A Computational Framework for Modelling Subject Specific Changes in Knee Ligaments and Tibiofemoral Contact Associated With Varus Deformity.

Varus deformity is a pathological knee condition described by outward bowing of the joint and increased medial compartment loading, both of which contribute to the development and progression of knee osteoarthritis. This study presents a subject-specific musculoskeletal modelling framework to investigate ligamentous characteristics and tibiofemoral contact mechanics for subjects having a varus deformity. A cohort of osteoarthritic subjects with varus deformity and undergoing total knee replacement surgery was analyzed in a standing posture. For each subject, musculoskeletal models with subject

Effects of thickness and Young's modulus of lateral ankle ligaments on ankle joint stability: a finite element study.

INTRODUCTION: Ankle sprains are common sports-related injuries, and recurrent sprains may lead to chronic ankle instability (CAI). The mechanical and geometric properties of lateral ankle ligaments are crucial for joint stability. This study investigated how variations in Young’s modulus and thickness of the lateral ankle ligaments influence ankle stability. METHODS: A finite element model of the ankle was developed, in which the thickness and Young’s modulus of the anterior talofibular ligament (ATFL), calcaneofibular ligament (CFL), and posterior talofibular ligament (PTFL) were individually…

Black phosphorus-doped silk fibroin coating improves osteogenesis for ameliorative graft-bone healing of polyethylene terephthalate artificial ligaments.

This study aimed to investigate the effect of black phosphorus-doped silk fibroin (BP/SF) coating on graft-bone healing of artificial ligaments made from polyethylene terephthalate (PET). A BP/SF-coated PET artificial ligament (BP/SF-PET) was prepared, and its surface characteristics were examined using transmission electron microscopy, scanning electron microscopy, Fourier transform infrared spectroscopy, and X-ray diffraction. The cytocompatibility and osteogenic induction ability of the BP/SF-PET ligaments were evaluated in vitro. In addition, uncoated PET (PET group, n = 15) and BP/SF-coat…

Development and validation of a novel approach for quantifying dimensions of the lateral ligaments in human ankle dissections.

The lateral ankle ligaments, composed of the anterior talofibular (ATFL), calcaneofibular (CFL), and posterior talofibular ligaments (PTFL) are frequently subject to injury. While conservative and surgical treatment methods have had some positive outcomes, high rates of re-injury, chronic ankle instability, and pain remain, prompting the investigation of tissue-engineered applications in the treatment of lateral ligament injuries. In order for tissue-engineered construct design to be undertaken, a complete understanding of the native anatomy of the lateral ankle ligaments must be obtained. To …

Advancing cervical spine finite element analysis: validation of foraminal ligaments and a nerve root submodel under helmet loads.

Neck pain is a highly prevalent condition in the general population, but the addition of helmet loads contributes to the neck pain that 97% of fighter pilots report. The mechanism for neck pain in pilots is unknown; however, there is likely nerve root (NR) dysfunction involved. This study implemented a spinal cord, NRs, and novel foraminal ligaments into the previously validated VIVA OpenHBM. After performing sensitivity analyses and validating added tissues, helmet loads and muscle forces were applied. The results indicate that helmet loads generate compression on NRs that temporarily impair …

Ultrasound of Postoperative Ankle Instability: How to Manage with Arthroscopic Reparation or Anatomical Reconstruction of Lateral Ankle Ligaments.

Surgical repair or reconstruction of lateral ankle ligaments is indicated in patients with persistent ankle instability, with arthroscopic/endoscopic treatment becoming more frequent. Although presurgical imaging of the ankle is always standard procedure, more is needed to understand the evolution of the repaired ligament or the tendinous graft to help improve length of postoperative time and functional results. This review describes the normal ultrasound aspects and possible complications of repaired/reconstructed ankle ligaments after an all-inside endoscopic repair/reconstruction of the ant…

Laparoscopic administration of bupivacaine at the uterosacral ligaments during benign laparoscopic and robotic hysterectomy: a randomized controlled trial.

BACKGROUND: Postoperative pain continues to be an undermanaged part of the surgical experience. Multimodal analgesia has been adopted in response to the opioid epidemic, but opioid prescribing practices remain high after minimally invasive hysterectomy. Novel adjuvant opioid-sparing analgesia to optimize acute postoperative pain control is crucial in preventing chronic pain and minimizing opioid usage. OBJECTIVE: This study aimed to determine the effect of direct laparoscopic uterosacral bupivacaine administration on opioid usage and postoperative pain in patients undergoing benign minimally

Conventional Resection Versus Preservation of the Nasal Dorsum and Ligaments: An Anatomic Perspective and Review of the Literature.

Preservation rhinoplasty may refer to preserving several anatomic components including: the nasal bones, upper lateral cartilages, the keystone area and/or ligaments of the nose. Preserving the osseocartilaginous framework or "dorsal preservation" minimizes or completely avoids violation of the dorsal aesthetic lines' architecture. Conventional hump reduction in open rhinoplasty disrupts these lines; however, it also provides versatility to reshape the entire dorsum. Surgical success with either technique requires a thorough understanding of the underlying nasal anatomy.

Validated Ultrasound Speckle Tracking Method for Measuring Strains of Knee Collateral Ligaments In-Situ during Varus/Valgus Loading.

Current ultrasound techniques face several challenges to measure strains when translated from large tendon to in-situ knee collateral ligament applications, despite the potential to reduce knee arthroplasty failures attributed to ligament imbalance. Therefore, we developed, optimized and validated an ultrasound speckle tracking method to assess the in-situ strains of the medial and lateral collateral ligaments. Nine cadaveric legs with total knee implants were submitted to varus/valgus loading and divided into two groups: "optimization" and "validation". Reference strains were measured using d…

How Close Are the Volar Wrist Ligaments to the Distal Edge of the Pronator Quadratus? An Anatomical Study.

Background: This cadaveric study defines the interval distance between the proximal insertion of the volar wrist ligaments and the distal edge of the pronator quadratus on the distal radius. It is important to be aware of this distance during surgical dissection for placement of volar locking plates for wrist fractures. Disruption of the volar wrist ligament insertion may have adverse biomechanical consequences such as carpal instability, which can lead to pain and eventually wrist arthritis. Methods: Thirteen cadaveric wrists were dissected using the trans-flexor carpi radialis volar approach…

Anatomic Reconstruction of the Anterior Oblique and the Dorsoradial Ligaments for Painful Subluxating Carpometacarpal Joint of the Thumb.

Thumb carpometacarpal osteoarthritis (CMC OA) is a common pathology of the hand that is characterized by pain, loss of grip and pinch strength, and deformity. Although conservative management is often preferred in earlier stage of CMC OA, surgical techniques can be used when symptoms are not fully relieved, especially with subluxation. We report a case series of 26 patients (32 operations) with Eaton stage I and II CMC OA who underwent a novel surgical technique that anatomically restores the CMC joint with autologous double ligament reconstruction. All cases were retrospectively reviewed as a

Salvage reconstruction of hip ligaments using absorbable material to treat recurrent instability of revision THA without abductor mechanism.

Dual-mobility and constrained cups can stabilise most recurrent dislocations of total hip arthroplasty (THA), but may fail in case of hip abductor mechanism loss. For such complex situations, we developed an original artificial iliofemoral and ischiofemoral ligament reconstruction technique using a polyglactin 910 mesh (Vicryl ™) associated to repositioning of a Lefèvre constrained liner adapted to hip range of motion to prevent cam effects. The technique was implemented in 2 patients showing recurrent dislocation after THA, associating total femur replacement and cemented constrained liner in
