Incidence of sepsis in neonates with clinical respiratory distress.

Respiratory distress is the most frequent symptom initiating referral to neonatal intensive care centers. Previous reports have indicated that 40% of infants with sepsis proved by blood culture had respiratory symptoms as their clinical presentation. In this prospective study there were 145 consecutive infants referred because of respiratory distress. Two-site blood cultures were collated with all other clinical and laboratory data to confirm a diagnosis of septicemia. There was a 4.8% incidence of bacteremia, and clinical and laboratory parameters confirmed septicemia in 3.5% of infants studi

Congenital Myasthenic Syndrome in Pregnancy: A Case Report and Literature Review on Peripartum Management Strategy.

Congenital myasthenic syndromes (CMS) are a group of rare inherited neuromuscular disorders caused by defects in neuromuscular transmission. Unlike autoimmune myasthenia gravis, CMS typically presents in childhood with variable severity and symptoms, and does not involve autoantibodies. This report presents the management of a primigravida diagnosed with CMS who was generally stable on pyridostigmine therapy but experienced a severe exacerbation during pregnancy. The diagnosis of CMS was made on clinical grounds, but the genetic testing for confirmation of the exact mutation was not available.

Mortality and morbidity in oesophageal atresia.

BACKGROUND: Several classification systems exist to predict mortality in oesophageal atresia, the most widely quoted of these being over 20 years old. No classification system exists to predict morbidity. We sought to test whether these classification systems remain relevant and to determine whether they can be useful to predict morbidity. In addition, we aimed to identify independent risk factors for predicting mortality and morbidity. METHODS: Neonates presenting with oesophageal atresia over a 20-year period (1990-2010) were retrospectively reviewed. Discriminative statistical analysis com

Assessment of Healthcare Provider Workload in Neonatal Resuscitation.

Objective: Human errors or protocol deviations during neonatal resuscitation are common. Excess workload has been proposed as a contributor to human error during medical tasks. We aim to characterize healthcare providers' perceived workload during neonatal resuscitation. Design: Perceived workload was measured using a multi-dimensional retrospective National Aeronautics and Space Administration Task Load Index (NASA TLX) survey. The NASA TLX collects data on mental, physical, and temporal demand, performance, effort, and frustration. Each section is rated independently by participants on a sca

Morbidity and mortality in neonates with Down Syndrome based on gestational age.

BACKGROUND: Greater than 50% of neonates with Down Syndrome (DS) have perinatal complications that require admission to the neonatal intensive care unit (NICU) at birth. Previous studies have shown increased morbidity and mortality rates in neonates without DS delivered prior to 39 weeks of completed gestation. OBJECTIVE: To determine if an association exists between gestational age at delivery and adverse outcomes in neonates with DS. STUDY DESIGN: Neonates with DS admitted to a large, tertiary care center NICU from 2010 to 2020 were evaluated. Gestational age (GA) was stratified into 4 gro

Adverse pregnancy outcomes associated with maternal syphilis infection.

BACKGROUND: Syphilis infections affect many pregnant women worldwide every year. The increasing rates of maternal and congenital syphilis infections in recent years in the United States necessitates further investigation into the adverse effects of syphilis infection on maternal and neonatal health. OBJECTIVE: We sought to examine the association of maternal syphilis infection with adverse pregnancy and perinatal outcomes. METHODS: We performed a retrospective cohort study of women with singleton pregnancies who delivered between 23 and 42 weeks in California between 2007 and 2011. We compar

Neonatal group B streptococcal septicaemia in a developing country.

During a 3-year study at the neonatal intensive care unit, University Hospital of the West Indies, the incidence of group B streptococcal (GBS) septicaemia was 13.6/1,000 admissions and 1.4/1,000 live births. GBS accounted for 35% of blood culture isolates and was the most frequent cause of septicaemia. Underlying disease or other possible predisposing factors were identified in 16 of 25 neonates with GBS septicaemia. Comparison of early onset and late onset disease indicated an increased incidence of prematurity, prolonged rupture of membranes, and respiratory distress in the former and a pre

Comparison of auditory brainstem response and behavioral screening in neonates.

This paper reports on two studies, one in Halifax and the other in Ottawa, which compared behavioral methods and BERA in the screening of hearing loss in neonates. The Halifax study used BERA as the screening procedure for infants of a neonatal intensive care unit (NICU) and as a supplementary procedure to behavioral screening test for non-NICU but "at risk" infants. The results of this study indicate false positive rates with the behavioral test of 86.1 and 50.5% for NICU and at risk groups respectively. The Ottawa study evaluated the Crib-o-gram as a screening test for NICU infants using BER

Comparative Assessment of Transcutaneous Bilirubin Versus Total Serum Bilirubin Measurements in Preterm Infants.

INTRODUCTION: Hyperbilirubinemia is a common condition, particularly among preterm neonates. Although transcutaneous bilirubin (TCB) measurement offers a non-invasive modality for bilirubin assessment, its diagnostic accuracy in preterm populations remains incompletely characterized. This study addresses a knowledge gap by focusing on preterm neonates between 28 and 35 weeks of gestation and evaluating the concordance between TCB and total serum bilirubin (TsB) measurements. MATERIALS AND METHODS: A prospective cohort study was conducted at a tertiary care academic medical center between Apri

New architectural design of delivery room reduces morbidity in preterm neonates: a prospective cohort study.

BACKGROUND: A multidisciplinary committee composed of a panel of experts, including a member of the American Academy of Pediatrics and American Institute of Architects, has suggested that the delivery room (DR) and the neonatal intensive care units (NICU) room should be directly interconnected. We aimed to investigate the impact of the architectural design of the DR and the NICU on neonatal outcome. METHODS: Two cohorts of preterm neonates born at < 32 weeks of gestational age, consecutively observed during 2 years, were compared prospectively before (Cohort 1: "conventional DR") and after ar

Artificial Intelligence-Based Video Analysis for Assessing Sucking Behavior in Preterm Infants: A Feasibility Study.

BACKGROUND/OBJECTIVES: Preterm infants often experience impaired swallowing function, and objective assessments for this population remain limited. In this prospective single-center study, we aimed to propose and validate an automated framework that quantitatively assesses neonatal sucking behavior by tracking facial key points in bottle feeding videos. METHODS: Fifty-eight preterm infants (corrected age [CA] ≤ 2 months) were enrolled, and 2 min videos of bottle-feeding were recorded. Certified therapists manually evaluated the videos using the Neonatal Oral Motor Assessment Scale (NOMAS), an

Evolution of Antimicrobial Resistance in Neonatal Sepsis: A Narrative Review.

Antimicrobial resistance is a growing threat in neonatal intensive care units (NICUs) worldwide, challenging the management of neonatal sepsis for decades. The aim of this narrative review is to compare the epidemiology and resistance patterns of neonatal sepsis in NICUs between two periods, 2000-2005 and 2020-2025, and to identify key insights that may inform future practices to limit the emergence and dissemination of antimicrobial resistance in NICUs. During the early 2000s, resistant pathogens, including extended-spectrum beta-lactamases (ESBL)-producing Enterobacterales, vancomycin-resist

Severe cholestasis in neonatal extracorporeal membrane oxygenation.

INTRODUCTION: Cholestasis is a common complication of Extracorporeal Membrane Oxygenation (ECMO) secondary to patient physiology and circuit-induced factors. In our institution's Neonatal Intensive Care Unit (NICU), we noted cases of severe cholestasis, with peak conjugated bilirubin levels much higher than previously reported in the literature. The objective of our study was to identify the contributing factors to the development of severe cholestasis in neonatal ECMO cases. METHODS: Using our institutional ECMO database, all neonates who received ECMO at our institution were identified. A r
