Acute right to left blood shunt is an unusual cause of acute hypoxia. We describe a case of a patient with an atrial septal defect who developed acute hypoxia due to cardiac tamponade. Acute haemopericardium developed as a complication of temporary transvenous cardiac pacing. Bubble contrast echocardiography confirmed right to left blood shunting at the atrial level. Acute hypoxaemia and the right to left blood shunt resolved when the pericardium was drained. The case underscores the importance of evaluating the presence of an intracardial shunt in patients with otherwise inexplicable hypoxia.

Outcomes of device closure of large and small secundum atrial septal defects (ASDs) as related to rim anatomy with the Amplatzer atrial septal occluder were compared. We describe a case in which recurrent vertebral osteomyelitis and psoas abscess due to Streptococcus constellatus and Fusobacterium nucleatum developed in a patient with a previously unrecognized atrial septal defect with bidirectional shunts. Disease recurrence was ascribed to the presence of dental disease, which served as the source of infection. To the best of our knowledge, this is the first case in which vertebral osteomyelitis and psoas abscess have been associated with atrial septal defect.

An atrial septum defect was closed with an ASDOS (Babic) occluder in 13 patients aged from 22 to 67 years. The diameters of the ASD ranged from 12 to 36 mm, the left to right shunt from 35% to 70%.
