BMP9 stimulates joint regeneration at digit amputation wounds in mice.

A major goal of regenerative medicine is to stimulate tissue regeneration after traumatic injury. We previously discovered that treating digit amputation wounds with BMP2 in neonatal mice stimulates endochondral ossification to regenerate the stump bone. Here we show that treating the amputation wound with BMP9 stimulates regeneration of a synovial joint that forms an articulation with the stump bone. Regenerated structures include a skeletal element lined with articular cartilage and a synovial cavity, and we demonstrate that this response requires the Prg4 gene. Combining BMP2 and BMP9 treat

[Research progress in occlusal splint therapy for temporomandibular joint disorders].

Temporomandibular disorders (TMD) is a common clinical disease with complex etiology and diverse treatment. There are many studies on the treatment of TMD. Occlusal splint therapy is the most commonly used TMD treatment solution, but its effectiveness remains unclear. A review of the latest literature is therefore conducted to evaluate the effectiveness of splint therapy for TMD.

Estrogen signaling impacts temporomandibular joint and periodontal disease pathology.

Women experience a higher incidence of oral diseases including periodontal diseases and temporomandibular joint disease (TMD) implicating the role of estrogen signaling in disease pathology. Fluctuating levels of estrogen during childbearing age potentiates facial pain, high estrogen levels during pregnancy promote gingivitis, and low levels of estrogen during menopause predisposes the TMJ to degeneration and increases alveolar bone loss. In this review, an overview of estrogen signaling pathways in vitro and in vivo that regulate pregnancy-related gingivitis, TMJ homeostasis, and alveolar bon

Hypermobility of the articular heads of the temporomandibular joint: pathology or variant of the norm?

INTRODUCTION: Among the large contingent of patients with TMJ dysfunctions, there are persons with hypermobility of the joint heads of the l … formance compared to healthy ones. Most of the examined did not note functional discomfort from the joints. With a significant degree of hypermobility of the articular heads, electromyography of the temporal and chewing muscles revealed a lack of synergism in their work. On TMJ zonograms with an open mouth, the articular heads extended anteriorly beyond …

Sacral Dysmorphism and its Implication on the Size of the Sacroiliac Joint Surface.

STUDY DESIGN: This is a retrospective data analysis. OBJECTIVE: The aim of this study was to analyze the prevalence of sacral dysmorphism and its correlation to the size of the sacroiliac joint (SIJ) surface based on computed tomography (CT) scans. SUMMARY OF BACKGROUND DATA: Sacroiliac screw fixation is a widely accepted technique for stabilization of posterior pelvic ring injuries. Safe sacral screw placement may be impaired by sacral dysmorphism. The prevalence and impact of sacral dysmorphism on the size of the SIJ surface is unknown. MATERIALS AND METHODS: In total, 269 CT scans were e

Clinical Applications of 3-Dimensional Printing Technology in Hip Joint.

Three-dimensional (3D) printing is a digital rapid prototyping technology based on a discrete and heap-forming principle. We identified 53 articles from PubMed by searching "Hip" and "Printing, Three-Dimensional"; 52 of the articles were published from 2015 onwards and were, therefore, initially considered and discussed. Clinical application of the 3D printing technique in the hip joint mainly includes three aspects: a 3D-printed bony 1:1 scale model, a custom prosthesis, and patient-specific instruments (PSI). Compared with 2-dimensional image, the shape of bone can be obtained more directly

Joint UK societies' 2019 consensus statement on renal denervation.

Improved and durable control of hypertension is a global priority for healthcare providers and policymakers. There are several lifestyle measures that are proven to result in improved blood pressure (BP) control. Moreover, there is incontrovertible evidence from large scale randomised controlled trials (RCTs) that antihypertensive drugs lower BP safely and effectively in the long-term resulting in substantial reduction in cardiovascular morbidity and mortality. Importantly, however, evidence is accumulating to suggest that patients neither sustain long-term healthy behaviours nor adhere to lif

The Ankle-Joint Complex: A Kinesiologic Approach to Lateral Ankle Sprains.

Copious research exists regarding ankle instability, yet lateral ankle sprains (LASs) persist in being among the most common recurrent musculoskeletal injuries. Key anatomical structures of the ankle include a triform articulating structure that includes the inferior tibiofibular, talocrural, and subtalar joints. Functionally, force absorption and propulsion through the ankle complex are necessary for any task that occurs in weight bearing. For optimal ankle performance and avoidance of injury, an intricate balance between stability and mobility is necessary to ensure that appropriate force tr…

Impact of femoroacetabular impingement and dysplasia of the hip on hip joint sphericity.

BACKGROUND: This paper presents a parametric investigation into the effect of femoroacetabular impingement (FAI) and developmental dysplasia of the hip (DDH) on the sphericity of the femoral supra-equatorial region and acetabulum. METHODS: Radiographic parameters from x-rays, sphericity calculations and visualisation and joint contact area and pressure from FE models of 10 DDH, FAI and normal hips were analysed and compared both within and between hip groups. RESULTS: The sphericity of the acetabulum and femoral head of both the DDH and FAI groups was found to be less than that for normal hi

Stribeck Curve Analysis of Temporomandibular Joint Condylar Cartilage and Disc.

Temporomandibular joint (TMJ) diseases such as osteoarthritis and disc displacement have no permanent treatment options, but lubrication therapies, used in other joints, could be an effective alternative. However, the healthy TMJ contains fibrocartilage, not hyaline cartilage as is found in other joints. As such, the effect of lubrication therapies in the TMJ is unknown. Additionally, only a few studies have characterized the friction coefficient of the healthy TMJ. Like other cartilaginous tissues, the mandibular condyles and discs are subject to changes in friction coefficient due to fluid p…

Running Economy: Neuromuscular and Joint-Stiffness Contributions in Trained Runners.

CONTEXT: It is debated whether running biomechanics make good predictors of running economy, with little known about the neuromuscular and joint-stiffness contributions to economical running gait. PURPOSE: To understand the relationship between certain neuromuscular and spatiotemporal biomechanical factors associated with running economy. METHODS: Thirty trained runners performed a 6-min constant-speed running set at 3.3 m·s-1, where oxygen consumption was assessed. Overground running trials were also performed at 3.3 m·s-1 to assess kinematics, kinetics, and muscle activity. Spatiotemporal

Moderate Associations of Muscle Elasticity of the Hamstring with Hip Joint Flexibility.

The main purpose of the present study was to identify whether and to what extent the individual differences in range of motion and stiffness of the hip joint can account for that in muscle elasticity of the hamstring. Hip extension torque and shear moduli (a measure of elasticity) of the biceps femoris, semitendinosus, and semimembranosus were assessed in 21 young males during unilateral passive hip flexion in the knee-extended position from the anatomical position to the individual's maximal hip flexion angle. Muscle shear modulus was quantified by using ultrasound shear wave elastography. Th

Characterizing adolescents with heavy menstrual bleeding and generalized joint hypermobility.

Patients with generalized joint hypermobility (JHM) may experience excessive bruising/bleeding, with heavy menstrual bleeding (HMB) commonly reported. We performed a retrospective review of 30 adolescents seen in a Young Women's Hematology Clinic with both HMB and JHM. We found that (1) a significant delay (mean 36 months, range 5-72) occurred between menarche and referral to specialty care, (2) HMB had moderate to severe impact on school and physical activities in 60% of patients, and (3) most patients (68%) required escalation of their initial therapy. We suggest providers consider JHM as a

Instability of the proximal radioulnar joint in Monteggia fractures-an experimental study.

BACKGROUND: A Monteggia fracture is defined as a fracture of the proximal ulna combined with a luxation of the radial head. The aim of the present work is to evaluate the extent of instability of the radius head in the proximal radioulnar joint (PRUJ) as a function of the severity of elbow fracture and ligamentous injury in an experimental biomechanical approach. METHODS: Eight fresh-frozen cadaver arms were used. All soft tissues were removed except for the ligamentous structures of the PRUJ and forearm. A tensile force of 40 N was exerted laterally, anteriorly or posteriorly onto the proxim

Total joint replacement in sub-Saharan Africa: a systematic review.

Outcomes of arthroplasty in sub-Saharan Africa are not widely reported. To our knowledge, this systematic review is the first to explore this topic. Scopus, EMBASE, Medline and PubMed databases were searched, utilising MeSH headings and Boolean search strategies. All papers from South Africa were excluded. Twelve papers reporting 606 total hip replacements (THRs) and 763 total knee replacements (TKRs) were included. Avascular necrosis was the most common indication for THR, whereas osteoarthritis was the main indication for TKR. HIV prevalence of up to 33% was seen. Improvements were seen in p

Optimal Timing of Glucose Measurements After Total Joint Arthroplasty.

BACKGROUND: Postoperative glucose levels after total joint arthroplasty are important to monitor as hyperglycemia has been linked to complications such as periprosthetic joint infection. The purposes of this study were to identify how postoperative glucose values vary during the perioperative period and determine the optimal time to check glucose levels to best evaluate for hyperglycemia. METHODS: A retrospective study was conducted from September 2017 to September 2018 on 314 patients who underwent knee and hip arthroplasties. Blood glucose levels were collected immediately preoperatively, i

Dual prophylaxis with teicoplanin and cefazolin in the prevention of prosthetic joint infection.

INTRODUCTION: There is a growing increase in prosthetic joint infection (PJI) incidence due to cephalosporin-resistant bacteria, used in surgical prophylaxis. The replacement of these with glycopeptides has not been shown to improve the results, but they have been shown to improve with their combination. METHODS: Comparative study of combination of teicoplanin and cefazolin before arthroplasty surgery against cefazolin alone from a previous control group. RESULTS: During the control period, there were 16 PJIs from 585 surgeries, while in the intervention group there were 6 from 579 (incidenc

[Present situation and prospect of diagnosis and treatment of periprosthetic joint infections].

Periprosthetic joint infection (PJI) as an insurmountable challenge has always represented a serious concern for orthopedic surgeons. Accurate diagnosis and reasonable treatment of PJI are the keys to deal with this challenge. The diagnosis of PJI is mainly based on clinical manifestations, joint fluid biochemistry, pathogen culture and biomarkers at present. New pathogen culture methods and specific biomarkers in recent years are hot topics in PJI research. In terms of treatment, two-stage revision is still considered to be the 'gold standard' protocol. However, more and more centers around th
