Pseudo-obstruction syndromes.

Chronic intestinal pseudo-obstruction (CIP) is a clinical syndrome characterized by symptoms and signs of intestinal occlusion, in absence of any mechanical obstruction of the gut lumen. It causes impaired transit of intestinal contents and is determined by abnormalities of motor activity. The term CIP is used to indicate a heterogeneous group of disorders with many different pathogenic mechanisms. The defect in the regulation of intestinal transit can be at any level of motility control. Two main types of CIP are recognized, termed respectively myogenic (when smooth muscle cells are affected)

An artificial intelligence platform provides an accurate interpretation of esophageal motility from Functional Lumen Imaging Probe Panometry studies.

BACKGROUND: Functional lumen imaging probe (FLIP) Panometry is performed at the time of sedated endoscopy and evaluates esophageal motility in response to distension. This study aimed to develop and test an automated artificial intelligence (AI) platform that could interpret FLIP Panometry studies. METHODS: The study cohort included 678 consecutive patients and 35 asymptomatic controls that completed FLIP Panometry during endoscopy and high-resolution manometry (HRM). "True" study labels for model training and testing were assigned by experienced esophagologists per a hierarchical classificat

[Colonic motility].

Colonic motility is provided by contraction of intramural smooth muscle under the control of the enteric and the extrinsic nervous system and humoral connections. In vivo measurement of colonic motility remains difficult because of the complexity of these interactions and anatomical considerations. The possibility that symptoms are due to colonic dysmotility should be considered in patients with normal barium enema and colonoscopy. Examples of this are patients with chronic constipation, irritable bowel syndrome or colonic symptoms associated with diabetes mellitus or diseases of the nervous s

Colonic capacitance and transit in man: modulation by luminal contents and drugs.

We have examined whether the human colon is able to adapt its capacity to receive fluid, specifically, whether the caecum and ascending colon ('right colon') exhibited any reservoir function. Healthy volunteers had the caecum intubated from above with a device that permitted intraluminal pressures to be recorded while fluids were instilled into the caecum. Perfusates contained 99mTc-DTPA (diethylenetriamine pentaacetic acid) and volumes of the right colon were determined by two plane scintigraphy using gamma cameras. Different individuals received caecal infusions of 154 mM NaC1 (saline), an e

Jiawei YigongSan improves gastric motility and is associated with modulation of PI3K/Akt/mTOR signaling and autophagy in a rat model of functional dyspepsia.

ETHNOPHARMACOLOGICAL RELEVANCE: Jiawei YigongSan (JWYGS), a modified traditional Chinese medicine formula derived from Yigong San, is traditionally used to tonify Qi, strengthen the spleen, and regulate gastrointestinal function. However, its potential mechanisms in functional dyspepsia (FD) remain insufficiently clarified. AIM OF THE STUDY: This study aimed to investigate the effects of JWYGS on gastric motility dysfunction in a rat model of FD and to explore its potential association with interstitial cells of Cajal (ICCs), autophagy-related protein regulation, and the PI3K/Akt/mTOR signali

Long-term functional assessment of antegrade colonic enema for combined incontinence and constipation using a modified Marsh and Kiff technique.

PURPOSE: Constipation and fecal incontinence can severely affect quality of life for patients, particularly when simultaneously present. Malone antegrade colonic enema enables periodic colonic emptying, thus preventing uncontrolled passage of feces and constipation. METHODS: Eleven patients with fecal incontinence and severe constipation or perineal colostomy after Miles' operation underwent a modified Marsh and Kiff ileostomy for antegrade colonic enema. Before and after surgery, the patients were fully evaluated for gastrointestinal functions, including gallbladder and stomach emptying time

Radiopaque markers to evaluate gastric emptying and small intestinal transit time in healthy cats.

Determinations of gastric emptying time (GET) and small intestinal transit time (SITT) are useful in detecting gastrointestinal motility disorders and partial obstructions of the pylorus or small intestine. Barium-impregnated, polyethylene radiopaque spheres with diameters of 1.5 mm and 5.0 mm have been developed for quantitative assessment of gastrointestinal transit. The purpose of this study was to evaluate GET and SITT using these radiopaque spheres in 10 healthy cats. The cats were randomly assigned to 1 of 3 treatment groups: fasted, fed, and fed plus sedation (acetylpromazine maleate 0.…

Glucagon-Like Peptide-1 Receptor Agonist Use Is Associated With and May Lead to Esophageal Motility Abnormalities.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are approved for the treatment of obesity and type 2 diabetes mellitus (T2D). GLP1-RA use has grown exponentially, with a recent survey reporting 1 in 8 adults (12%) in the United States have ever taken a GLP-1RA.1 GLP-1RAs are known to cause a variety of effects on gastrointestinal motility, including increased gastric accommodation, delayed gastric motility, and decreased small intestinal transit. However, little is known regarding the direct effects of GLP-1RAs on esophageal function beyond reported increased risk of gastroesophageal refl

A double-blind fluoroscopic study of cisapride on gastrointestinal motility in patients with functional dyspepsia.

Twenty patients with functional dyspepsia were referred for radiologic examination and, upon confirmation of a hypomotile stomach, were given either 10 mg cisapride or placebo in a double-blind manner (10 patients per group). The movement of a 250-ml barium meal was assessed by means of television fluoroscopy performed at regular time intervals. Cisapride significantly improved antral contractility and enhanced gastric emptying compared with placebo. Deep peristaltic waves occurred over the entire small bowel, and motility and small-bowel transit time of the barium meal were significantly incr

[Gastrointestinal dysfunction in idiopathic Parkinson's disease].

Gastrointestinal dysfunction is frequent during all stages of Parkinson's disease. The entire gastrointestinal tract becomes involved and symptoms include sialorrhea, dysphagia (difficulties swallowing), delayed gastric emptying, absorption problems and constipation. These non-motor symptoms can be manifested even prior to the initial Parkinson diagnosis, i.e. during the so-called premotor phase of the disorder and may serve as prodromal markers of the early non-motor disease phase. In addition to causing patients major discomfort and a reduced quality of life, such gastrointestinal complaints

Hemizygosity Enables a Mutational Transition Governing Fungal Virulence and Commensalism.

Candida albicans is a commensal fungus of human gastrointestinal and reproductive tracts, but also causes life-threatening systemic infections. The balance between colonization and pathogenesis is associated with phenotypic plasticity, with alternative cell states producing different outcomes in a mammalian host. Here, we reveal that gene dosage of a master transcription factor regulates cell differentiation in diploid C. albicans cells, as EFG1 hemizygous cells undergo a phenotypic transition inaccessible to "wild-type" cells with two functional EFG1 alleles. Notably, clinical isolates are of

The Role of In Vitro Model Complexity in Lipid-Based Formulation Screening: A Case Study with Ticagrelor.

Lipid-based formulations (LBFs) offer a promising strategy to enhance the oral bioavailability of poorly water-soluble drugs, such as biopharmaceutical classification system (BCS) Class IV compounds. However, their in vitro characterization remains challenging because of the complex interplay between dispersion, dissolution, lipid digestion, and absorption. In this study, several LBFs of ticagrelor were investigated using a progressive set of in vitro models of increasing physiological complexity, including dissolution testing in biorelevant media using USP II and USP IV apparatuses, intestina

Lipid Nanocapsule-Chitosan and Iota-Carrageenan Hydrogel Composite for Sustained Hydrophobic Drug Delivery.

Hydrophobic drug delivery via oral or pulmonary routes presents significant challenges for clinical translation, particularly for poorly soluble antiviral drugs. Physiological barriers-such as enzymatic degradation, harsh pH, and rapid transit in the gastrointestinal tract, or mucociliary clearance and alveolar macrophage uptake in the lungs-can severely limit therapeutic efficacy. To address these challenges, we developed a novel lipid nanocapsule (LNC) and chitosan/iota-carrageenan hydrogel composite tailored for sustained delivery of hydrophobic antiviral agents. This composite system was d

Microencapsulation of Salmonella-Specific Bacteriophage Felix O1 Using Spray-Drying in a pH-Responsive Formulation and Direct Compression Tableting of Powders into a Solid Oral Dosage Form.

The treatment of enteric bacterial infections using oral bacteriophage therapy can be challenging since the harsh acidic stomach environment renders phages inactive during transit through the gastrointestinal tract. Solid oral dosage forms allowing site-specific gastrointestinal delivery of high doses of phages, e.g., using a pH or enzymatic trigger, would be a game changer for the nascent industry trying to demonstrate the efficacy of phages, including engineered phages for gut microbiome modulation in expensive clinical trials. Spray-drying is a scalable, low-cost process for producing pharm
