Stroke patterns of internal carotid artery dissection in 40 patients.

BACKGROUND AND PURPOSE: Internal carotid artery dissection (ICAD) is a frequent cause of ischemic stroke in young patients. Whether cerebral ischemia is of embolic or hemodynamic origin remains to be determined. Heparin is often administered in ICAD; however, a drug trial can hardly be conducted because of the low recurrence rate after the acute stage. Therefore, the best therapeutic approach should be determined on the basis of the presumed mechanism of cerebral ischemia. One way to approach the mechanism of stroke in ICAD is to determine stroke patterns. We postulated that most cortical and

Flow reversal for cerebral protection in carotid artery stenting: a review.

Carotid artery stenting has established itself as a valid treatment option for carotid stenosis. Many neuroprotective devices have been developed to minimize the risk of embolic events and stroke. Of the devices available today, flow reversal is unique in its conceptual similarity to carotid endarterectomy shunting techniques that maintain cerebral flow. This review focuses on the technical aspects, results, advantages, and disadvantages of carotid flow reversal for embolic protection during carotid artery stenting.

Should we abandon the common practice of withholding oral anticoagulation in paroxysmal atrial fibrillation?

AIMS: To assess the relation between the atrial fibrillation (AF) subtype and thrombo-embolic events. METHODS AND RESULTS: The observational Euro Heart Survey on AF (2003-04) enrolled 1509 paroxysmal, 1109 persistent, and 1515 permanent AF patients, according to the 2001 American College of Cardiology, American Heart Association, and the European Society of Cardiology guidelines definitions. A 1 year follow-up was performed. Permanent AF patients had at baseline a worse stroke risk profile than paroxysmal and persistent AF patients. In paroxysmal AF, the risk for stroke, any thrombo-embolism,

Late cerebral embolization after emboli-protected carotid artery stenting.

Carotid artery stenting (CAS) is nowadays considered as alternative therapeutic option to carotid endarterectomy for patients suffering from carotid artery disease. Recent studies and meta-analyses have demonstrated equal performance of carotid stenting to endarterectomy as regard as the overall adverse events (death/stroke rates), especially when periprocedural myocardial infarction and nerve pulses are also included. However, carotid stenting was inferior to endarterectomy when compared in terms of acute and late embolic events. In the present review, we collect all studies available in the

[Clinical imaging correlations in lacunar infarcts].

PURPOSE: This paper is a study concerning the clinical-imagistic correlations in lacunar infarcts causes and associated lesions in these ones. We made an attempt to find out and emphasize the risk factors, causes and pathogenic mechanisms involved in lacunar syndromes. MATERIALS AND METHODS: We studied 138 patients with lacunar infarcts diagnosed by clinical examination and computer tomography. The investigation of risk factors evidenced arterial hypertension in 75 cases (54%). The clinical features of lacunar syndromes were: 1) pure motor stroke 41c; 2) pure sensory stroke 8c; 3) ataxic hemi

Cerebral Embolic Protection in Transcatheter Aortic Valve Implantation Using the Sentinel Cerebral Protection System: A Systematic Review and Meta-Analysis.

INTRODUCTION: Transcatheter aortic valve implantation (TAVI) plays a vital role in patients with symptomatic aortic stenosis. Despite the mortality benefit of TAVI, embolic stroke remains a feared complication. As a result, transcatheter cerebral embolic protection (TCEP) devices have been developed to reduce this risk. Given the ongoing debate of TCEP in TAVI, we performed a systematic review and meta-analysis of all randomized controlled trials to date to identify outcomes of periprocedural stroke using the Sentinel™ cerebral protection system (CPS). METHODS: MEDLINE, Cochrane, and Scopus d…

Patent foramen ovale complicated by paradoxical embolism and brain infarct in a patient with advanced ovarian cancer.

BACKGROUND: Recent investigations of patients with cerebral and peripheral arterial emboli of unknown cause suggest that paradoxical embolis … doxical embolism should be considered in the differential diagnosis of ovarian cancer patients with embolic stroke and it may be appropriate to include a cardiac echo as part of the diagnostic evaluation.

The value of diagnostic procedures in treating ischemic attacks and stroke.

Diagnostic procedures for cerebral ischemia make clear strategies mandatory. Careful recording of case history, internal and neurological investigation either substantiate or make unlikely TIA and stroke. CT scan not only differentiates bleedings, tumors etc from ischemia but also can give very valuable information about the underlying vascular pathology. Multiple lacunes are characteristic of cerebral microangiopathy. Territorial and branch occlusion infarcts mostly indicate embolic occlusion of pial vessels from either cardiac or arterial sources. Endzone and borderline infarctions are seen

Intravenous thrombolysis is unsafe in stroke due to infective endocarditis.

Embolic stroke is the most common neurological complication of infective endocarditis and a major source of morbidity and mortality. Septic embolism is considered a contraindication to intravenous thrombolysis in patients with ischaemic stroke because of concerns over an increased risk of intracranial haemorrhage. We describe a patient with occult endocarditis who was treated with thrombolysis for acute stroke and review other cases reported in the literature.

Mitral annular calcifications and aortic plaques as predictors of increased cardiovascular mortality.

OBJECTIVES: Mitral annular calcifications (MAC) and aortic plaques are predictive for higher incidence of recurrent embolic events, the presence and severity of coronary artery disease and total cardiac deaths. However, no data exist comparing the impact of combined presence of both aortic plaques and MAC on embolic events, deaths associated with cerebral embolism or cardiac deaths during the follow-up. PATIENTS AND METHODS: In 100 consecutive patients with aortic plaques detected by transesophageal echocardiography (mean age 69.8 + 8.9 years, range 39-93 years, 49 males, 51 females) and in 4

Patent Foramen Ovale (PFO) Closure in Thrombophilia as Primary Prevention in Patients Without Prior PFO-Associated Thromboembolic Events.

Patent foramen ovale (PFO) is a common anatomical variant with potential clinical implications in embolic phenomena. In some situations, transcatheter closure is considered a treatment option. For patients with thrombophilia who have not had embolic events, it is not always clear whether this approach is appropriate. Individuals with clotting disorders may face different considerations, and approaches can vary between cases. This study was conducted based on the PICO-formatted research question: Population - thrombophilic patients with no history of embolic events; Intervention - PFO closure;

[Recurrent cerebral embolism due to the disseminated carcinomatosis of bone marrow with early gastric cancer].

A 67-year-old woman who had undergone laparoscopic proximal gastrectomy for early gastric cancer 10 months previously was admitted to our hospital due to dysarthria. Brain MRI demonstrated acute multiple small infarcts in the right middle cerebral artery (MCA) and the right posterior inferior cerebellar artery (PICA) territory, and she was diagnosed as embolic stroke. Anticoagulant therapy did not prevent further ischemic stroke. No embolic sources were detected by MR angiography, carotid duplex sonography, transthoracic and transesophageal echocardiography, and Holter electrocardiography. We …

Transnasal transesophageal echocardiography in the detection of left atrial thrombus.

BACKGROUND: The widespread use of transesophageal echocardiography (TEE) is limited by disadvantages, including patient intolerance and increased medical costs. We aimed to investigate the feasibility and safety of transnasal TEE in the detection of possible embolic sources in patients with atrial fibrillation (AF) and/or stroke, using an ultrathin TEE probe. METHODS: Sixty-two patients with AF and/or stroke underwent transnasal TEE without conscious sedation. The presence or the absence of the following parameters was evaluated: left atrial (LA) thrombus; LA spontaneous echocardiographic con

Outcomes Following Patent Foramen Ovale Percutaneous Closure According to the Delay From Last Ischemic Event.

BACKGROUND: Randomised controlled trials evaluating percutaneous closure of patent foramen ovale (PFO) have included only patients with a recent embolic event. We aimed to evaluate outcomes after percutaneous PFO closure according to the delay from the last embolic episode. METHODS: This international ambispective cohort included consecutive patients from 2 centres in France and Canada undergoing PFO closure for secondary prevention of a paradoxical embolic event. The primary end point was the composite of stroke or transient ischemic attack (TIA). A logistic regression model was used to eval

Broad ischemic stroke is mainly due to a cardiac embolus or to an atheromatous plaque. In young subjects, one of the main causes of ischemic stroke (broad ischemic stroke in particolar) is embolic heart disease including infective endocarditis. Infective endocarditis is a contraindication against the anticoagulant therapy (which is indicated for the treatment of embolic heart disease complicated by ischemic stroke). One neurologic complications of infective endocarditis is ischemic stroke which often occurs in multiple sites. We here report the case of a 44-year old man with afebrile acute ons
