Efficacy of needle and endoscopic lavage on the recuperation of microspheres from the adult equine metacarpo-/metatarsophalangeal joint and digital flexor tendon sheath.

OBJECTIVES: To measure microsphere recovery following needle-through-and-through lavage (NTAT) of the metacarpo-/metatarsophalangeal joint (fetlock) and digital flexor tendon sheath (DFTS) compared to endoscopic lavage (EL). STUDY DESIGN: Ex vivo experimental study. ANIMALS: Adult equine cadavers immediately following euthanasia (n = 10). METHODS: Colored 15 μm microspheres (2 million) were injected into fetlock joints and DFTS. Synovial structures were assigned to NTAT or EL groups. Each lavage was performed using 5 L of 0.9% NaCl, sequentially collecting egress fluid for microsphere quant…

Anatomical Study of Nerve Injuries With Minimally Invasive Calcaneus Osteotomies.

BACKGROUND: Calcaneal osteotomy is frequently used in correcting various foot deformities. Minimally invasive procedures such as minimally invasive calcaneus osteotomy (MICO) have become established because of their more favorable risk profile. The literature describes a safe zone for performing the osteotomy to protect the nerve structures. Overall, the existence of a "safe zone" remains controversial. The aim of this anatomical study was to determine the risk of nerve injury in the context of MICO. METHODS: Twenty fresh frozen specimens were randomized to a V-shaped and oblique MICO groups.

Anatomical Structures at Risk After Medial Column Nailing.

BACKGROUND: Medial column nails have been introduced into Charcot's neuroarthropathy reconstruction as superconstruct fixation with high fatigue strength and pullout resistance. Similar to intramedullary nailing throughout other long-bone fixation, injury to neurovascular and musculotendinous structures secondary to percutaneous interlocking screw fixation may exist. We sought to identify structures at risk for injury during the interlocking of a medial column nail. METHODS: Medial column nails were inserted into ten cadaveric limbs. The proximal (talar), middle (first metatarsal), and distal…

Natural Openings in the Paraneurium and Epineurium of the Human Sciatic Nerve and Their Relationship to the Entry and Exit of Blood Vessels. A Microanatomical Study.

The microvasculature of peripheral nerve not only is important for the understanding of the development of compression syndromes, but it plays a critical role in the evolution of other nerve pathologies, including, for example, the distribution of intraneural ganglion cysts and lymphoma. We investigated the anatomical course of vessels around the human sciatic nerve and its bifurcation in eight human cadavers. Specifically, the presence of fenestrations on the epineurium and paraneurium's thickness in relation to the intraneural vessels was investigated. The sciatic, tibial, peroneal and sural…

Ultrasonographic study of the thumb pulleys and their entheses: correlations with anatomy and histology.

OBJECTIVES: The entheseal nature of digital annular pulleys insertion was recently described, with high resolution ultrasound (US) reliably identifying these submillimetre structures with impressive accuracy. Since the thumb was not examined, the aim of the present study was to characterise the thumb annular pulley system, including entheses, from an anatomical, histological and ultrasonographic perspective. METHODS: US assessment and gross anatomical dissection were performed on cadaveric thumbs to investigate the thickness and structural features of the four thumb pulleys, including the ide…

Percutaneous Plating of the Medial Distal Femur: Cadaveric Dissection.

Percutaneous medial plating of distal femur fractures, particularly comminuted fractures, offers improved biomechanical stability without the soft tissue insult of an open approach. The addition of a medial plate neutralizes the cantilever bending force that arises from the offset between the single lateral locking plate and the femur's anatomical axis. This cantilever bending force has been theorized to be a contributor to the high nonunion rate of distal femur fractures. Although concerns about the proximity of the superficial femoral artery have previously limited widespread adoption of med

A1 pulley cutting potential and safety of three ultrasound-guided percutaneous A1 pulley release techniques for trigger finger: a cadaveric study.

BACKGROUND: There is no consensus as to the best technique for percutaneous trigger finger release. METHODS: This assessor-blinded study compared three ultrasound-guided percutaneous trigger finger release techniques using a needle (N), a needle-knife (NK), and a specially designed knife (K). Three physicians simulated A1 pulley release surgery on 56 fingers of 14 fresh-frozen hand cadaver body donors. Both the physicians and the fingers included were randomly selected. RESULTS: The results of repeated-measures ANOVA revealed significantly longer cuts for the NK and K techniques, than for th…

A cadaveric study of ultrasound guided nonincisional trigger finger release with newly developed threads.

This study investigated the effectiveness and safety of ultrasound-guided non-incisional thread trigger finger release on cadavers using a newly developed domestic thread (Smartwire-01). Ultrasound-guided non-incisional thread trigger finger release was performed on 12 fresh cadaveric hands, including 12 thumbs and 48 long fingers. Two experts, experienced in an ultrasound-guided thread transecting technique, performed the procedures independently. The distal-to-proximal and proximal-to-distal approaches were performed in 6 hands each to determine which is safer and more effective. After the p…

Fluoroscopic images of the sacroiliac joint alone are unable to identify simulated flexion or extension malreduction of the anterior pelvic ring in AO/OTA 61-B2.3 pelvic injuries.

PURPOSE: Reduction of AO/OTA 61-B2.3 (APC2) pelvic fractures is challenging in the setting of anterior ring comminution. The anterior ring is visually much simpler to evaluate for flexion or extension hemipelvis deformity than the posterior ring, except in the setting of comminution, necessitating some other visual reference to judge hemipelvis reduction. We sought to test whether pelvic inlet and outlet fluoroscopy of the contours of the sacroiliac joint could be used in isolation to judge hemipelvis flexion or extension. METHODS: Symphyseal and anterior SIJ ligaments were cut (6 cadaveric p…

A novel therapeutic pathway to the human cochlear nerve.

Traditional approaches to the human cochlear nerve have been impeded by its bony encasement deep inside the skull base. We present an innovative, minimally invasive, therapeutic pathway for direct access to the nerve to deliver novel regenerative therapies. Neuroanatomical studies on 10 cadaveric human temporal bones were undertaken to identify a potentially safe therapeutic pathway to the cochlear nerve. Simulations based on three-dimensional delineation of anatomical structures obtained from synchrotron phase-contrast imaging were analyzed. This enabled the identification of an approach to t…
