Fluid control in elderly patients with nocturia.

OBJECTIVES: To investigate the pathophysiology of nocturnal polyuria associated with aging. METHODS: Fifty patients (mean age 67.7 years, range 50-87) with nocturia were recruited for this prospective study. Patients were classified into nocturnal polyuria (NP) and non-nocturnal polyuria (non-NP) groups based on records of their frequency-volume charts. A hypertonic saline infusion test was carried out to evaluate individual osmotic and volume control. RESULTS: In the NP group, there was a significantly increased nocturnal diuretic rate compared with the daytime diuretic rate. In the non-NP

Rapid restoration of microcirculatory blood flow with hyperviscous and hyperoncotic solutions lowers the transfusion trigger in resuscitation from hemorrhagic shock.

Resuscitation from hemorrhagic shock relies on fluid retransfusion. However, the optimal properties of the fluid have not been established. The aim of the present study was to test the influence of the concentration of hydroxyethyl starch (HES) solution on plasma viscosity and colloid osmotic pressure (COP), systemic and microcirculatory recovery, and oxygen delivery and consumption after resuscitation, which were assessed in the hamster chamber window preparation by intravital microscopy. Awake hamsters were subjected to 50% hemorrhage and were resuscitated with 25% of the estimated blood vol

Selective tissue uptake of agouti-related protein(82-131) and its modulation by fasting.

The blood concentration of agouti-related protein (AgRP), a protein related to hyperphagia and obesity, is increased in obese human and fasted lean subjects. Because there is no saturable transport system at the blood-brain barrier for circulating AgRP to reach its central nervous system target, uptake of AgRP by peripheral organs might be physiologically meaningful. Using the biologically active fragment AgRP(82-131), we determined the pharmacokinetics of its radioactively labeled tracer after iv bolus injection and compared it with that of the vascular marker albumin. AgRP enters peripheral

The role of arterial and venous pressure for volume regulation of an organ enclosed in a rigid compartment with application to the injured brain.

BACKGROUND: Treatment of increased intracranial pressure often includes an active change in arterial and venous pressure, sometimes with draining of cerebrospinal fluid. We evaluated tissue and perfusion pressure during corresponding interventions in an organ enclosed in a rigid compartment with capillaries permeable for electrolytes and impaired autoregulation, conditions comparable to those present in the injured brain. METHOD: An isolated cat skeletal muscle enclosed in a closed fluid-filled plethysmograph served as a model for the injured brain surrounded by cerebrospinal fluid and the cr

EEG burst suppression is not necessary for maximum barbiturate protection in transient focal cerebral ischemia in the rat.

Barbiturates have been demonstrated to reduce the cerebral metabolic rate (CMR) in a dose-dependent manner but investigations of a dose-response relationship for their neuroprotective efficacy are scant. It has been suggested that barbiturates possess other mechanism of action that may be critical to their protective effect. If so, it is conceivable that the peak effect of such mechanisms does not parallel the reduction in CMR. Thus, maximal neuroprotection may be achieved with a substantially lower dose of the drug. Thirty Sprague-Dawley rats were subjected to 2 h of middle cerebral artery oc

Effect of hypertonic saline solution and dextran on ventricular blood flow and heart-lung interaction after hemorrhagic shock.

BACKGROUND: Hypertonic saline solutions may have beneficial hemodynamic effects in the resuscitation of hemorrhagic shock. The effects on cardiac function and potential interaction with lung function are controversial and served as the basis for this study. METHODS: Domestic swine were resuscitated from hemorrhagic shock with equivalent sodium loads of lactated Ringer's solution (LR) or 7.5% NaCl plus 10% dextran (HSD). Hemodynamic data were obtained at baseline, shock, and after resuscitation. Right ventricular ejection fraction and left ventricular change in pressure with respect to time (d

Determination of cyanide in whole blood by capillary gas chromatography with cryogenic oven trapping.

Cyanide, one of the most important toxic substances, has been found measurable with high sensitivity by capillary gas chromatography (GC) with cryogenic oven trapping upon injection of headspace (HS) vapor samples. The entire amount of cyanide in the HS sample could be cryogenically trapped prior to on-line GC analysis. A 0.5-mL volume of blood in the presence or absence of cyanide and propionitrile (internal standard, IS) was added to a vial containing 0.25 mL of distilled water, 0.3 g of Na2-SO4, 0.2 mL of 50% H3PO4, and 0.1 g of ascorbic acid (when needed), and the mixture was heated at 70

Intracardiac shunting in children with ventricular septal defect: evaluation with Doppler color flow mapping.

In children with a ventricular septal defect, transseptal blood flow has been demonstrated angiographically to be bidirectional in all but the smallest defects. To investigate this phenomenon noninvasively, two-dimensional Doppler color flow echocardiography was used in 77 patients (aged 1 day to 15.5 years, mean 24 months). During isovolumetric contraction, flow was seen from the left to the right ventricle in 82 (98%) of 84 studies. During isovolumetric relaxation, right to left flow was noted in 72 (96%) of 75 studies. During early diastole in patients with an isolated defect, flow was init

[Pulmonary edema after laryngospasm].

A case is reported of rapidly resolving pulmonary oedema following post-extubation laryngospasm in a 23 year-old healthy man who underwent emergency resection of a rectal polyp. The laryngospasm occurred immediately after extubation and resolved after administration of curare. The patient was reintubated and auscultation disclosed bilateral coarse and moist rales. Chest X-ray displayed a right pulmonary opacity. Because of the deteriorating respiratory status, mechanical ventilation was used with positive end-expiratory pressure for 18 h. Chest examination, chest X-ray and arterial blood gas l

Pulmonary and cardiovascular responses to repeated ozone exposure during exercise in adults with exercise-induced bronchoconstriction.

We tested the hypothesis that exercising while breathing real-world ozone (O3) concentrations impairs pulmonary and cardiovascular function in individuals with exercise-induced bronchoconstriction (EIB) and that repeated exposures induce acclimation. Participants completed consecutive 30-min cycling sessions breathing room air (RA) and 170 ppb O3. A mixed-effects model and ANOVA were used. Sixteen individuals (50% male) with mild-moderate EIB completed the study. O3 exposure produced pulmonary impairment on Day 2 in forced expiratory volume in 1 s (Estimate = -5.1, 95% CI: -8.5 to -1.7, p = 0.

Protein biomarkers associated with left bundle branch block in patients with heart failure and reduced ejection fraction.

INTRODUCTION: Left bundle branch block (LBBB) is common in heart failure with reduced ejection fraction (HFrEF) and causes dyssynchrony, which accelerates cardiac remodelling. The biological mechanisms behind LBBB-associated remodelling remain largely unknown. Therefore, we used an omics approach to test the hypothesis that LBBB is associated with plasma protein dysregulation aiming at defining a proteome that contributes to the specific disease driving phenotype in dyssynchronopathy. METHODS: Patients were selected from the BIOlogy Study to TAilored Treatment in Chronic Heart Failure databas

Microscopic giant platelets versus automated mean platelet volume for severity assessment in COVID-19.

COVID-19-associated coagulopathy (immunothrombosis) contributes to mortality. Automated hematology analyzers are widely used for risk stratification but may not capture morphological platelet abnormalities. We examined whether peripheral blood smear (PBS) findings, particularly giant platelets, are associated with severe COVID-19 beyond automated mean platelet volume (MPV) in hospitalized patients. In this retrospective cohort study, 96 patients with confirmed COVID-19 were classified at hospital admission as mild/moderate (n = 60) or severe (n = 36). Admission PBS samples were independently r

Evaluation of modified hematological scoring system and platelet indices in diagnosing neonatal sepsis-A cross-sectional study.

The gold standard for identifying neonatal sepsis is blood culture. However, there are a few drawbacks: Getting results takes time and requires a complex procedure; also, it frequently yields false-positive or false-negative findings. Platelet indices and the modified hematological scoring system (mHSS) aid in the precise diagnosis of newborn sepsis. This study was conducted to assess the diagnostic value of platelet indices and mHSS in the diagnosis of neonatal sepsis separately and in combination. This cross-sectional study was performed on 104 neonates having clinical suspicion of sepsis. V

Assessing cardiovascular benefits of intensive blood pressure lowering in high-risk undiagnosed HFpEF patients.

AIMS: Heart failure with preserved ejection fraction (HFpEF) is often underdiagnosed. This study evaluates the HFpEF-ABA score's ability to identify high-risk, undiagnosed HFpEF subgroups with elevated cardiovascular event rates and assesses the impact of intensive blood pressure control in these populations. METHODS: A post-hoc analysis of the Systolic Blood Pressure Intervention Trial (SPRINT) was performed. The HFpEF-ABA score identified high-risk individuals with undiagnosed HFpEF. Cox proportional hazards regression was used to examine interactions between HFpEF-ABA score groups and inte

Platelet indices and inflammatory bowel disease: a Mendelian randomization study.

BACKGROUND: Platelets play a significant role in the innate and adaptive processes of immunity and inflammation. Inflammatory bowel disease (IBD) is an autoimmune disease that is widely understood to be caused by a combination of genetic predisposition, aberrant immune responses, etc. METHODS: To examine the relationships between genetically determined platelet indices and IBD, we conducted a Mendelian randomization (MR) study. Data associated with platelet count (PLT), mean platelet volume (MPV), platelet distribution width (PDW), plateletcrit (PCT) were used from the UK Biobank. The outcome

Physiological impact of oral carbohydrate preload in healthy volunteers.

BACKGROUND: Oral carbohydrate loading has become a key component of Enhanced Recovery After Major Surgery (ERAS) pathways, with potential improvements in patient comfort, nausea and vomiting, ileus and length of stay. The contribution of each component of ERAS, including carbohydrate beverages, remains unclear. We aimed to determine the impact of standardized oral carbohydrate fluid loading on haemodynamics, stroke volume and gastric state in a group of healthy volunteers. METHODS: Twenty-three volunteer participants free of known cardiovascular or gastrointestinal disease, consumed 400 mL of

Cardiac Remodeling After Hypertensive Pregnancy Following Physician-Optimized Blood Pressure Self-Management: The POP-HT Randomized Clinical Trial Imaging Substudy.

BACKGROUND: Hypertensive pregnancy disorders are associated with adverse cardiac remodeling, which can fail to reverse in the postpartum period in some women. The Physician-Optimized Postpartum Hypertension Treatment trial demonstrated that improved blood pressure control while the cardiovascular system recovers postpartum associates with persistently reduced blood pressure. We now report the effect on cardiac remodeling. METHODS: In this prospective, randomized, open-label, blinded end point trial, in a single UK hospital, 220 women were randomly assigned 1:1 to self-monitoring with research

Assessing Genetic Overlap Between Platelet Parameters and Neurodegenerative Disorders.

Neurodegenerative disorders such as Parkinson's disease (PD) and Alzheimer's disease (AD) suffer from the lack of risk-predictive circulating biomarkers, and clinical diagnosis occurs only when symptoms are evident. Among potential biomarkers, platelet parameters have been associated with both disorders. However, these associations have been scarcely investigated at the genetic level. Here, we tested genome-wide coheritability based on common genetic variants between platelet parameters and PD/AD risk, through Linkage Disequilibrium Score Regression. This revealed a significant genetic correla

A low end-tidal CO2/arterial CO2 ratio during cardiopulmonary resuscitation suggests pulmonary embolism.

INTRODUCTION: Identifying reversible causes of cardiac arrest is challenging. The diagnosis of pulmonary embolism is often missed. Pulmonary embolism increases alveolar dead space resulting in low end-tidal CO2 (EtCO2) relative to arterial CO2 (PaCO2) tension. Thus, a low EtCO2/PaCO2 ratio during resuscitation may be a sign of pulmonary embolism. METHODS: Post hoc analysis of data from two porcine studies comparing ultrasonographic measurements of right ventricular diameter during resuscitation from cardiac arrest of different causes. Pigs were grouped according to cause of arrest (pulmonary
