We report 4 cases of ischemic brain and spinal cord injury after shoulder surgery in the beach chair position, using data from medical legal case reviews. We argue that the correlation between cardiovascular risk factors and cerebral ischemic complications for this type of surgery is poor in these middle-aged patients. Rather, our analysis suggests that the sitting position and the head position create specific physiological conditions that may be conducive to cerebral and spinal cord ischemia during this type of surgery. Madelung's disease, also known as benign symmetrical lipomatosis, is a rare proliferative disorder of unknown etiology that was first mentioned by Brodie in 1846. Characterized by multiple symmetrical deposits of unencapsulated fat in the head and neck region, the disease is most common in middle-aged men with a history of alcohol abuse. The only effective therapy in cases of dyspnea and dysphagia, indicating the necessity of treatment, is the surgical resection of the adipose tissue. The authors report on the evaluation and therapy in a case of giant Madelung's disease.

Cellular angiofibroma (CAF) is a recently described rare soft tissue neoplasm of the vulva (with only four reported cases) that typically occurs as a well-circumscribed solid rubbery vulvar mass in middle-aged women. Large coronary fistulas are considered to cause myocardial ischemia due to diversion of the coronary blood flow. In this case the authors report the reverse effect--the spontaneous closure of a large fistulation between the left circumflex artery and the coronary sinus evoked angina pectoris in a middle aged man, who died several years later. Postmortem examination revealed a coronary aneurysm that had ruptured and dissected into the coronary sinus and finally thrombosed. The origin of the aneurysm could be congenital but perhaps also represents a healed stage of Kawasaki disease.

We examined the operated 53 cases with delayed paraparesis after osteoporotic vertebral fracture. This article reviews the management of degenerative lumbosacral stenosis. Degenerative lumbosacral stenosis occurs when soft tissue and bony changes, possibly in conjunction with abnormal motion of the lumbosacral joint, impinge on the nerve roots or vasculature of the cauda equina. A 14-year-old girl presented with a rare symptomatic Rathke's cleft cyst manifesting as diabetes insipidus and growth retardation. Neuroimaging demonstrated the suprasellar cyst. Spontaneous internal carotid dissection typically occurs in young or middle-aged patients. It is associated with a high rate of recovery and low mortality. Anticoagulant and antiplatelet drugs fail to demonstrate a significant improvement when compared with placebo. Seven patients with arthritis due to infection with human parvovirus B19 are reported, and the literature association reviewed. Twenty-three cases of achalasia of the cardia admitted to Tikur Anbessa Hospital, Addis Abeba, during a ten-year period, 1976-85, were analyzed for clinical features, means of diagnosis, treatment and results. Fractures of the facial skeleton are infrequent in children. A series of 147 fractures in 116 children aged from 10 days to 13 years old was retrospectively reviewed. The male to female ratio was 1.4:1 and the highest incidence occurred in the age group 8 to 10 years. The main aetiological factor was road traffic accidents. The mandible was predominantly involved (83.7%). The incidence of fractures of the middle third of the face was very low (4.0%). Closed reduction was undertaken most frequently. Complications were found to be minimal.

Intertrochanteric osteotomies (Pauwels) were performed in 203 hips. Their average age at operation was 41 years (range 15 to 66 years). Average follow-up was 16 years and 5 months. 78 of 203 hips had been undergone varus osteotomies (P1) and valgus osteotomies (P2) were in 125 hips. All the hips were evaluated clinically and radiologically. 140 hips (69%) had no pain. 132 hips (65%) showed no arthritic changes. In summary, we feel that Pauwels' osteotomy is a reliable method in the treatment of the young and middle aged patient.
