Post-Tuberculosis Lung Disease: The Role of Surgical Management.

Post-tuberculosis lung disease (PTLD) is a frequent and underrecognized cause of chronic respiratory morbidity, characterized by heterogeneous structural and functional sequelae following microbiological cure of tuberculosis. Although most patients are managed with medical therapy, rehabilitation, and bronchoscopic interventions, a selected subset develops irreversible disease requiring surgical treatment. This article focuses on PTLD, emphasizing the role of surgical management within multidisciplinary care, and discusses the underlying pathophysiology, clinical phenotypes, diagnostic approac

Reactivation of tuberculosis in patient treated with the antifibrotic drug nintedanib.

The novel antifibrotic drugs for idiopathic pulmonary fibrosis give us an opportunity for change the course of the disease. Their usages are expected to rise with the widening of their indications. We report a case of a patient who complains of fatigue breathlessness on exertion, dry cough, and fever up to 38°C while on the antifibrotic drug nintedanib treatment. We proved the reactivation of latent tuberculosis (TB) by microbiology of bronchial lavage. The outpatient died from massive hemoptysis. To our knowledge, we present the first case in the world for reactivation of TB in a patient on t

[Vaccination against M. tuberculosis - what next after BCG?].

Tuberculosis (TB) still remains a huge global health problem. An increase in TB has been observed in many parts of the world, especially in poor and densely populated sub-Saharan Africa and Asia. Tuberculosis affects not only the developing countries but also the relatively wealthy regions of Europe, particularly Eastern Europe, where drug-resistant mycobacterial strains are increasingly reported. Control of tuberculosis expansion is very difficult. It requires the long-term use of anti-mycobacterial drugs. Additionally, the HIV epidemic and the phenomenon of multi-drug resistance are assumed

[The sequelae of tuberculosis].

During the 20th century, tuberculosis has been the most prevalent and most harmful disease in Japan. Enormous medical researches have ever been performed to conquer the disease. Nevertheless tuberculosis has left various somatic and psychological residues on vast convalescents. On the other hand, researches to conquer tuberculosis have made considerable contribution to other fields of medicine. 1. Somatic and psychological residues on convalescents from tuberculosis. Chest x-ray findings, cardio-pulmonary disturbance, secondary infection, serum-hepatitis due to mass transfusion during the ches

Poncet's disease: tuberculous rheumatism.

Poncet's disease is characterized by a polyarthritis that occurs during acute tuberculosis infection in which no mycobacterial involvement can be found or other known cause of polyarthritis detected. It is a different entity from tuberculous arthritis, which is usually monoarticular and is caused by direct tuberculin infection. Poncet's disease remains a diagnosis of exclusion. Thirteen cases have been reported in the English-language literature since 1974. All patients who received the Mantoux test reacted positively, and 10 of 11 patients treated with antituberculous drugs had resolution of

Diffuse pulmonary infiltrates after bone marrow transplantation: the role of open lung biopsy.

BACKGROUND: Diffuse pulmonary infiltrates is the major complication and cause of mortality after bone marrow transplantation. We analyzed the etiologies and prognostic factors in bone marrow recipients with diffuse pulmonary infiltrates and assessed the role of open lung biopsy in managing this complication. METHODS: Medical records of patients with diffuse pulmonary infiltrates after bone marrow transplantation were reviewed. Possible prognostic factors were analyzed by multivariate logistic regression. RESULTS: Sixty-eight (20%) of 341 bone marrow recipients had diffuse pulmonary infiltrat

Pulmonary amyloidosis: a case report.

Amyloidosis is a rare disease in which abnormally folded proteins form accumulations called amyloid fibrils that accumulate in different tissues and organs, sometimes leading to organ dysfunction, organ failure and death. Rarely, pulmonary involvement can take one of three forms: nodular, diffuse alveolar-septal, or tracheobronchial. We report the case of a 60-year-old woman previously treated for confirmed pleural tuberculosis. She was admitted with progressive dyspnea for three months and a chest CT scan revealed a pulmonary nodule in the middle lobe. She underwent CT-guided biopsy and diagn

Epidemiological, clinical and outcomes of pleural tuberculosis: insights from a 646-patient cohort in the province of Khémisset, Morocco.

INTRODUCTION: pleural tuberculosis, the second most common extrapulmonary manifestation after lymph node involvement, poses a considerable diagnostic and therapeutic challenge. This study examines the epidemiological, clinical, biological, and therapeutic features of pleural tuberculosis cases in Khémisset, a province in Morocco's Rabat-Sale-Kenitra region, which reports the country's second-highest tuberculosis incidence. METHODS: this retrospective study examined the medical records of patients diagnosed with pleural tuberculosis at the Khémisset Diagnostic Center for Tuberculosis and Respi

Mucoid Pseudomonas aeruginosa-derived alginate modulates innate immune transcriptional responses and promotes intracellular growth of Mycobacterium tuberculosis and non-tuberculous mycobacteria.

Pulmonary dysbiosis has been increasingly associated with tuberculosis susceptibility, relapse, and treatment failure; however, the mechanistic contribution of specific airway pathogens remains poorly understood. Pseudomonas aeruginosa, particularly its mucoid phenotype characterized by alginate overproduction, is frequently detected in chronic lung diseases and tuberculosis-associated microbiomes. Here, we investigated whether mucoid P. aeruginosa and alginate modulate innate immune responses during Mycobacterium tuberculosis and non-tuberculous mycobacteria infection. Using human type II pne

Targeting HspX of Mycobacterium tuberculosis: Advances in diagnostics, treatment, and vaccine development.

Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis (MTB), Bacille Calmette-Guérin (BCG) is the only licensed vaccine for tuberculosis, but it provides limited and inconsistent protection against pulmonary TB in adults. Furthermore, current diagnostic methods show low sensitivity in latent stages, and the standard treatment is long, complex, and conducive to the emergence of drug-resistant strains. In this context, heat shock protein X (HspX), also known as Rv2031c or α-crystallin, has emerged as a potential biomarker and vaccine candidate. This 16-kDa protein is pr

Rare presentation of intractable tuberculous panophthalmitis with intraocular and intraorbital abscesses: a case report.

BACKGROUND: We report a rare presentation of extrapulmonary tuberculosis. CASE PRESENTATION: A 29-year-old Burmese woman with human immunodeficiency virus infection and known pulmonary tuberculosis who had been treated for 5 months presented to our hospital with unilateral progressive painful visual loss of 1 month's duration. She was diagnosed with tuberculous panophthalmitis with subretinal and intraorbital abscesses, conjunctival abscess, and extraocular muscle tuberculoma. The diagnosis was confirmed by a conjunctival pus swab with a positive result for acid-fast bacilli and a positive re

Transcriptional responses in mouse lungs induced by vaccination with Mycobacterium bovis BCG and infection with Mycobacterium tuberculosis.

Transcriptome analyses enable the assessment of signature alterations in whole tissues and organs undergoing pathological processes. We analyzed gene expression profiles of lungs from mice infected with Mycobacterium tuberculosis or vaccinated with Mycobacterium bovis bacille Calmette-Guérin (BCG). We compared high-dose systemic and low-dose aerosol M. tuberculosis infections as well as systemic BCG vaccination. Expression profiles in lungs were analyzed at day (d) 1 and d 30 post infection / vaccination using a custom tailored 'in situ' synthesized 60-mer oligonucleotide microarray with focus

Enhancement of human T cell response to a peptide epitope of 38 kDa antigen of Mycobacterium tuberculosis by liposomes.

Diagnosis of tuberculosis a problem, specially in the regions harboring an abundance of both pathogenic and non-pathogenic mycobacteria. This study was undertaken to assess in such a situation the predictive value of proliferative T cell response to a peptide epitope ('38G') of the 38 kDa membrane protein of Mycobacterium tuberculosis. 3[H]-thymidine incorporation assays were done with peripheral blood mononuclear cells of tuberculoid leprosy and pulmonary tuberculosis patients. The donors were also classified as PPD responders (Stimulation Index, SI> 3) or non-responders (SI < or = 3) on the
