In Situ Amplified Mutational mRNA Imaging Using a Spatially Confined CRISPR Nanoplatform.

Highly sensitive spatial analysis of RNA mutations is essential for understanding cellular heterogeneity and disease mechanisms. Herein, we developed an integrated CRISPR/Cas13a-based nanoprobe system for rapid detection of RNA in tissue sections (Integrated CRISPR/Cas13a-based RNA Rapid Detection, InCasRD). Unlike conventional "always-on" probes that rely on accumulated probe hybridization, InCasRD leverages the trans-cleavage activity of Cas13a to achieve spatially confined signal amplification and a high signal-to-background ratio (SBR). Using InCasRD, we achieved imaging of multiple target

Biomarkers for advancing diagnosis and prognosis in stroke.

The complexity and incomplete understanding of the pathophysiology of stroke poses substantial challenges to personalised medicine and the development of novel therapies, as reflected in the neutral results of many randomised trials. Current clinical algorithms lack the precision to diagnose stroke before hospital admission, predict disease progression and recovery, or assess recurrence risk. Biomarkers are urgently needed, yet the absence of a robust evidence base hinders their clinical translation. To lay the groundwork for addressing this knowledge gap, an international multidisciplinary pa

Recent advances in the bioanalysis of acylcarnitines: Methodologies, challenges, and clinical perspectives.

Acylcarnitines (ACs) are a diverse class of fatty acid esters of L-carnitine that serve as critical mediators in energy homeostasis and mitochondrial function. Beyond their classical role in newborn screening for inborn errors of metabolism, ACs have emerged as promising biomarkers for complex pathologies, including cardiovascular diseases, diabetes, and drug-induced toxicities. However, the accurate quantification and comprehensive profiling of ACs in biological matrices remain analytically challenging due to their broad polarity range, vast concentration disparities, and the presence of isom

Transfer RNA-derived small RNAs in female reproductive disorders: emerging roles and clinical implications.

Female reproductive disorders present significant challenges worldwide. Transfer RNA-derived small RNAs (tsRNAs), a structurally diverse class of non-coding RNAs mainly comprising tRNA halves (tiRNAs) and tRNA-derived fragments (tRFs), have emerged as key regulators in human health and disease. This review outlines the biogenesis and functions of tsRNAs and summarizes their mechanistic involvement and clinical implications across a range of reproductive pathologies. In pregnancy-related complications, such as embryo implantation failure and recurrent spontaneous abortion, aberrant tsRNA profil

Uncovering hidden biomarkers in systemic lupus erythematosus through mqTrans analysis.

Transcriptomic data is a valuable resource in precision medicine, providing crucial insights into disease diagnosis and treatment. Differential regulation relationship analysis, focusing on changes in regulatory relationships between different phenotypes, is an essential research direction to elucidate the mechanisms of complex molecular networks. However, many existing studies overlook associations with non-differential genes that exhibit quantitative changes in phenotypes. This study analyzes transcriptomic features by examining correlations among high-dimensional features through feature co

Endothelial dysfunction in APS: advancing pathophysiological understanding to improve management.

Endothelial dysfunction (ED) is a hallmark of antiphospholipid syndrome (APS) driven by chronic antiphospholipid antibody (aPL) exposure. Beyond acute thrombotic events, ED contributes to atherosclerosis, vascular remodelling, stenosis and multi-organ manifestations, positioning the endothelium as a putative target for disease monitoring and therapeutic intervention. In this review, we integrate new experimental and clinical studies with emerging data presented at recent international meetings that advance our understanding of endothelial pathophysiology in APS. These studies reveal novel APS

Development of a Highly Sensitive pH Probe for Spermatozoa and Its Application in the Etiological Diagnosis and Treatment of Asthenospermia.

Declining fertility rates, mainly due to reduced sperm quality, challenge the health of mankind. Thus, there is an urgent need for advanced male infertility diagnostics. Despite the known importance of sperm intracellular pH (pHi) in sperm activity during fertilization, the absence of tools to detect subtle changes in spermatozoa limits the knowledge and clinical application of sperm pHi. Here, we developed RCPH, a novel dual-color fluorescent probe, demonstrating high precision (ΔpHi < 0.1) in monitoring spermatozoa pHi with spatial-temporal response. In a cohort of 79 healthy individuals and

A comprehensive toolkit for analyzing cell-free DNA genomic sequencing data in liquid biopsy.

Liquid biopsy via plasma cell-free DNA (cfDNA) is transforming precision medicine by enabling non-invasive insights into the genomic and epigenomic landscapes of disease, especially cancer. However, the increasing diversity of cfDNA-derived features and machine learning applications has outpaced the availability of unified, versatile computational tools. To bridge this gap, we present cfDNAanalyzer-a user-friendly toolkit for streamlined cfDNA analysis, integrating feature extraction, selection, and machine learning model construction. Designed mainly for researchers and clinicians with limite

DKK1 in Cancer: A Bench-to-Bedside Review of Molecular Mechanisms and Clinical Applications.

Dickkopf-1 (DKK1) is a secreted glycoprotein that traditionally acts as an antagonist of canonical Wnt/β-catenin signaling. Although it functions as a tumor suppressor in some specific biological background and disease stages, growing evidence links DKK1 to tumor progression, immune evasion, and therapy resistance in a variety of multiple malignancies. This review provides a comprehensive bench-to-bedside overview of DKK1 in cancer. We first delineate how DKK1 regulates both Wnt-dependent and Wnt-independent pathways. From a clinical perspective, we evaluate the application potential of DKK1 a

The Fastest NICU Genome is One Completed (or at Least Coordinated) Prenatally: A Joint Perspective From United States-Based Maternal-Fetal Medicine and Neonatologist-Geneticists.

Genomic sequencing (GS) in the neonatal period has advanced over the past several years. Results are available within days in many centers and cost-effectiveness and utility data continue to accumulate. Though less prevalent in clinical practice, prenatal GS is also demonstrating increasing evidence of efficacy. Clearly, information on a genetic disorder's presence or absence is very useful for families and providers, both during pregnancy and postnatally in the intensive care units. Precision therapies for genetic disorders continue to advance in the pediatric space and, in some instances, ar

A multi-center study on the consistency of drug sensitivity testing in patients with acute myeloid leukemia.

Clinical implementation of patient cell-based drug testing is hampered by inconsistencies and irreproducible results. Using linear mixed models and drug response data from four cohorts of patients with acute myeloid leukemia (n = 993 samples), we identified sources of variability in ex vivo drug sensitivity testing across the studies. We identified several experimental factors, including the time until sample was tested, the type of culture media and viability assay, which led to systematic differences in drug sensitivity profiles. We further investigated the effects of sample types (fresh or

Molecular mechanisms and translational prospects in osteoporosis.

Osteoporosis is the most prevalent chronic metabolic bone disease, affecting approximately 200 million individuals worldwide. With the rapid acceleration of global aging, osteoporosis has emerged as a major global health challenge, significantly increasing the occurrence rate of fractures and the associated medical burden. In recent years, considerable progress has been made in understanding the molecular mechanisms underlying osteoporosis, including bone remodeling, osteoblast-osteoclast communication, and signaling pathways such as RANKL/RANK/OPG and Wnt/β-catenin. These mechanistic studies

Deciphering the genetic underlying causes of sex differences in multiple myeloma incidence and mortality.

Multiple myeloma (MM) is a genetically complex cancer that has a higher incidence and mortality in males when compared to females. These sex differences are well documented epidemiologically, yet their biological mechanisms remain uncharacterized. We hypothesized that sex-specific genetic regulation by somatic mutations affects MM cell gene expression differently in each sex and subsequently leads to differential functional genetic dependencies between the two sexes. To test this, we first identified genes with sex-biased expression in MM, then identified sex-specific somatic regulators of the

Urine-Derived Cells in Kidney Transplantation: Linking Cellular Phenotypes, Secretome Signatures and Multi-Omic Technologies.

Kidney transplant monitoring and early identification of graft dysfunction are central needs for long-term graft survival. Although kidney biopsy represents the gold standard in diagnostic procedures, its invasiveness may limit frequent longitudinal evaluation, highlighting the necessity of non-invasive diagnostic tools. Urine has emerged over the years as an easily obtainable source of cellular and molecular components derived from transplanted kidneys. Particularly, exfoliated cells and extracellular vesicles (EVs) represent complementary and interconnected markers able to reflect tissue inj

Integrative pipeline to profile and target endocrine therapy-insensitive cell populations in ER+ breast cancer.

Up to 40% of patients with estrogen receptor positive breast cancer will experience relapse, either while on endocrine therapy (ET) or after ET is completed. A major contributor to ET failure is the presence of ET-insensitive cell populations within tumors. Here, we developed an analytical pipeline to systematically identify and target these populations by integrating single-cell RNA sequencing of ER+ tumors from the FELINE clinical trial with functional validation in a panel of patient-derived xenograft organoid models. We found that ET-insensitive cells are detected in all tumors regardless

Gestational diabetes mellitus and life-course health.

Gestational diabetes mellitus (GDM), traditionally defined as a disorder of hyperglycaemia with first onset or recognition during pregnancy, is a common condition which is complicated, costly, and controversial. Advances in understanding and awareness of GDM pathophysiology are resulting in an increasingly loud call for the application of personalised medicine approaches to both research in, and clinical care of, GDM. There is a shift away from viewing GDM as a condition to be diagnosed at 24 weeks of gestation and followed until 12 weeks postpartum, to a condition of metabolic vulnerability t

Targeting TDP-43 in ALS: Regulatory hurdles, trial design deficiencies, and the causal evidence gap for CTx1000.

The therapeutic landscape for amyotrophic lateral sclerosis (ALS) has been characterized by decades of clinical trial failures, often attributed to biological heterogeneity, end-point insensitivity, and a profound evidence gap regarding target engagement. With TAR DNA-binding protein 43 (TDP-43) aggregation emerging as a hallmark feature in the vast majority of ALS cases, new precision-medicine modalities - most notably the proteolysis-targeting chimera (PROTAC) CTx1000 - aim to address the underlying causal pathology through selective degradation of mislocalized TDP-43. This review critically
