodegenerative langerhans cell histiocytosis.

Langerhans cell histiocytosis (LCH) is a rare inflammatory myeloid neoplasm characterized by proliferation of tumor histiocytes that involves multiple organs including central nervous system. The physiopathologic process underlying degenerative neuro-LCH (i.e., DN-LCH) remains imperfectly settled. Since the main clinical features of DN-LCH are cerebellar ataxia and dysexecutive syndrome, eye movements might be disrupted and may help in disease diagnosis and monitoring. We retrospectively analyzed the medical records of twenty DN-LCH patients investigated using eye movement recording (EMR) in o

Pituitary Metastasis of Lung Neuroendocrine Carcinoma Mimicking Pituitary Adenoma:Case Report and Literature Review.

Pituitary metastasis is an unusual situation in clinical practice, while the incidence is increasing with age. Breast cancer for women and lung cancer for men were the most frequent primary origins of pituitary metastasis. Diagnosing asymptomatic patients with unknown primary malignant origin is difficult, thus pituitary metastasis may be diagnosed as primary pituitary adenoma. Here, we report a case of a 65-year-old patient with visual changes and diabetes insipidus, showing an extensive mass in the sellar region which was initially thought to be a primary pituitary adenoma. Patient corticotr
