Forty men with bladder outflow obstruction from benign prostatic hypertrophy were investigated urodynamically by medium-fill water cystometry and pressure flow study. The most obstructed patients proved to be unstable. A positive link was found between the degree of obstruction, as assessed by the opening pressure (i.e. the detrusor pressure needed to start micturition), and the maximum mechanical power generated by the contracting bladder during voiding. What seems most striking, is that neither detrusor contraction velocity nor the urine flow rate fully mirrored the degree of obstruction--i.

An effectively functioning artificial bladder would be useful after cystectomy because of the ease of insertion and the availability to all patients. In this report, we describe a new design concept based on negative pressure drainage of the kidneys and active voiding, and we present the results of testing this device. Although previous prosthetic bladders have had difficulties with renal failure from hydronephrosis, infection from retention of urine, and encrustation of the luminal surface, the current artificial complete bladder system addresses these problems.

The uroflow curves of 45 men with either bladder outlet obstruction or impaired detrusor contractility were retrospectively reviewed. The definitive diagnoses were attained by clinical and video-urodynamic studies with simultaneous detrusor pressure and uroflow measurements. Eight parameters were analyzed to determine if uroflow can differentiate obstruction from impaired contractility. There were no differences between the 2 groups in any of the parameters. We present a case of a cirrhotic patient suffering an intradiverticular squamous carcinoma with perforation to the peritoneal cavity, diagnosed after performance of an emergency laparotomy due to acute abdomen with infaust evolution. There have been a number of reports recently on vesical calculi in women with a history of gynecological procedures. Within 1 year we encountered 2 patients with vesical calculi after vesicourethral suspension performed with concomitant hysterectomy. These calculi formed on a retained intravesical suture and a monofilament knitted polypropylene pledget, and they produced interesting pathological and radiographic findings.

Carcinoma of urachus accompanied with calcification is rarely encountered. A 37-year-old man presented with the complaint of mucus discharge on voiding. Two hundred and fifty-three men (age 69.2 +/- 7 years) undergoing prostatectomy for bladder outflow obstruction were studied. The aims were to determine the distribution of voiding pressures and their relationship to presenting symptoms and urodynamic findings. One of the most frequent difficulties in the rehabilitation of the hyperreflexic bladder in patients with spinal cord injuries is vesico-sphincter-dyssynergia. A light microscope and neurohistochemical study is presented of full thickness biopsies from the bladder neck of 10 middle-aged males with urodynamically proven bladder neck obstruction. Apart from varying degrees of collagen deposition in seven cases, no abnormality was present in the detrusor and pre-prostatic components of the bladder neck muscles or in the distribution of cholinergic nerve fibres. The normal noradrenergic innervation to the pre-prostatic sphincter was absent in all specimens; this may be age-related.

22 patients diagnosed as having bladder neck disease were evaluated retrospectively point of view. Urodynamic study including intravesical pressure, rectal pressure, intrinsic detrusor pressure and urine flow was performed in every case. A relation was established between the findings and the definite etiology. Two urodynamic patterns were found, one with high pressures and low voiding flow, and one with low pressures and low voiding flow. Functional differences were not found between the two etiological groups, i.e. fibrotic and idiopathic.
