Ten-year follow-up on the first five inner ear valve implants for intractable vertigo in Sweden.

Ten-year clinical results are reported on the first five inner ear valve implants done in Sweden in 1975 and 1976. If the patient had a positive glycerol dehydration test (n = 4) the chances of excellent results for hearing at one year as well as complete elimination of vertigo were high. At ten years, two of the four patients with positive glycerol dehydration tests maintained excellent hearing and complete elimination of vertigo. One patient's hearing deteriorated, but he was still completely free of vertigo at ten years. One patient had a labyrinthectomy at three years, but now we would hav…

Type II collagen mediated autoimmune middle ear disease: eustachian tube disease, otitis media with effusion and tympanosclerosis.

Intact Eustachian tube and tympanic cavity functions are essential for normal middle ear physiology. Type II collagen is an essential component of ear tissue. Autoimmune response to this type II collagen produces sensorineural hearing loss, vestibular dysfunction, endolymphatic hydrops, otospongiosis-like lesions, Eustachian tube inflammation and Eustachian tube chondritis. In addition, these autoimmune responses occasionally produce otitis media with effusion (not infectious). Tympanosclerosis, which is a frequent sequelae of otitis media with effusions, was also induced by type II collagen i…

Autosomal dominant inheritance of conductive deafness due to stapedial anomalies, external ear malformations and congenital facial palsy.

Three siblings of Indian stock had profound bilateral conductive deafness with variable malformations of the external ears, stapedial abnormalities and facial paralysis. Their mother was similarly affected and inheritance of this private syndrome is evidently autosomal dominant. Some improvement of auditory function was obtained by surgical intervention in the middle ear.

Photobiomodulation as a Potential Regenerative Signal in the Inner Ear: Linking Mitochondrial Redox Signaling to Cell-Fate Regulation.

Photobiomodulation (PBM) is a non-thermal therapeutic modality in which low-intensity red or near-infrared light is used to modulate biological activity. Experimental inner-ear studies report preservation of auditory neurons, attenuation of vestibular injury, improved survival of transplanted stem cells, and changes in otic organoid differentiation. These observations raise the possibility that PBM may influence differentiation-associated cellular states in addition to providing cytoprotection. The classical mechanistic model proposes that photons interact with mitochondrial chromophores, part…

Microsurgical Reconstruction of the Ear and Temporal Region: Structural and Functional Considerations Including Hearing Rehabilitation-A Narrative Review.

Reconstruction of the ear and temporal region presents unique challenges due to the complex anatomy of the lateral skull base and the need to restore both structural integrity and auditory function. Historically managed as separate entities, auricular reconstruction and hearing rehabilitation are increasingly approached in an integrated manner, supported by advances in microsurgical techniques and implantable hearing technologies. This narrative review synthesizes contemporary evidence on microsurgical reconstruction of the ear and temporal region in conjunction with hearing rehabilitation, an…

Pancoast Syndrome and Middle Ear Mass as the First Manifestation of Small-Cell Lung Carcinoma: A Case Report.

Small-cell lung carcinoma (SCLC) is a rare cause of Pancoast syndrome. We present a 54-year-old man with shoulder pain, arm weakness, and a middle ear mass, leading to a diagnosis of SCLC at the lung apex. This case highlights the importance of considering SCLC in the differential diagnosis of Pancoast syndrome. Early recognition and a comprehensive diagnostic approach are crucial for optimizing patient outcomes.

The impact of topical or oral antibiotics in children with acute otitis media on their middle ear, nasopharyngeal, and gut microbiomes.

Acute otitis media (AOM) is a major driver of paediatric antibiotic prescriptions. We assessed the impact of oral and topical antibiotics on middle ear, nasopharyngeal, and gut microbiome compositions, and the gut resistome, in children with AOM and ear discharge (AOMd). Fifty-eight children with AOMd and ear pain and/or fever were randomized to oral amoxicillin suspension (n = 31) or hydrocortisone-bacitracin-colistin eardrops (n = 27) for 7 days. From 57 out of 58 children, baseline, and Week-2 middle ear fluid (MEF) and nasopharyngeal (NP) samples were sequenced, along with baseline, Week-2…

The Effect of Between-Ear Speech Perception Asymmetry on Localization, Spoken Language, and Literacy for Adolescents With Cochlear Implants.

PURPOSE: This study aimed to examine the effects of between-ear speech perception asymmetry on localization, spoken language, and reading in adolescents with early cochlear implants (CIs). METHOD: Eighty adolescents, who have long-term bilateral device use, participated: 10 with bimodal (BM) devices (CI plus hearing aid at the nonimplanted ear) and 70 with bilateral CIs (BCIs; 14 simultaneous, 56 sequential). Participants completed localization, unilateral speech perception, receptive language, and reading tests. Principal component analysis created composite scores for better-ear speech perc…

[Research progress and prospects on the pathogenesis, clinical characteristics, and auditory intervention of congenital external and middle ear malformations].

Congenital external and middle ear malformations (CEMEMs) represent a key etiology of conductive hearing loss, arising from dysplastic development of the first and second pharyngeal arches during embryogenesis. These malformations may occur in isolation or as a localized manifestation of a syndrome. This review synthesizes recent evidence-based medical findings to elaborate on advances in understanding the pathogenic mechanisms, clinical characteristics, and auditory intervention strategies for CEMEMs, thereby offering a reference framework for clinical diagnosis and management.

Right aural myiasis in a hypertensive patient: a rare and disturbing ear, nose, and throat emergency (case report).

Aural myiasis is an uncommon parasitic infestation involving the external auditory canal by fly larvae, typically seen in tropical climates, especially among individuals with poor hygiene or immunocompromised status. Though rare, it can lead to serious complications, including tympanic membrane perforation or intracranial invasion. We describe a 55-year-old man with hypertension who had been experiencing pain and bleeding in his right ear for four days. Live maggots were found in the right external auditory canal during otoscopic examination. It was impossible to see the tympanic membrane. Sys…

A bacterial pangenome association study identifies a role for polyamine metabolism and alkaline tolerance in middle ear infections.

Infection of the middle ear, or otitis media (OM), is a disease that commonly afflicts children, and Haemophilus influenzae is the most prevalent OM-associated bacterium. We sought new insights into mechanisms of pathogenesis by identifying bacterial genes associated with OM disease. We whole-genome sequenced >200 clinical isolates collected over time from 9 healthy and 12 OM-prone children. Genome analysis first annotated and clustered homologous genes from across assemblies, inferred the phylogeny, and identified closely related set of strains, or clonal lineages, finding both transient and …

Metagenomic analysis of the middle ear microbiome: A next-generation sequencing approach in pediatric patients with and without effusion.

OBJECTIVES: Otitis media with effusion (OME) is one of the most common causes of reversible hearing loss in childhood. In recent years, host-microbiota interactions and alterations in microbiota composition associated with health and disease have gained increasing attention in the context of OME. This study aimed to investigate the bacterial microbiota composition of middle ear cavity (MEC) samples obtained from pediatric patients with and without OME. Microbiome differences were analyzed according to clinical groups and variables such as age and gender. METHODS: 16S rRNA-based metagenomic se…

Use of PBPK modelling to extrapolate in vitro porcine ear skin permeability to in vivo human dermal absorption.

Porcine ear skin is widely accepted as a surrogate for human skin in in vitro permeation tests (IVPT); however, differences in permeability between the two have been reported in several studies. Physiologically based pharmacokinetic (PBPK) modelling has been recognised as a valuable tool for supporting the correlation and extrapolation of in vitro data to human in vivo conditions. In the present study, the feasibility of using IVPT data obtained from porcine ear skin was evaluated for the optimisation of model input parameters to improve the prediction of systemic exposure to substances in hum…

Transcriptomic and metabolomic basis of seasonal auditory sensitivity variations in the red-eared slider (Trachemys scripta elegans).

BACKGROUND: Auditory brainstem response (ABR) tests have demonstrated that the peripheral auditory sensitivity of the red-eared slider (Trachemys scripta elegans) exhibits seasonal variation, with enhanced sensitivity observed in spring compared to winter. However, the molecular mechanisms underlying these seasonal shifts remain unclear. RESULTS: Combined transcriptomic and metabolomic analyses revealed significant seasonal changes between sexes. Transcriptomic profiling identified pronounced organ-specific patterns, with the inner ear exhibiting the most profound seasonal reprogramming, as id…
