OBJECTIVE: To compare the extrapyramidal features of pathologically confirmed cases of diffuse Lewy body disease (DLBD) and Parkinson's disease (PD).
BACKGROUND: The proportion of pathologically confirmed cases of DLBD diagnosed clinically as PD is as high as 88%. Few papers focus specifically on the extrapyramidal features of DLBD. Further characterization of these features might facilitate antemortem diagnosis, in particular, distinguishing DLBD from PD.
METHODS: Review of prospective and retrospective clinical data on a large series of pathologically diagnosed cases of DLBD (N = 31) and P

Spinal cavernous hemangioma is rare, and it is extremely rare for cavernous hemangioma to develop in the cauda equina. There has been only one report of hydrocephalus associated with cavernous hemangioma in the cauda equina. We report a case of cavernous hemangioma in the cauda equina diagnosed on the basis of the headaches due to hydrocephalus. A 67-year-old man was being treated for Parkinson's disease because of tremor of both upper extremities for several years. In December 1991 he complained of occasional headaches. On February 15, 1992 the headaches became severe and frequent, with nause

A study of parkinsonian signs was conducted in 29 patients fulfilling Lees' criteria for progressive supranuclear palsy, or Steele-Richardson-Olszewski syndrome, and 2 patients with incomplete ophthalmoplegia but 4 other cardinal features. The patients comprised 17 men and 14 women and their ages at onset ranged from 52 to 77 yrs (mean, 65.1). Parkinsonian features: akinesia, rigidity or resting tremor were present in 29 cases and appeared a mean 0.5 yrs (range 0-3) after onset. In 9 cases (29 p. 100), the parkinsonian features were similar to those of Parkinson's disease, including a resting

We report a 41-year-old male of citrullinemia associated with argininosuccinate acid synthetase deficiency. He was admitted to the Hitachi General Hospital because of finger tremor, restlessness and urinary incontinence. He had short stature and a poor appetite. Laboratory evaluation was summarized as follows: mild hypoglycemia, low plasma cortisol levels, delayed response of 17-OHCS and 17-KS to ACTH administration in urine, and delayed response of plasma ACTH level to insulin administration. In this case, ACTH deficiency is estimated to be a dysfunction of the hypothalamus. Replacement thera

High doses of inhaled salbutamol produce substantial improvements in airway response in patients with asthma, and are associated with dose-dependent systemic beta-adrenoceptor responses. The purpose of the present study was to investigate whether tachyphylaxis occurs during prolonged treatment with high dose inhaled salbutamol. Twelve asthmatic patients (FEV1, 81 +/- 4% predicted), requiring only occasional inhaled beta-agonists as their sole therapy, were given a 14-day treatment with high dose inhaled salbutamol (HDS), 4,000 micrograms daily, low dose inhaled salbutamol (LDS), 800 micrograms

Baboons received continuous intragastric infusions of diazepam (20 mg/kg per day) for one or more months. While diazepam treatment continued, baboons received intragastric doses of Ro 15-1788 (0.032-32.0 mg/kg) or CGS 8216 (1.0-100.0 mg/kg) at intervals of two or more weeks. Baboons were observed following administration of these antagonists for the presence of precipitated withdrawal signs. The following results were obtained: (1) both Ro 15-1788 and CGS 8216 produced signs of precipitated withdrawal in the baboon; (2) a more severe overall withdrawal syndrome was precipitated with Ro 15-1788

Drug-induced movement disorders are a rare but distressing complication of many anesthetic agents. When such reactions occur, it is difficult to identify a causative agent. We report a case of recurrent ataxia, dysarthria, chorea, and tremors following multiple anesthetics in a 32-year-old patient with a history of significant postoperative nausea and vomiting. Over the course of four anesthetics, the exclusion of propofol, along with collaborative planning via a biopsychosocial approach, prevented the onset of a functional movement disorder. Therefore, for the first time, we highlight a case

A 50-year-old male patient presented with a 10-year history of progressive cerebellar ataxia, multifocal dystonia with dystonic tremors, and oculomotor abnormalities including bilateral ptosis, slow saccades, and reduced range of ocular movements. There were no signs of cognitive impairment, parkinsonism, autonomic dysfunction, or muscle weakness, and the family history was unremarkable. This case presents the diagnostic approach to adult-onset cerebellar ataxia with dystonia and abnormal eye movements. In this era of genomic testing, our case highlights the role of clinical phenotyping and th

Hyperthyroidism is an endocrine disorder characterized by excess thyroid hormone production. Its classic symptoms include weight loss, palpitations, tremors, and anxiety. We present a case of a 33-year-old female who initially presented with anxiety-like symptoms, leading to a misdiagnosis of generalized anxiety disorder. Upon further evaluation, the patient was diagnosed with hyperthyroidism, highlighting the importance of evaluating endocrine causes in patients presenting with anxiety-like symptoms. This case report underscores the significance of a comprehensive medical evaluation in patien

"Vascular parkinsonism (VP)" is a condition which presents with the clinical features of parkinsonism that are presumably caused by cerebrovascular disease. It is classically described as symmetrical lower-body parkinsonism with gait unsteadiness and absence of tremors and is usually associated with pyramidal signs. Treatment for VP remains challenging as available data on the efficacy of current treatment options are contentious. VP is generally considered to be poorly responsive to levodopa, the most effective of the current treatment modalities for parkinsonism. However, there is evidence t

The newborn infant is prone to clinical motor phenomena that are not epileptic in nature. These include tremors, jitteriness, various forms of myoclonus and brainstem release phenomena. They are frequently misdiagnosed as seizures, resulting in unnecessary investigations and treatment with anticonvulsants, which have potentially harmful side effects. Unfortunately, there is a paucity of literature about many of these phenomena in the newborn, and some of the major textbooks refer to these events as nonepileptic seizures, leading to further confusion for the practitioner. The present paper aims

The cases of six patients with pronounced, persistent, isolated gaze-evoked orbicularis oculi myokymia (GEOM) discovered fortuitously are described. None had brain-stem lesions, and two had associated, unusual cranial synkinesis (trigeminofacial, hypoglossal-facial, and spinal accessory-facial). Additional features included asymmetric blinking and pseudoblepharoclonus in a third patient and buccopharyngeal synkinesis and post-deglutional tremors in a fourth. Ptosis and pupillary changes were absent. With the possible exception of one case, GEOM seems to be a congenital benign oculofacial synki

Two dogs were examined because of anorexia, lethargy, muscle tremors, weakness, and seizures that were associated with an acute onset of hypocalcemia. Both dogs had histories of chronic hypercalcemia. Examination of the parathyroid glands revealed infarction of focal parathyroid adenomas, with atrophy of the remaining parathyroid glands. It was concluded that the acute onset of hypocalcemia was caused by infarction of functional parathyroid adenomas that were previously responsible for the cause of persistent hypercalcemia. Infarction of a parathyroid adenoma should be included in a list of di

In a group of spasmophilic subjects, symptoms imputable to the syndrome were compared with EMG signs of ischemic neuromuscular hyperexcitability, to evaluate their significance by logistic statistical analysis. Subjects with a duration of postischemic EMG spontaneous activity greater than the mean of normal values + 2.5 DS were considered positive for a spasmophilic syndrome. The following symptoms were selected as positive by the analysis: palpitations, nail and hairs abnormalities, headache, lipothymia; thermoregulation disorders and tremors were negatively related to the syndrome, while sle

Of 10 horses in a heavily overgrazed pasture, 4 died within 1 week. Clinical signs included muscle tremors, ataxia, reluctance to walk, heavy sweating, and myoglobinuria. Serum creatine kinase, aspartate transaminase, and alkaline phosphatase activities were high. Histopathologic findings were nonspecific. On the basis of clinical signs, clinicopathologic findings, nonspecific histopathologic findings, the condition of the pasture, the identification of numerous white snakeroot plants from which trematone was extracted, and evidence that these plants had been heavily browsed, it was believed t

A chronic and progressive neurological disorder, Parkinson's disease (PD) has a significant influence on patients and their caregivers. Motor symptoms such as postural instability, tremors, and gait dysfunction lead to increased dependency, high caregiver vigilance, and chronic psychological distress in terms of stress, anxiety, and reduced quality of life. Caregivers of patients with PD experience chronic emotional and cognitive distress due to the uncertainty and unpredictability of motor symptoms in daily life, which are not effectively managed by traditional episodic clinical evaluations.

OBJECTIVE: NeoNatal Neurobehavioral Scale (NNNS-II) measures behavioral signs of stress according to systems, including the autonomic and central nervous systems. These systems influence heart rate variability (HRV), which is a biomarker for regulatory capacity. The present study aims to investigate whether patterns of autonomic dysregulation are evident in both behavioral and physiological measures.
STUDY DESIGN: HRV metrics-short- and long-term fractal scaling exponents (α1, α2) and root mean square fluctuations (RMS1, RMS2)-were calculated from the heart rates of 51 preterm infants at 33 w

BACKGROUND: Atypical porcine pestivirus (APPV), genus Pestivirus, is the causative agent of congenital tremor type A-II, a neurological disease in newborn piglets that is characterized by tremors. Although the virus can spread horizontally, clinical disease is observed only after in utero infection. While venereal transmission has been proposed as a potential route, the pathogenesis and transmission dynamics of APPV remain incompletely understood. In this study, we aimed to investigate the presence of APPV in semen from Norwegian breeding boars, its temporal shedding patterns, and the potentia

BACKGROUND: Functional neurological disorder (FND) is prevalent in neurologic clinics. FND includes subtypes, such as Functional Movement Disorder (FMD), which involves movement-related symptoms. There is a lack of reproducible FMD treatment guidelines, especially in outpatient physical therapy (PT). This case series aims to describe detailed interventions and outcomes for individuals with FMD in outpatient clinics.
CASE DESCRIPTIONS: Four patients with FMD, presenting with functional gait abnormality and tremors, participated in PT. Sessions lasted 45 min, once a week, with total number of v
