A prospective evaluation of anorectal function after total mesorectal excision in patients with a rectal carcinoma.

BACKGROUND: Rectum resection with total mesorectal excision (TME) and neorectal anastomosis often compromises anorectal function. Insight into the underlying mechanisms is lacking. Therefore, a prospective study was designed to investigate the relationship between clinical and functional outcomes preoperatively and postoperatively. METHODS: Eleven patients with rectal cancer were examined before and 4 and 12 months after surgery and compared with 11 healthy volunteers (HVs). Anorectal (neorectal) function was examined by clinical outcome questionnaire, anal manometry, rectal compliance, and s

Prophylactic ureteral catheterization in colon surgery. A five-year review.

PURPOSE: The preoperative placement of prophylactic ureteral catheters in operations of the distal colon is both commonplace and controversial. We assessed the frequency, safety, and effectiveness of their use over a five and one-half-year period in a teaching hospital. METHODS: The charts of 561 consecutive patients who underwent sigmoid or rectosigmoid colectomy from 1986 to 1991 were analyzed for age, sex, diagnosis, type of colectomy, placement of ureteral catheters, and ureteral complications. RESULTS: Ureteral catheterization was attempted in 92 patients (16.4 percent); it was successf

Left-Sided Dominance of Early-Onset Colorectal Cancers: A Rationale for Screening Flexible Sigmoidoscopy in the Young.

BACKGROUND: National databases show a recent significant increase in the incidence of colorectal cancer in people younger than 50. With current recommendations to begin average-risk screening at age 50, these patients do not have the opportunity to be screened. We hypothesized that most of the cancers among the young would be left sided, which would create an opportunity for screening the young by flexible sigmoidoscopy. OBJECTIVE: This study aims to analyze the anatomic distribution of sporadic colorectal cancers in patients under the age of 50. DESIGN: This is a retrospective review of a p

CT-colonography after incomplete colonoscopy: what is the diagnostic yield?

BACKGROUND: Computed tomography-colonography is a diagnostic modality that can be used when the colon is not completely intubated during colonoscopy. It may have the additional advantage that information on extracolonic lesions can be obtained. OBJECTIVE: The aim of this study was to investigate the yield of CT-colonography of relevant intra- and extracolonic findings in patients after incomplete colonoscopy. DESIGN: This was an observational, retrospective study. DATA SOURCES: Data were be obtained from standardized radiology and endoscopy reports and electronic medical records. STUDY SEL

Anorectal atresia with gross terminal colonic distension in Africa.

UNLABELLED: Anorectal malformation (ARM) is a group of significant birth defects with geographic variation in incidence, individual phenotypes and regional geographic subtypes occurring in approximately 1:5,000 live births. Anorectal atresia with gross terminal colonic distension in the presence of an absent anal canal has also been known as 'pouch colon' which is mainly associated with the Indian subcontinent. Its prevalence is unknown but it is rare, possibly representing a fraction of 1 % of ARM. The problem seems to revolve around the gross terminal distension of the distal pouch which rem

The mismatch repair protein status of colorectal small cell neuroendocrine carcinomas.

Small cell neuroendocrine carcinoma (SCNC) of the colorectum is a rare and highly aggressive malignancy. It can be associated with conventional-type adenocarcinoma, and an overlying adenoma can often be identified. A disproportionate number has been noted to arise in the right colon. Although some phenotypes (eg, mucinous adenocarcinoma) have been shown to be associated with deficient mismatch repair (MMR) and thus microsatellite instability (MSI), the MMR protein status of colorectal SCNCs has not been investigated. This study investigated the status of 3 MMR proteins, hMLH1, hMSH2, and hMSH6

Endoscopic submucosal dissection for a colorectal tumor with coexisting muscle-retracting sign and underlying diverticulum: the first reported case.

Endoscopic submucosal dissection (ESD) for colorectal lesions becomes particularly challenging in the presence of the muscle-retracting sign (MRS), and the risk of perforation is further increased when a diverticulum underlies the lesion. We report a case of ESD for a 25-mm Paris type 0-Is tumor in the sigmoid colon surrounded by multiple diverticula. During ESD, a broad MRS obscured the appropriate submucosal plane. Careful dissection revealed a diverticulum directly beneath the lesion, indicating a focal defect of the muscularis propria. Subsequently, an intraoperative perforation occurred b

Metastatic breast cancer diagnosed on upper gastrointestinal endoscopy.

A woman in her late 60s was referred with a positive faecal immunochemical test and change in bowel habit; a suspicion of gastrointestinal (GI) cancer led to straight-to-test vetting for colonoscopy, which was unsuccessful due to a nontraversable, fixed sigmoid colon, secondary to prior hysterectomy and adhesions.Radiological evaluation with CT scanning revealed an abnormally thickened stomach, with subsequent upper GI endoscopy and biopsy findings confirming the presence of invasive poorly differentiated carcinoma consistent with metastatic invasive lobular carcinoma originating from the brea

Left-Sided Acutely Irreducible Terminal Ileum Hernia With Cecum, Appendix, and Sliding Sigmoid: Case Report and Review of the Literature.

Amyand's hernia is the presence of the vermiform appendix within the inguinal hernia sac, while a sliding hernia involves the herniation of a retroperitoneal organ outside the abdominal cavity. The hernia sac can contain various intraabdominal organs, including the small bowel, cecum, appendix, omentum, or ovary and fallopian tube. A hernial sac containing the appendix on the left side is rare, and the presence of other organs in combination is also uncommon. We are presenting a case of a 45-year-old male who presented with irreducible left inguinal swelling and obstructing symptoms. Interesti

Prophylactic intestinal resection following carbon-ion radiotherapy for locally recurrent rectal cancer: a case report.

Treatment of locally recurrent rectal cancer (LRRC) after surgery is often complex and challenging. A 52-year-old man received emergency surgery (Hartmann's procedure) for bowel perforation caused by a huge sigmoid colon cancer, followed by treatment for concurrent advanced lower rectal cancer with neoadjuvant chemoradiotherapy and abdominoperineal resection. A solitary lung metastasis emerged afterwards, and was surgically removed. However, the patient developed LRRC in front of the sacrum. As surgical resection for the local recurrence was considered too invasive, carbon-ion radiotherapy (CI

The Hidden Hernia: A Rare Sciatic Hernia Masquerading as Gluteal Pain in an Elderly Woman.

Sciatic hernias are one of the rarest pelvic floor hernias and a highly uncommon cause of gluteal or abdominal pain. Their variable presentation often delays diagnosis, particularly in frail patients. We report a case of a 68-year-old woman with severe chronic obstructive pulmonary disease and chronic comorbidities who presented with vague abdominal and gluteal pain. CT imaging revealed a left-sided sciatic hernia involving the sigmoid colon without obstruction. She underwent a successful open hernia reduction and sigmoidopexy, avoiding mesh due to high operative risk. Sciatic hernias can mimi

Endoscopic lavage for an infected pelvic hematoma in a patient with pelvic sepsis after anterior pelvic exenteration.

Anastomotic leakage and subsequent pelvic sepsis are serious complications after surgery for pelvic malignancies, particularly challenging due to the large pelvic cavity dead space post-exenteration. We report a 47-year-old man treated for a severely infected pelvic hematoma and sepsis following anastomotic leakage after anterior pelvic exenteration. Post robot-assisted exenteration for locally advanced sigmoid colon cancer treated with neoadjuvant chemotherapy, a pelvic abscess from anastomotic dehiscence was identified. Initial CT-guided drainage and subsequent laparoscopic drainage were per

Serum β‑hCG as a prognostic marker for colorectal adenocarcinoma in women of reproductive age: A case report and literature review.

Serological and histological examinations have unveiled the presence of free β subunit of human chorionic gonadotropin (β-hCG) in some cases of colorectal adenocarcinoma. The present study describes an unusual case of a young female patient afflicted with rapidly advancing adenocarcinoma of the sigmoid colon. Despite the absence of pregnancy indications on an ultrasound and curettage, a notable elevation of β-hCG was detected in the serum, alongside positive staining in the tumor tissue. β-hCG-positive colorectal adenocarcinoma is associated with a lower survival rate, tissue invasion and meta

Enteritis of the ileal conduit as an adverse event related to immune checkpoint inhibitor use.

INTRODUCTION: Gastroenterocolitis is one of the adverse events related to immune checkpoint inhibitors. However, inflammation of the intestinal lesion used for urinary diversion is not well known as an adverse event related to their use. CASE PRESENTATION: A patient with metastatic bladder cancer was administered pembrolizumab as second-line treatment. After 12 days of administration, he felt abdominal distention. Computed tomography demonstrated thickening of the ileal conduit wall and bilateral hydronephrosis. Biopsy of the ileal conduit revealed inflammatory granulation tissue. Biopsy of t

[A Case of Phenytoin Intoxication Due to Interaction with Capecitabine].

We report a case of phenytoin(PHT)toxicity due to a drug interaction with capecitabine started as postoperative adjuvant chemotherapy for rectal cancer. Nevertheless the patient was able to complete 8 courses with therapeutic drug monitoring (TDM). Case: 42-year-old female, diagnosed with epilepsy at age 15, taking PHT. She started CAPOX therapy as postoperative adjuvant chemotherapy for recto-sigmoid colon cancer. 23 days after starting capecitabine, she presented with dizziness and weakness of the legs. She was diagnosed with PHT intoxication because her serum PHT concentration was elevated

A case of bloodstream infection caused by Ruminococcus gnavus without gastrointestinal involvement.

We report a case of bloodstream infection due to Ruminococcus gnavus (R. gnavus) associated with pelvic abscess in a 74-year-old female patient undergoing radiotherapy for cervical cancer. Gram staining of positive anaerobic blood cultures revealed short chains of gram-positive cocci. Matrix-assisted laser desorption ionization time-of-flight mass spectrometry was performed directly on the blood culture bottle, and 16S rRNA sequencing identified the bacterium as R. gnavus. There was no leakage from the sigmoid colon to rectum on enterography, and R. gnavus was not found in the culture of her p

Unusual paediatric sigmoid perivascular epithelioid cell tumour with regional lymph node metastasis treated using gemcitabine and docetaxel: a case report and literature review.

Perivascular epithelioid cell tumour (PEComa) is an extremely rare neoplasm with distinctive morphology and specific expression of immunohistochemical markers. The lesion is typically diagnosed in middle-aged women, with few reports of paediatric cases, and there is no standardized treatment for the tumour type. Here, the case of a 17-year-old female, who presented with painless haematochezia for 2 days and was diagnosed with gastrointestinal PEComa of the sigmoid colon with regional lymph node metastasis after serial examination, is presented. She was treated by surgical resection of the tumo

Successful Resolution of Fecal Impaction During Endoscopy Using a Looped Guidewire.

Fecal impaction is the impaired excretion of a large fecal mass, and mild cases are treated by enema and osmotic laxatives. However, treatment-resistant cases need more invasive alternatives. A woman in her 60s presented with abdominal discomfort. Her abdomen was soft and without tenderness. Computed tomography revealed a large mass of feces in her sigmoid colon and no intestinal dilatation proximal to the mass. Endoscopy confirmed a fecal mass occupying the lumen. A glycerin enema, oral administration of polyethylene glycol, and enteral administration of amidotrizoic acid during colonoscopy w

Reactive arthritis: an unusual presentation of acute Clostridioides difficile colitis.

A 20-year-old Caucasian man with a history of psoriasis presented to the emergency department due to a 2-week history of severe polyarthralgia and a 3-week history of non-bloody diarrhoea. The initial workup 2 days prior in an urgent care clinic returned negative for all enteric pathogens including Clostridioides difficile nucleic acid amplification test. Investigations revealed colitis on CT and pseudomembranous colitis on colonoscopy. The aspirate returned positive for C. difficile toxin. Tissue biopsies of the ascending, transverse, sigmoid colon and rectum were negative for chronicity to s

[A Case of Good Quality of Life(QOL)and Favorable Response to Transarterial Chemoembolization(TACE)against Synchronous Multiple Liver Metastases].

We report a case of good quality of life(QOL)and favorable response to transarterial chemoembolization(TACE)against synchronous multiple liver metastases. An 85-year-old man was admitted to our hospital because of melena. Colonoscopy showed multiple type 2 tumors in the sigmoid colon and upper rectum. CT and EOB-MRI examinations revealed that there were multiple liver metastases. Because of his age and surgical stress, he underwent a laparoscopic Hartmann's procedure. After the resection of the primary tumor, he received tegafur/uracil for his liver metastases. However, he discontinued receivi
