The asymmetry spinal kinematics in C-shaped adolescent idiopathic scoliosis during level walking.

Adolescent idiopathic scoliosis (AIS) patients exhibit trunk movement asymmetry during gait, reflecting altered motion patterns and postural control strategies. To study movement asymmetry at specific spinal joints, it is necessary to develop gait models precise to these joints and avoid mixed AIS classification. Based on an articulated thoracolumbar spine model and a refined marker placement protocol, we achieved a detailed analysis of spinal kinematics in patients with mild AIS. Gait data were collected from 10 PUMC type IA AIS patients, 13 PUMC type IB AIS patients, and 13 healthy controls.…

Plasma metabolites and physical function in patients undergoing hemodialysis.

Impaired physical function contributes to falls, fractures, and mortality among patients undergoing dialysis. Using a metabolomic approach, we identified metabolite alterations and effect size-based composite scores for constructs of impaired gait speed and grip strength. 108 participants incident to dialysis had targeted plasma metabolomics via liquid chromatography-mass spectrometry and physical function assessed (i.e., 4 m walk, handgrip strength). Physical function measures were categorized as above/ below median, with grip utilizing sex-based medians. To develop composite scores, metaboli…

Upper and Lower Limb Movement Kinematics in Aging FMR1 Gene Premutation Carriers.

Fragile X-associated tremor/ataxia syndrome (FXTAS) is a neurodegenerative disorder associated with a premutation cytosine-guanine-guanine (CGG) trinucleotide repeat expansion of the FMR1 gene. FXTAS is estimated to be the most common single-gene form of ataxia in the aging population. Gait ataxia and intention tremor are the primary behavioral symptoms of FXTAS, though clinical evaluation of these symptoms often is subjective, contributing to difficulties in reliably differentiating individuals with FXTAS and asymptomatic premutation carriers. This study aimed to clarify the extent to which q…

Research Toward Understanding the Benefits and Limitations of Orthotic Use To Improve Mobility and Balance for Individuals With Neuropathic Conditions.

BACKGROUND: Walking is a vital activity often compromised in individuals with neuropathic conditions. Charcot-Marie-Tooth (CMT) disease and Cerebral Palsy (CP) are two common neurodevelopmental disabilities affecting gait, predisposing to the risk of falls. With guiding scientific evidence limited, there is a critical need to better understand how surgical correction affects mobility, balance confidence, and gait compared to ankle foot orthosis (AFO) bracing. A systematic approach will enable rigorous collaborative research to advance clinical care. METHODS: Key elements of this vision includ…

A measure of kinematic limb instability modulation by rhythmic auditory stimulation.

A mathematical method based on computations of residual absolute value sums (RAVS) was developed for the quantitative analysis of tremor-like perturbations of knee angle during the gait cycle. The method was tested on simulation data created by adding sinusoidal tremor of varying frequency and amplitude to the knee-angle graph of a healthy test subject. The method was then applied to compare knee tremor reduction, with and without auditory rhythm, in a group of five traumatically brain-injured patients with gait hemiparesis. Deviations from normal gait performance due to tremor were assessed b…

A novel method for locomotion training.

This article describes a novel therapeutic system for locomotion training and learning for patients with a wide range of neurological and musculoskeletal disorders. The technique embraces the notion that locomotion therapy should be goal oriented and task specific. The task specificity includes a partial weight-bearing device that permits the posture/equilibrium, movement, and weight-bearing components of gait function to operate concurrently, even in patients with serious deficits. In addition, it allows interaction with therapists and others to facilitate locomotion control, particularly dur…

Historical review of research on functions of basal ganglia.

Basal ganglia have been known as a motor center because their lesions cause motor disturbances in involuntary movements such as chorea, ballism or akinesia in parkinsonism. The different types of involuntary movements are closely related to the underlying muscle tone. Mechanisms of bradykinesia or akinesia have been elaborated in physiological studies on Parkinson's disease and the significance of sensorimotor processing or attention arousal has been disclosed as a relevant factor of bradykinesia. Analysis of short-stepped gait, frozen gait or apraxia of gait, has claimed the frontal lobe and …

Influence of dystonia on the response to long-term L-dopa therapy in Parkinson's disease.

The gait of normal subjects was examined electromyographically and the pattern was altered during preferential blockade of large nerve fibres to alternating activity in flexor and extensor muscles. The EMG activity was disrupted more in flexor than extensor muscles by preferential ischaemic blockade. Normal gait was associated with flexor contraction only when the foot was lifted and placed on the ground, whereas during ischaemic blockade flexor contraction continued during the interval between foot lifting and foot placement. The `freezing' or `blocking' gait in Parkinson's disease was found to…

Physical Performance Moderates the Association Between Age and Executive Function in Older Adults at Elevated Risk of Dementia: Cross-Sectional Analysis From the CITA GO-ON Trial.

BACKGROUND: Preservation of executive function is vital for independence and dementia prevention, yet the physical performance levels at which age-related executive vulnerability becomes detectable remain uncertain. This study examined whether gait speed, handgrip strength and leg muscle power influence the relationship between age and executive function in older adults at increased dementia risk and assessed how sex and white matter hyperintensity (WMH) burden affect this relationship. METHODS: Cross-sectional analysis of 920 community-dwelling older adults (aged 60-85) at high dementia risk…
