Progression of squamous carcinoma in situ of the vulva to invasive carcinoma, in the same area, after systemic Bleomycin therapy, is described. Such progression is an exception and not the rule in the natural history of this disease. Conservative therapy should be used, especially in young women.

Five cases of malignant change of ovarian dermoid cyst are presented. Two of these cases were of special interest due to the unusual nature of the malignancy. One case (an adenosquamous cell carcinoma) was apparently a malignant change of the columnar epithelium and its squamous metaplasia. The other case (a clear cell carcinoma) appeared to represent coincidental development of two tumors.

We review our experience with 13 patients in whom esophageal dilation was used for palliation of malignant esophageal obstruction and with five patients treated with esophageal prostheses for malignant esophagorespiratory fistula. Twelve of the 13 patients with dilation alone received satisfactory palliation and were able to eat a soft or regular diet. Four of five patients with esophagorespiratory fistulas had esophageal dilation followed by insertion of a esophageal prosthesis and received good palliation. Endoscopic insertion of esophageal prosthetic devices has become increasingly popular

The use of the End-to-End Anastomosis (EEA) circumferential stapler for reconstruction following resection of the esophagus and stomach for carcinoma of the cardia is reported here in 9 consecutive patients with fifteen anastomoses performed with this unit. The technical aspects of the EEA stapler are emphasized. Two frequent problems associated with this type of operation namely, stenosis or leak at the suture line, are completely avoided by the use of this stapler unit. Its use is recommended.

This is a report of a gastric perforation secondary to a metastasis from a primary squamous cell carcinoma of the lung in a 70-year-old white man who was being treated with cimetidine for a presumed benign gastric ulcer. The literature concerning bronchogenic carcinoma metastasizing to the stomach and small bowel is reviewed, and the effect of cimetdine on malignant gastric ulcers is discussed.

A 33-year-old patient with malignant cells in a cervical smear (Class V) had a cone biopsy of the cervix according to the technique described by Scott; this technique involves infiltration with local vasoconstrictive solution. Histological examination of the cone biopsy specime showed a squamous cell carcinoma of the endocervix and also appearances suggestive of the dissemination of tumour cells (tumor cells in lymphatic vessels and in tissue spaces at some distance from the primary lesion) as result of the surgical procedure. One hundred four patients with epidermoid carcinoma of the pyriform fossa were reviewed retrospectively. Survival, local recurrence and metastatic rates were all compiled for the purpose of comparing the efficacy of combined therapy with radiation therapy and surgery alone in treating such lesions. Combined therapy consisted of 4500 rad Cobalt60 therapy at 200 rad per day preoperatively. All lesions were staged according to AJC classification. Malignant acanthosis nigricans is rarely associated with carcinoma of the bowel or cervix. However, this dermatosis is described in a case of adenocarcinoma of the ascending colon, and in another with carcinoma of the cervix.

The report deals with a case of epidermoid carcinoma of the vallecula with widespread sebaceous differentiaton. The sebaceous areas were first observed in a cervical lymph-node metastasis, which preceded the discovery of the primary lesion. No similar lesion has been described. Recent literature dealing with extracutaneous sebaceous lesions is cited.
