Compulsive buying.

BACKGROUND: Compulsive buying is a chronic, repetitive purchasing that becomes a primary response to negative events and feelings, and may i … about the neurobiology and genetics of compulsive buying and relatively little about its treatment. Cognitive behavioral therapy has some efficacy, but no medication has been effective in controlled trials. CONCLUSIONS: Compulsive buying can be described as a behavioral dependence. A great deal of future research is needed to improve our understanding of compulsive buying.

Leflunomide-associated fatal cryptococcal meningoencephalitis presenting as rapidly progressive dementia: a case report.

BACKGROUND: Cryptococcal meningoencephalitis is a serious opportunistic infection, classically associated with advanced immunosuppression. The increasing use of immunomodulatory drugs, such as leflunomide, has broadened the at-risk population. CASE REPORT: A 66-year-old woman with rheumatoid arthritis, on long-term leflunomide therapy, presented with progressive cognitive decline, behavioral changes, and myoclonus over a six-week period. The initial presentation mimicked rapidly progressive dementia. Cerebrospinal fluid analysis showed elevated opening pressure, hypoglycorrhachia, and hyperpr

Brain-targeted delivery of siRNA via non-viral delivery systems, the therapeutic strategy for Alzheimer's disease-Unveiling challenges and prospects.

Alzheimer's disease (AD) is a neurodegenerative disorder characterized by cognitive and behavioral issues, posing significant public health challenges. Small interfering RNAs (siRNAs) offer the potential to selectively silence AD-related pathogenic genes. This review first outlines the diverse pathogenic mechanisms and hallmark pathologies of AD, then spotlights the key genes now being silenced by siRNA for therapeutic intervention. These genes encompass those directly implicated in amyloidogenesis, tau phosphorylation, and neuroinflammation, along with those aberrantly up-regulated and associ

Chronic Stress and Headaches: The Role of the HPA Axis and Autonomic Nervous System.

Chronic stress significantly influences the pathogenesis of headache disorders, affecting millions worldwide. This review explores the intricate relationship between stress and headaches, focusing on the dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis and autonomic nervous system (ANS). Persistent stress could lead to neuroinflammation, increased pain sensitivity, and vascular changes that could contribute to headache development and progression. The bidirectional nature of this relationship creates a vicious cycle, with recurrent headaches becoming a source of additional stress

Functional Tremor.

Functional tremor (FT) is the most prevalent subtype of functional movement disorders, characterized by variability, distractibility, and entrainability on clinical examination. Diagnosis relies on positive clinical and electrophysiological signs, shifting emphasis from a diagnosis of exclusion to "rule-in" criteria. Surface electromyography and tremor analysis are essential tools in establishing the diagnosis. Pathophysiology involves abnormal motor co-activation, disrupted volitional awareness, and impaired predictive processing, resulting in tremor perceived as involuntary despite intact mo

Extracellular vesicles in the treatment of neurological disorders.

Extracellular vesicles (EVs) are small, cell-derived membranous particles containing various nucleic acids, proteins, and lipids that play essential roles in intercellular communication. Evidence indicating that part of the regenerative benefit from stem cell therapy arises through EVs released from transplanted cells created interest in using EVs for clinical applications. EVs from various cellular sources, including mesenchymal stem cells, neural stem cells, and glia, are efficacious in models of neurological disease. In these models, EVs attenuate reactive gliosis, neuronal death, pro-infla

Extinction blunts paraventricular thalamic contributions to heroin relapse.

Here, we use optogenetics and chemogenetics to investigate the contribution of the paraventricular thalamus (PVT) to nucleus accumbens (NAc) pathway in aversion and heroin relapse in two different heroin self-administration models in rats. In one model, rats undergo forced abstinence in the home cage prior to relapse testing, and in the other, they undergo extinction training, a procedure that is likened to cognitive behavioral therapy. We find that the PVT→NAc pathway is both sufficient and necessary to drive aversion and heroin seeking after abstinence, but not extinction. The ability of ext…

The treatment of cocaine use disorder.

Cocaine use continues to be a serious worldwide public health problem. Cocaine abuse is associated with substantial morbidity and mortality. Cocaine overdose deaths are increasing in the United States and, in certain populations, outnumber heroin and opiate overdose deaths. Psychosocial treatments remain the treatments of choice for cocaine use disorder (CUD), with standard approaches including contingency management and cognitive behavioral therapy. However, the effect sizes of these treatments are not large, and they are not effective for most patients. Consequently, investigators have sough…

Usability and Naturalness of Videoconference-Based Exposure and Response Prevention for Obsessive-Compulsive Disorder at the Patients' Homes: A Pilot Study.

The treatment of choice for obsessive-compulsive disorders is cognitive behavioral therapy, including exposure and response prevention (ERP). Therapist-supervised ERP is more effective than self-controlled ERP, and exposure should take place at the patient's home. This standard might be achieved with ERPs at home via videoconference supervision from the therapist. The aim of this study was to (a) assess the usability and naturalness of videoconference exposure and (b) to compare it with face-to-face exposure at an inpatient clinic and exposure at home alone regarding therapeutic alliance, pati…

Irritable Bowel Syndrome: A Review for General Pediatricians.

Irritable bowel syndrome (IBS) is a prevalent gastrointestinal disorder characterized by abdominal pain or discomfort and altered bowel habits. The bothersome and chronic nature of IBS symptoms negatively affects patients' activity level and quality of life, imparting an economic burden on the patient, the family, and the health care system. Research is still trying to elicit a clear pathophysiologic mechanism for IBS, and its clinical presentation and manifestations vary from patient to patient. The importance of a comprehensive history and physical examination cannot be emphasized enough in

The efficacy of Acceptance and Commitment Therapy: an updated systematic review and meta-analysis.

Acceptance and Commitment therapy (ACT) has attracted a lot of interest during the last 10-15 years with a strong increase of the number of randomized controlled trials (RCTs). The present review and meta-analysis includes 60 RCTs (4234 participants) on psychiatric disorders, somatic disorders, and stress at work. The mean effect size across all comparisons was small (0.42). Compared to the Öst (2008) meta-analysis there was no significant improvement in methodological quality and deterioration in effect size (from 0.68). When ACT was compared to various forms of cognitive or behavioral treatm
