Late surgical problems in children born with abdominal wall defects.

During a 17 year period 55 patients with abdominal wall defects were treated. A questionnaire concerning late surgical problems was distributed to the parents of the 47 surviving children and 44 (94%) answers were received. The mean follow up time was 5.4 years. There was no mention of remaining problems regarding 16 of the 28 omphalocoele patients and 10 of the 16 gastroschisis patients. Postoperative abdominal wall hernia was reported in 7 cases with omphalocoele and in 6 with gastroschisis; postoperative intestinal stoma occurred in 1 child with omphalocoele associated with anal atresia, an

Molecular signature of active fibrogenesis prevails in biliary atresia after successful portoenterostomy.

BACKGROUND: In biliary atresia mechanisms of progressive liver injury leading to need of liver transplantation after successful portoenterostomy remain unknown. A better understanding is a prerequisite for development of novel therapies to extend native liver survival, and we aimed to unravel molecular characteristics of liver injury after successful portoenterostomy. METHODS: Liver biopsies obtained from 28 biliary atresia children during successful portoenterostomy and at median age 3.0 years were studied. Biopsies were analyzed for histology and immunohistochemical expression of collagen 1

Pyloric atresia: 15-year review from a single institution.

BACKGROUND: Pyloric atresia (PA) is a rare pathology. Calder presented the first pyloric atresia case in 1749 and Touroff, Sussman, Meltz, and their colleagues presented the first successful operation in 1940. PA has 3 types of anatomic variations: (1) type A, pyloric membrane or web; (2) type B, the pyloric channel is a solid cord; and (3) type C, in which there is a gap between the stomach and duodenum. Associated anomalies also have been described. Epidermolysis bullosa (EB) and intestinal anomalies occur most often with this condition. METHODS: Charts of 16 cases of congenital PA, aged 1

Evaluation of sodium deficit in infants undergoing intestinal surgery.

BACKGROUND: Sodium is a critical growth factor for children. Severe deficits cause growth impairment and cognitive dysfunction. Both the dia … ge of 32.2 weeks and weight of 1.43 kg were identified. The most common diagnoses were NEC (38.5%), intestinal atresia (20.5%), and isolated perforation (10.3%). Sodium deficiency was documented in 36/39 (92%) and 92.9% for those in continuity. Severe deficiency occurred in 64%. Urine sodium was significantly correlated with weight gain (p=0.002). Weight gain in pat …

[Acquired diaphragmatic hernia in a child with a related liver transplant].

Acquired diaphragmatic hernia in children is rare. Even less often, this disease occurs after liver transplantation for biliary atresia. In our case, diaphragmatic hernia was acquired, since the patient underwent repeated chest X-ray examinations including CT prior to liver transplantation. There were no signs of hernia. Clinical manifestations of diaphragmatic hernia were absent throughout 9 months after liver transplantation and manifested acutely with combination of symptoms of respiratory failure and intestinal obstruction. Surgical treatment was performed after emergency consultation with

Duodenal atresia: a comparison of techniques of repair.

Although duodenal atresia is a common cause of congenital intestinal obstruction, the optimal technique of repair remains controversial. In a 10-year period, 41 newborns (20 male, 25 premature) underwent either side-to-side duodenoduodenostomy (SDD, 10 infants), side-to-side duodenojejunostomy (SDJ, 9 infants), or diamond-shaped duodenoduodenostomy (DDD, 22 infants) in a nonrandomized series. One-layer anastomosis was used in each case, and gastrostomy was placed in most patients. Groups were compared for survival, prematurity, associated anomalies, time until feeding onset, total hospitalizat

New perspectives on biliary atresia.

An investigation into the aetiology, diagnosis, and treatment of biliary atresia was carried out because the prognosis remains so poor.In an electron microscopical study no viral particles or viral inclusion bodies were seen, nor were any specific ultrastructural features observed. An animal experiment suggested that obstruction within the biliary tract of newborn rabbits could be produced by maternal intravenous injection of the bile acid lithocholic acid.A simple and atraumatic method of diagnosis was developed using(99) (m)Tc-labelled compounds which are excreted into bile. Two compounds, (

Fetal megacystis: differential diagnosis.

The purpose of our retrospective observational series was to determine whether the sonographic characteristics of fetal megacystic bladders can be used to reliably establish the most likely diagnosis in fetuses with this condition. The sonographic records of pregnant patients referred to our institutions over a 10-year period who were found on initial 2-dimensional sonography to be carrying fetuses with megacystis were examined for evidence of a keyhole sign, bladder thickness, amniotic fluid index, and fetal sex. When available, 3-/4-dimensional sonography, Doppler angiography, tomographic ul

Factors Conducive to Catch-Up Growth in Postoperative Jejunoileal Atresia Patients as Prognostic Markers of Outcome.

INTRODUCTION: Jejunoileal atresia (JIA) is a major congenital anomaly that requires surgical intervention in the neonatal period. During follow-up after surgery, there is usually a period of catch-up growth (CUG) that is sufficient for patients to regain normal weight for age. However, in some cases, CUG is inadequate. The aim of this study was to assess postoperative JIA patients to determine factors that may be associated with good CUG. MATERIAL AND METHODS: We retrospectively reviewed JIA patients treated at our institution by classifying them into three groups based on a comparison of pos

Postobstructive enteropathy in infants with transient enterostomy: its consequences on the upper small intestinal functions.

Repeated or prolonged organic obstruction of the small intestine in the neonatal period can lead to severe refeeding problems, despite a transient ostomy. These problems are thought to result from a postobstructive enteropathy (POE) of the apparently normal small intestine segment above the obstruction. Ten infants with a POE, characterized by limited oral caloric and carbohydrate intakes and increased ostomy effluent, were compared with 8 controls with an enterostomy and a normal postoperative refeeding pattern. There was no statistical difference in the histomorphometric appearance of the mu

Tapering enteroplasty versus primary anastomosis without tapering in neonatal small bowel atresia: a systematic review and meta-analysis.

PURPOSE: Small bowel atresia (SBA) affects approximately 1 in 1,500-4,500 neonates and represents one of the most common congenital causes of intestinal obstruction. Management of the characteristically dilated proximal bowel segment, whether by tapering enteroplasty (TAP) or primary anastomosis without caliber reduction (non-TAP), has remained controversial for five decades in the absence of randomized evidence. We conducted a systematic review and meta-analysis to systematically synthesize comparative evidence on postoperative outcomes following TAP versus non-TAP in neonatal SBA. METHODS:

Congenital hernia of the cord.

Several embryopathies involve umbilicus including midgut herniation, omphaloceles, urachal and vascular anomalies. Although described earlier, hernia into umbilical cord has not found enough mention in the literature. Poor understanding of its clinical characteristics has made to miscategorization of this entity as "omphalocele minor" by many. Some of these undergo spontaneous reduction or epithelialisation if left alone. The present study describes a series of four consecutive cases of this entity and describe their clinical characteristics, associated GI anomalies and possible embryogenesis.

[Vitamin B1 deficiency as cause of a severe lactic acidosis and ileus symptoms].

A 2-year-old boy presented with severe lactic acidosis and ileus symptoms after 3 weeks of parenteral nutrition. Diagnosis of thiamine deficiency was made only when further complications had occurred. This case shows the importance of sufficient substitution of thiamine during parenteral nutrition. In lactic acidosis of unknown etiology thiamine deficiency must be considered.
