Reverse total shoulder arthroplasty in wheelchair-dependent patients.

BACKGROUND: Wheelchair-dependent patients have a high incidence of shoulder pathology, often causing severe impairment. This study reports outcomes of wheelchair-dependent lower extremity-impaired patients with symptomatic shoulder arthritis or severe rotator cuff pathology treated with reverse total shoulder arthroplasty (RTSA). METHODS: Data for 19 wheelchair-dependent patients who had an RTSA for symptomatic arthritis or rotator cuff pathology, or both, were obtained from the University of Florida Shoulder Arthroplasty Database. Included were 16 of 19 shoulders with adequate follow-up aver

Toward a Recognition System for Mexican Sign Language: Arm Movement Detection.

This paper describes ongoing work surrounding the creation of a recognition system for Mexican Sign Language (LSM). We propose a general sign decomposition that is divided into three parts, i.e., hand configuration (HC), arm movement (AM), and non-hand gestures (NHGs). This paper focuses on the AM features and reports the approach created to analyze visual patterns in arm joint movements (wrists, shoulders, and elbows). For this research, a proprietary dataset-one that does not limit the recognition of arm movements-was developed, with active participation from the deaf community and LSM exper

Arthroscopic transosseous foveal footprint repair of the triangular fibrocartilage complex.

Foveal disruption of the triangular fibrocartilage complex contributes to the instability of distal radioulnar joint. We have developed an arthroscopic transosseous foveal footprint repair technique, which maximizes the contact between the disrupted triangular fibrocartilage complex and its foveal footprint for better healing by using four sutures in a divergent configuration through a 1.6-mm bone tunnel. Twelve patients with triangular fibrocartilage complex foveal injuries who had undergone this repair technique were reviewed with a mean follow-up of 53 months. All patients achieved signific

Chikungunya virus: host pathogen interaction.

Chikungunya is a re-emerging arthropod-borne viral disease caused by Chikungunya virus (CHIKV) belonging to the Togaviridae family of genus Alphavirus. It is a virus with a single stranded, positive sense RNA, as its genome. It is maintained in a sylvatic and urban cycle involving humans and the mosquito species Aedes aegypti and Aedes albopictus. It has garnered the attention of scientists in the past 5-6 years due to the massive outbreaks in the Indian Ocean region in 2005-2006. It has a major health impact on humans as it causes fever, rashes, arthralgia and myalgia. Polyarthralgia is the m

[Arthroscopic resection of pigmented villonodular synovitis pseudotumor of the shoulder: a case report with three year follow-up].

Pigmented villonodular synovitis can be localized or diffuse. Lesions predominate in the knee but all of the joints can be involved. Thirty cases in the glenohumeral joint have been reported in the literature. The different reports to date have not identified any specific clinical signs. Our female patient presented non-specific shoulder pain which persisted for three years. The plain x-rays were normal. MRI and arthroscan revealed an intra-articular pseudotumor. Arthroscopy was performed for tumor biopsy which was followed by total resection. The diagnosis of villonodular synovitis pseudotumo

The dangers of snowboarding: a 9-year prospective comparison of snowboarding and skiing injuries.

We studied 2,552 snowboarding injuries and 5048 skiing injuries sustained during 1988-97. The number of snowboarding injuries had been increasing year by year and was 6 times as many as skiing injuries (2.0 versus 0.35 per 1,000 visits). The types of snowboarding injuries included fractures (39%), lacerations (21%), dislocations (17%), and contusions (15%). Upper extremity injuries were more frequent than those in the lower extremity in snowboarders. The commonest fractures involved the radius (48%), clavicle (11%), humerus (11%), and ulna (7-8%). The shoulder joint was most commonly dislocate…

Decreased joint and spinal mobility associated with low back pain in young adults.

Anthropometric factors, spinal and limb-joint mobility, and trunk strength were measured in young students--55 men and 48 women (mean age 21.4 years, SD 1.6). Twenty-six of the men and 29 of the women had had back pain during the preceding year and they were compared with those without back pain. In the male back-pain group, extension, lateral flexion and the sum of mobility in the lumbar spine, and hip flexion and external rotation of the shoulders were significantly smaller. In the female back-pain group, extension and the sum of mobility in the thoracic spine, and extension, external rotati

Inflammatory bowel disease related osteonecrosis: report of a large series with a review of the literature.

BACKGROUND: Osteonecrosis is a major complication of inflammatory bowel disease usually associated with steroid use. There are few large series available detailing the specifics of affected patients. AIM: To identify any specific characteristics of osteonecrosis in this cohort. A major focus was placed on steroid dose, the average time between diagnosis of IBD and appearance of osteonecrosis and the frequency of multiple joint involvement. METHODS: Our study identified 23 patients in the practices of five gastroenterologists at the Mount Sinai Medical Center. We retrospectively reviewed thei

Treatment of persistent shoulder pain with sodium hyaluronate: a randomized, controlled trial. A multicenter study.

BACKGROUND: Presently, there are no approved nonoperative therapies for the ongoing treatment of persistent shoulder pain. Preliminary data suggest that intra-articular sodium hyaluronate injections may be beneficial for the treatment of persistent shoulder pain resulting from various etiologies. The present study evaluated the efficacy and safety of sodium hyaluronate (Hyalgan; molecular weight, 500 to 730 kDa) for these patients. METHODS: Six hundred and sixty patients with persistent shoulder pain and limitation resulting from glenohumeral joint osteoarthritis, rotator cuff tear, and/or ad

Medially based anterior capsular shift of the glenohumeral joint. Passive range of motion and posterior capsular strain.

The effect of a medially based anterior capsular shift on translational and rotational range of motion and posterior capsular strain was investigated in an in vitro model. Six cadaveric shoulders were tested in a robot-assisted shoulder simulator. Translational and rotational range of motion were reduced by the capsular shift, particularly with the shoulder at higher elevation angles. At 60 degrees of elevation, anterior translation was decreased 1.9 +/- 2.9 mm, and posterior translation was decreased 2.3 +/- 6.2 mm. External rotation was decreased 11.5 degrees +/- 10.2 degrees, and internal r

[What information do we need to treat the injured shoulder of a hockey player?].

Successful patient care of the injured shoulder of a hockey player involves interdisciplinary cupertino. Images allow an interpretation of a static dimension but may not be sufficient enough to develop a surgical treatment strategy, since all injuries to the shoulder have its origin in a dynamic pathology with some kind of external impact. This article classifies various traumatic shoulder problems and the mechanism of injury, it outlines clinical tests and stresses key facts an imaging report should include in order to plan conservative or surgical treatment. Basic treatment options as perfor

Clinical features of first attack of rheumatic fever in adults.

The clinical features of the first attack acute rheumatic fever in 32 adults are described in a prospective study. Classical migratory polyarthritis occurred in 11 patients whilst the arthritis in the remaining 21 was additive in type, rapidly becoming symmetrical. The lower limb joints were involved more frequently than those of the upper limb. Mild transient carditis was present in only one patient. All patients showed evidence of antecedent streptococcal infection but none had chorea, subcutaneous nodules or erythema marginatum. The disease ran a benign course so that the sensitivity of the

The effect of supraspinatus tears on glenohumeral translations in passive pitching motion.

BACKGROUND: Supraspinatus tears are common in pitchers. However, the effect of these tears on glenohumeral (GH) mechanics is incompletely understood. PURPOSE/HYPOTHESIS: To describe the effect of supraspinatus tears and repairs on GH kinematics during an abbreviated throwing motion using the intact shoulder girdle. The hypothesis was that supraspinatus tears would lead to an increase of GH translation in the coronal plane and supraspinatus repairs would restore GH kinematics. STUDY DESIGN: Controlled laboratory study. METHODS: Six shoulders from 3 fresh-frozen cadavers were tested in a nove

OBJECTIVE: Aim of the procedure is shifting the arc of shoulder rotation for an improved external rotation, reaching a physiological elbow flexion without striking of the lower arm against the thorax; improvement of the activities of daily living because guidance of the hand to the face is possible without any simultaneous evasive movements of the shoulder. INDICATIONS: Palsy of infraspinatus and teres minor muscles after complete neurosurgical therapy (neurolysis, reconstruction of the brachial plexus), which leads to loss of external rotation and an internally rotating posture of the arm.
