Culturally-Based Communication about Health, Eating, and Food: Development and validation of the CHEF scale.

Ethnic minority populations in the United States are disproportionately affected by obesity. To address this disparity, research has begun to investigate the role of culture, ethnicity, and experiences with racism on food choices and health interventions. The aim of the current study was to develop and evaluate a new scale measuring the extent to which individuals' culture, as they perceive it, influences perceptions of food-related health messages. A diverse sample of 422 college students responded to the item pool, as well as surveys on race-related stress, self-efficacy in making healthy fo

From ischochymia to gastroparesis: proposed mechanisms and preferred management of dyspepsia over the centuries.

Dyspeptic symptoms are common with most patients suffering functional disorders that remain a therapeutic challenge for medical practitioners. Within the last three decades, gastric infection, altered motility, and hypersensitivity have gained and lost traction in explaining the development of functional dyspepsia. Considering these shifts, the aim of this review was to analyze changing understanding of and approaches to dyspepsia over a longer time period. Monographs, textbooks, and articles published during the last three centuries show that our understanding of normal gastric function has i

Reconsideration of postoperative oral intake tolerance after pancreaticoduodenectomy: prospective consecutive analysis of delayed gastric emptying according to the ISGPS definition and the amount of dietary intake.

OBJECTIVE: A prospective consecutive study was planned to evaluate the postpancreaticoduodenectomy (PD) oral intake tolerance. The occurrence of delayed gastric emptying (DGE), as defined by the International Study Group of Pancreatic Surgery (ISGPS), and the amount of dietary intake were analyzed. The risk factors for low oral intake tolerance were additionally determined. SUMMARY BACKGROUND DATA: The causation of DGE after PD is still unclear. Several possible factors have been discussed, such as reconstruction methods and other complications. However, none of them has followed the definiti

Hospital discharge of the high-risk neonate.

This policy statement updates the guidelines on discharge of the high-risk neonate first published by the American Academy of Pediatrics in 1998. As with the earlier document, this statement is based, insofar as possible, on published, scientifically derived information. This updated statement incorporates new knowledge about risks and medical care of the high-risk neonate, the timing of discharge, and planning for care after discharge. It also refers to other American Academy of Pediatrics publications that are relevant to these issues. This statement draws on the previous classification of h

[Reduction of mother-child transmission of HIV infection in Africa: from clinical research to public health programs].

More and more African women are infected by HIV. As a result the mother-to-child transmission (MCT) rate is rising. Various prevention techniques have been assessed in randomized clinical trials in Africa but results need to be discussed to understand the full implications for prevention programs. To gain insight into this issue, we reviewed 11 randomized trials conducted in Africa and several other studies from developed countries. Trials using antiretroviral (ARV) drugs demonstrated good results for prevention of MCT in the first six months of life using abbreviated regimens involving either

Enhanced recovery after surgery for inflammatory bowel disease surgery: A systematic review.

Enhanced Recovery after Surgery (ERAS) protocols, though well-established across surgical disciplines, remains underutilized in inflammatory bowel disease (IBD) surgeries due to complexities like malnutrition and immunosuppression. This systematic review evaluated 13 studies involving 3,539 patients to assess the impact of ERAS implementation in IBD surgery. Data consistently showed that ERAS protocols reduced hospital stay, expedited bowel function return and decreased complications and readmissions compared to conventional care. Key components included nutritional optimization, laparoscopic

Esophageal Perforation Resulting From Nonaccidental Trauma in a Neonate: A Case Report.

Esophageal perforation (EP) resulting from nonaccidental trauma in a neonate is extremely rare. We report a previously healthy 12-day-old neonate presenting with stridor, respiratory distress, and bloody vomitus. Clinical, radiographic, and endoscopic evaluations confirmed the diagnosis of EP. The patient received respiratory support, remained on nothing by mouth (NPO) status, and was administered parenteral nutrition for seven days until healing was confirmed. Upon resuming oral feeding, the neonate tolerated it well and was discharged home. A follow-up endoscopy, six weeks later, revealed no

Year in Review 2022: Noteworthy Literature in Cardiothoracic Critical Care.

The past year in critical care medicine was notable for ongoing sequelae of the COVID-19 pandemic, including nationwide shortages and critical care demand in many regions in excess of usual operating capacity. Despite these challenges, evidence-based medicine and investigations into the optimal management of the critically ill continued to be at the forefront. This article is a collection of studies published in 2022 which are specifically relevant to cardiothoracic critical care. These noteworthy publications add to the existing literature across a broad spectrum of topics, from optimal timin

Bone marrow oxalosis with pancytopenia in a patient with short bowel syndrome: Report of a case and review of the literature.

Systemic oxalosis is a condition in which calcium oxalate crystals deposit into various bodily tissues. Although this may occur as the result of a rare primary syndrome in which an error of glyoxylate metabolism causes an overproduction of oxalate, it is more often seen as a secondary process characterized by increased enteric oxalate absorption. Here, we describe a patient with short bowel syndrome on long-term parenteral nutrition support who developed a unique manifestation of systemic oxalosis, leading to deposition of oxalate crystals within the bone marrow contributing to pancytopenia. Effects of Ionophores on Ruminal Function of Beef Cattle.

Ionophores have been widely used in the beef and dairy industry for decades to improve feed efficiency and performance by altering ruminal fermentation dynamics, increasing the level of propionate. Ionophores can also reduce ruminal proteolysis and ammonia synthesis, thus increasing the influx of protein into the small intestine in cattle, leading to improvements in performance and efficiency responses. Ionophores indirectly impact ruminal methanogenesis by decreasing the substrate used to produce methane. Despite the consistent benefits of using ionophores in cattle nutrition, their utilizati

Novel management of critically induced polyneuropathy in intensive care patients: A case report of two patients.

There has been tremendous growth in patients requiring critical care with severe infections. During a prolonged stay in the intensive care unit (ICU), patients develop critical illness polyneuropathy (CIP). The early identification of neurological involvement requires special attention during ICU care. We describe two cases who developed complete motor weakness after a prolonged stay in ICU. Patients were successfully managed with pyridostigmine and testosterone hormonal therapy initially and later with pyridostigmine only. The present case series highlights the need for early recognition, ass

An update on the use of inositols in preventing gestational diabetes mellitus (GDM) and neural tube defects (NTDs).

INTRODUCTION: Obstetric history and maternal body composition and lifestyle may be associated with serious complications both for the mother, such as gestational diabetes mellitus (GDM), and for the fetus, including congenital malformations such as neural tube defects (NTDs). AREAS COVERED: In view of the recent knowledge, changes in nutritional and physical activity habits ameliorate glycemic control during pregnancy and in turn improve maternal and neonatal health outcomes. Recently, a series of small clinical and experimental studies indicated that supplemenation with inositols, a family o

Enhanced recovery after esophageal resection.

ERAS is a multimodal perioperative care program which replaces traditional practices concerning analgesia, intravenous fluids, nutrition, mobilization as well as a number of other perioperative items, whose implementation is supported by evidence-based best practices. According to the RICA guidelines published in 2015, a review of the literature and the consensus established at a multidisciplinary meeting in 2015, we present a protocol that contains the basic procedures of an ERAS pathway for resective esophageal surgery. The measures involved in this ERAS pathway are structured into 3areas: p
