Anosognosia in individuals with schizophrenia: toward recovery of insight.

This paper addresses the growing evidence that lack of insight, or unawareness of illness in people with schizophrenia, may actually be a medically based condition known as anosognosia. Major explanatory models and related research findings are discussed. A case scenario is presented, as well as current perspectives regarding assessment of insight and alternative treatment approaches. Implications for further research, nursing education, and practice also are included. With better understanding of the characteristics and treatment of diminished insight in individuals with schizophrenia, nurses

[Memory and affect].

In the last decade, numerous studies have been devoted to the cognitive approach of emotions and their relationship with memory. Cognitive neuropsychologists consider affectivity as a contextual factor which can facilitate the processes of encoding or restitution. It is not clear if emotions are memorized by the same processes as the neutral stimuli. In man, a dissociation was found between loss of memory for affective stimuli in subjects with lesions restricted to amygdala and loss of memory for neutral stimuli in subjects with lesions limited to hippocampus. Experimental studies showed that

Williams syndrome: from genotype through to the cognitive phenotype.

Williams syndrome, due to a contiguous gene deletion at 7q11.23, is associated with a distinctive facial appearance, cardiac abnormalities, infantile hypercalcemia, and growth and developmental retardation. The deletion is approximately 1.5Mb and includes approximately 17 genes. Large repeats containing genes and pseudogenes flank the deletion breakpoints, and the mutation mechanism commonly appears to be unequal meiotic recombination. Elastin hemizygosity is associated with supravalvular aortic stenosis and other vascular stenoses. LIM Kinase 1 hemizygosity may contribute to the characteristi

Cardiac surgery in special populations, Part 3: Patients with cognitive dysfunction.

Patients with cognitive dysfunction, usually Down's syndrome, frequently require corrective cardiac surgery for congenital cardiac anomalies. The most common cardiac anomalies in Down's syndrome are patent ductus arteriosus, atrial septal defect, ventricular septal defect, atrioventricular canal defects, and Tetralogy of Fallot. This article, the third and last in a series, discusses the physiology of the defects and the nursing care required for comprehensive care of the patient with delayed development.

Familial nature of reading disability.

An extensive psychometric test battery was administered to 125 children with a reading disability, to their parents and siblings, and to members of 125 matched control familes (N = 1,044). In addition to expected differences in spelling and reading, probands obtained significantly (P less than or equal to .01) lower scores than controls on tests of other cognitive abilities. Manifold deficits were also found in siblings and parents of probands, conclusively demonstrating the familial (genetic and/or common-family environmental) nature of the disorder.

Frontostriatal circuitry and the tryptophan kynurenine pathway in major psychiatric disorders.

RATIONALE: An imbalance of the tryptophan kynurenine pathway (KP) commonly occurs in psychiatric disorders, though the neurocognitive and network-level effects of this aberration are unclear. OBJECTIVES: In this study, we examined the connection between dysfunction in the frontostriatal brain circuits, imbalances in the tryptophan kynurenine pathway (KP), and neurocognition in major psychiatric disorders. METHODS: Forty first-episode medication-naive patients with schizophrenia (SCZ), fifty patients with bipolar disorder (BD), fifty patients with major depressive disorder (MDD), and forty-tw

Different aspects of impulsivity in chronic alcohol use disorder with and without comorbid problem gambling.

BACKGROUND AND AIMS: Alcohol use disorder (AUD) and problem gambling are highly comorbid disorders. This study aims to explore the role of four aspects of impulsivity (trait concept of impulsivity, choice impulsivity, impulsive aggression and response inhibition/decision-making) in long-term chronic AUD patients with and without problem or pathological gambling symptoms. METHODS: Cognitively intact chronic AUD patients were enrolled with (n = 32) and without (n = 71) problem gambling symptoms in an inpatient clinic for chronic alcohol users. Multiple facets of impulsivity, cognitive ability,

Effectiveness of metacognitive interventions for mental disorders in adults-A systematic review and meta-analysis (METACOG).

We evaluated the effectiveness and acceptability of metacognitive interventions for mental disorders. We searched electronic databases and included randomized and nonrandomized controlled trials comparing metacognitive interventions with other treatments in adults with mental disorders. Primary effectiveness and acceptability outcomes were symptom severity and dropout, respectively. We performed random-effects meta-analyses. We identified Metacognitive Training (MCTrain), Metacognitive Therapy (MCTherap), and Metacognition Reflection and Insight Therapy (MERIT). We included 49 trials with 2,60

Dopaminergic Perturbation in the Aetiology of Neurodevelopmental Disorders.

Brain development may be influenced by both genetic and environmental factors, with potential consequences that may last through the lifespan. Alterations during neurogenesis are linked to neurodevelopmental cognitive disorders. Many neurotransmitters and their systems play a vital role in brain development, as most are present prior to synaptogenesis, and they are involved in the aetiology of many neurodevelopmental disorders. For instance, dopamine (DA) receptor expression begins at the early stages of development and matures at adolescence. The long maturation period suggests how important

Serious Games in the new era of digital-health interventions: A narrative review of their therapeutic applications to manage neurobehavior in neurodevelopmental disorders.

Children and adolescents with neurodevelopmental disorders generally show adaptive, cognitive and motor skills impairments associated with behavioral problems, i.e., alterations in attention, anxiety and stress regulation, emotional and social relationships, which strongly limit their quality of life. This narrative review aims at providing a critical overview of the current knowledge in the field of serious games (SGs), known as digital instructional interactive videogames, applied to neurodevelopmental disorders. Indeed, a growing number of studies is drawing attention to SGs as innovative a

Natural Compounds as Medical Strategies in the Prevention and Treatment of Psychiatric Disorders Seen in Neurological Diseases.

Psychiatric disorders are frequently encountered in many neurological disorders, such as Alzheimer's and Parkinson diseases along with epilepsy, migraine, essential tremors, and stroke. The most common comorbid diagnoses in neurological diseases are depression and anxiety disorders along with cognitive impairment. Whether the underlying reason is due to common neurochemical mechanisms or loss of previous functioning level, comorbidities are often overlooked. Various treatment options are available, such as pharmacological treatments, cognitive-behavioral therapy, somatic interventions, or elec

[Systematics and treatment of anxiety disorders].

Anxiety disorders are the most common type of mental disorders and include such core forms as generalized anxiety disorder, panic disorder, agoraphobia, specific phobias, and social anxiety disorder. Anxiety disorders contribute significantly to the global burden of disease, and the diagnosis and treatment of these disorders is of great social and economic importance. Current approaches to the treatment of anxiety disorders involve the use of cognitive behavioral therapy, antidepressants, benzodiazepines, antipsychotics, few anticonvulsants, and some other medications.

Symptoms of ADHD, antisocial behavior, and cognitive performance: A longitudinal twin study.

We investigated longitudinal associations between adolescent attention-deficit/hyperactivity disorder (ADHD) symptoms and later antisocial behavior and tested 1) whether these associations are independent of familial factors shared by co-twins and 2) whether cognitive performance moderates the associations. In a population-based sample of Finnish twins (N = 602-1330, 53% female), we assessed ADHD symptoms with teacher assessments and a diagnostic interview in adolescence, symptoms of antisocial personality disorder with a diagnostic interview in young adulthood, and criminal behavior across th

Interplay Between Aging and Tau Pathology in Alzheimer's Disease: Mechanisms and Translational Perspectives.

Aging is a key risk factor for neurodegenerative disorders and is associated with widespread systemic and brain-specific changes. Alzheimer's disease (AD), a progressive and irreversible brain disorder, primarily affects older adults and leads to a gradual decline in cognitive function. The underlying disease mechanisms often begin years before clinical symptoms appear, limiting the effectiveness of current treatments. Several factors linked to aging-including inflammation, oxidative stress, impaired metabolism, and protein aggregation-contribute to the onset and progression of AD. A central f
