Gall bladder carcinoid: a case report and review of literature.

Gall bladder carcinoid tumours are rare and usually lack specific symptoms. In most instances, they are incidentally detected after a cholecystectomy and rarely do they manifest with carcinoid syndrome. We report a case of gall bladder carcinoid in a patient who had undergone surgery for rectal carcinoid 10 years back. Occurrence of rectal carcinoid and gall bladder carcinoid in the same patient has not been reported before.

Spontaneous gall bladder perforation: a rare entity in infants.

Spontaneous gall bladder perforation in infants is rare. We report a 3-month-old male infant who presented with progressive abdominal distension, low-grade fever, bilateral hydrocele and acholic stools. Ultrasonography showed free fluid in the peritoneal cavity, which was bile-stained on paracentesis. Surgical exploration revealed sterile biliary peritonitis and a gangrenous gall bladder. Partial cholecystectomy with external biliary drainage resulted in satisfactory recovery.

Acute non-calculous cholecystitis in burns.

Seven burned patients developed acute cholecystitis within 10-37 days of injury, associated with jaundice in 5. There were 6 males and 1 female, with 5 patients aged 28-40 years, the others being 56 and 69 years. Cholecystectomy was performed in all patients with 1 death. It is suggested that the cause of post-burn cholecystitis is traumatic and septic shock with consequent biliary stasis.

Incidentally Detected Gallbladder Adenocarcinoma Presenting as Acute Calculous Cholecystitis: A Case Report and Management Considerations.

Gallbladder cancer is an aggressive malignancy of the biliary tract with a poor prognosis and variable geographic distribution. It may be identified incidentally after a cholecystectomy performed for presumed benign disease, with diagnosis established on final histopathological examination. Surgeons should be aware of this possibility, as incidental detection has important implications for staging and subsequent management. Early recognition and appropriate evaluation are essential to guide treatment and improve outcomes.

Pancreatic Heterotopia of the Gallbladder: A Rare Phenomenon That Might Be on the Rise.

A young patient presenting with recurrent biliary colic had a laparoscopic cholecystectomy. The routine histology on the gallbladder revealed a section of pancreatic tissue in the neck of the gallbladder. Pancreatic tissue separate from the pancreas is known as pancreatic heterotopia and is particularly rare. However, there has been a recent increase in the publications of cases. In this report we present the case history of our patient and discuss it with relevance to the wider literature.

Pseudoaneurysm of the Gastroduodenal Artery: A Rare Complication of Bile Duct Surgery.

Pseudoaneurysm of the gastroduodenal artery (GDA) is an exceptional complication of common bile duct (CBD) resection. We present the case of a 60-year-old woman with a history of cholecystectomy. The patient was admitted to our hospital for surgical management of the cystic dilatation of the CBD. The patient presented on postoperative day 21 with hemodynamic instability related to a pseudoaneurysm of the GDA. An urgent open surgery was performed with dissection and ligation of the GDA.

Emphysematous Cholecystitis and Necrotizing Pancreatitis: Unusual Combination of Critical Pathology in a Healthy Young Patient.

Emphysematous cholecystitis (EC) is a rare form of severe cholecystitis, more common among old, frail males, with comorbidities like diabetes mellitus and peripheral vascular disease. The coexistence of emphysematous cholecystitis with necrotizing pancreatitis is extremely uncommon and in the literature, though it has been reported, no case was of a young patient like the one we are describing. He was treated with laparoscopic cholecystectomy, and his pancreatitis managed conservatively.

Laparoscopic cholecystostomy tube-guided hepatotomy and cholecystolithotomy: alternative strategy for treatment of severe chronic cholecystitis.

Laparoscopic cholecystectomy can be a challenging procedure in gallbladders with chronic disease. We describe a patient with chronic cholecystitis and difficult visualisation of the gallbladder at surgery who underwent laparoscopic hepatotomy along the drainage tube of the cholecystostomy. In this way, the gallbladder was identified to avoid non-visualisation of ductal anatomy. This exceptional solution should be added to the surgical options if anatomical recognition is difficult and complete removal of the gallbladder is too risky.

Neuroendocrine tumor of gallbladder with liver and retroperitoneal metastases and a good response to the chemotherapeutical treatment.

Neuroendocrine carcinoma of the gallbladder is an uncommon disease. We present the case of a 45-year-old woman with a mass located in the gallbladder, whose diagnosis was based on contrast-enhanced ultrasound and magnetic resonance imaging. The tumor involved the liver and retroperitoneum, and was histopathologically confirmed by liver biopsy as a neuroendocrine tumor grade 3. The patient received chemotherapy with good response, followed by surgery with cholecystectomy and partial hepatectomy.

Gallstone ileus of the sigmoid colon: a rare complication of cholelithiasis.

The authors report a case of gallstone ileus of the sigmoid colon in an 80-year-old woman admitted to the hospital with symptoms and signs of large bowel obstruction and asymptomatic cholelithiasis. Radiological investigation (abdominal X-ray and CT scan) showed a large gallstone impacted in the sigmoid colon. At first, the patient was managed conservatively, but the recurrence of the intestinal obstruction required open cholecystectomy, suturing of the colonic fistula and sigmoidectomy.

Cholecystitis caused by a fish bone.

The accidental ingestion of a foreign body is not uncommon. However, the presence of a foreign body in the gallbladder is extremely rare. Here, we present a case of cholecystitis due to a fish bone that may have penetrated through the stomach wall and into the gallbladder without causing peritonitis. A laparoscopic cholecystectomy was performed; a fish bone, measuring 4.0 cm in length, was found in the gallbladder. To the best of our knowledge, this is the first such case to be reported.

Gallstone impacted in the rectosigmoid junction causing a biliary ileus and a sigmoid perforation.

A gallstone ileus is an unusual form of bowel obstruction. Colonic gallstone ileus is rare, difficult to diagnose, and still has a high mortality rate. We present a case of biliary ileus caused by an impacted gallstone, causing necrosis and perforation of the rectosigmoid junction. A Hartmann's procedure was performed to treat the perforation and the obstruction. Cholecystectomy and closure of the cholecysto-colonic fistula were delayed until restoration of the intestinal continuity.

Ascariasis cholecystitis: An unusual cause.

Ascariasis is the most common helminthic infection to infest man. Usually the adult worm lives in the small intestine. Rarely it migrates through the ampulla of vater and enters the common bile duct. We are reporting a case of gall bladder ascariasis causing acute cholecystitis treated by laparoscopic cholecystectomy. Presence of Ascaris lumbricoides in gallbladder is rare entity as it is difficult to reach there due to the narrow and tortuous cystic duct.

Laparoscopic cholecystotomy.

Laparoscopic cholecystotomy (LCT) with primary closure of the gallbladder is a treatment option for patients with symptomatic cholecystolithiasis in a well-functioning gallbladder. In contrast to other gallbladder-preserving minimally invasive or interventional methods, LCT is a one-session procedure avoiding the need for post-operative drainage of the gallbladder by a balloon catheter. LCT does not cause functional disturbances or severe bile duct injury, as is observed after laparoscopic cholecystectomy.

Laparoscopic-assisted rectopexy using a novel hand-access port.

Many surgeons who were ready converts to laparoscopic cholecystectomy did not translate the technique to more advanced procedures. There are several reasons: increased operating times, a steeper learning curve, concern for oncological efficacy, and the loss of manual tactile ability. As shown with this report on laparoscopic assisted rectopexy, many of these difficulties can be overcome using the IntromitTM while still maintaining the benefits of minimally invasive surgery.

[Gallbladder torsion in infants].

The infrequent case of gallbladder torsion and accompanying Meckel's diverticulum in a 3-year-old girl, is described. Detorquation, cholecystectomy and resection of the diverticulum were performed. Condition of the development of the disease as well as its clinical aspects and therapy are discussed. It is suggested to explore the abdominal cavity for other developmental anomalies in the course of the operation. The patient recovered without complications and left the hospital 14 days after the operation.

[Spontaneous external biliary fistula].

Spontaneous biliary cutaneous fistula is a rare complication of inflammatory perforation of the gallbladder. We describe a 70-year-old woman who developed a fistula between the gallbladder and the skin following drainage of an abdominal wall abscess. In such cases primary drainage of the abscess alone is advisable. Only after the active infection has subsided should definitive cholecystectomy and excision of the fistulous tract be carried out.

[Cystic dilatation of the common bile duct initially treated by cystojejunostomy and reoperation 25 years later because of infectious and lithiasic complications].

A case of cystic dilatation of the common bile duct is reported. Initially treated by Roux-Y cystojejunostomy, twenty five years after, the patient presented a cholecystitis. This patient was treated by cholecystectomy and a few months after by excision of the cyst with Roux-Y hepato-jejunostomy. This attitude is proposed to avoid the complications of the cystic dilatation of the common bile duct: secondary biliary cirrhosis, angiocholitis, and malignant degeneration.

Intramural dissection of superior mesenteric artery. A complication of attempted renal artery balloon dilation.

A patient with a right renal artery stenosis and renovascular hypertension was admitted for balloon dilation of the stenotic artery. During the procedure the catheter entered the superior mesenteric artery and caused a mural dissection and occlusion, which was successfully treated by endarterectomy and vein patch angioplasty. Delayed ischemia of the transverse colon required resection and colostomy, but the patient recovered fully after colostomy closure and cholecystectomy were performed.

[Bouveret syndrome].

Bouveret syndrome, a rare form of gallstone ileus predominantly encountered in elderly female patients, is clinically characterized by the sequelae of duodenal obstruction. Two patients observed within one year are presented and the clinical, diagnostic and therapeutic aspects of the disease are discussed. The surgical treatment of choice is one-stage removal of stone, closure of fistula and cholecystectomy completed by a careful search for residual stones in the small and large intestine.
