[Restoration of sound transmission in the middle ear by reconstruction of the ossicular chain in its physiologic position. Results of incus reconstruction with ionomer cement].

We report the use of an ionomeric based bone substitute to reconstruct the ossicles in their normal position. To reconstruct small defects of the incudo-stapedial joint, this bone replacement material is applied to the long process of the incus and the head of the stapes. After setting and hardening a stable contact to the adjacent bone is formed. The audiological results after reconstruction of small incus defects with ionomeric cement are discussed in comparison to stapedectomy and tympanoplasty.

The styloid process in ossicular chain reconstruction. II: Long-term analysis.

The human cadaveric styloid process was used as a homograft material for ossicular chain reconstruction in 41 cases of chronic suppurative otitis media over a five-year period. Although technical success was achieved in about 88.23 per cent of the cases, actual patient benefit was around 70.58 per cent; this is comparable to results obtained with most other homologous graft materials. The problem of breakage while remodelling the bone has also increased the risk of transmitting the AIDS virus and consequently undermines the use of this material.

[Anatomical and functional results of middle ear reconstruction with tympano-ossicular allografts].

Twenty five patients with severely damaged middle ears underwent reconstructive surgery with using total tympano-ossicular allografts. Two years long follow-up showed average air-bone gap about 20 dB or better in 44% of patients. Materials and methods used for reconstruction in two groups (part I and II) were compared, basing on anatomical and functional results. In patients with total perforation and lack of malleus, reconstruction with the tympanic membrane allograft attached to the malleus handle and shaped incus columella revealed long-term hearing results improvement (63.6% closure of air

Electrocoagulation improving bone cement use in middle-ear surgery: short-term and middle-term results.

BACKGROUND: Bone cement is used for ossicular chain repair and revision stapes surgery. Its efficient use requires cautious removal of mucosa from the ossicles. This paper reports a technique for easy, fast and safe removal of this mucosa prior to cement application. It consists of the application of monopolar electrocoagulation on the ossicles prior to bone cement application. METHODS: The outcomes of six cases of revision stapes surgery and seven cases of partial ossiculoplasty, conducted between 2007 and 2012 using this new technique, were evaluated. Intra-operative reports and audiometric

Morphological Variations of Middle Ear Ossicles and its Clinical Implications.

INTRODUCTION: The middle ear ossicles form a semi rigid chain in the middle of the ear for conduction and amplification of sound waves from the tympanic membrane to the inner ear. Although, our knowledge of the ear ossicles dates back to the 15th century, and various studies have been carried out on their morphometry, morphology, anomalies, embryology, function and structure throughout the world, information about the morphology of middle ear ossicles is meagre in Indian subjects. AIM: To find out the morphological variations of middle ear ossicles of right and left sides. MATERIALS AND METH

Surgical treatment of otosclerosis using a unique stapes prosthesis without a hook.

BACKGROUND: During stapes surgery, the Teflon wire piston prosthesis is prone to postoperative 'slips' and subsequent necrosis and fracture of the long process of the incus. AIMS/OBJECTIVE: We invented and used a novel cup-shaped apatite prosthesis to reduce the incidence of necrosis of the long process of the incus and analysed the postoperative results. MATERIAL AND METHODS: Thirty-one ears in 25 patients with otosclerosis who underwent stapes surgery with our apatite prosthesis were evaluated. RESULTS: The air conduction improved by 24.0 dB (the average) from pre- to post-operation. Addi

[The state of the stirrup prosthesis estimated from the results of functional multispiral computed tomography].

The objective of the present study was to estimate the state of the stirrup prosthesis based on the results of functional multispiral computed tomography (fMSCT). A total of 30 patients suffering otosclerosis and treated by stapedoplasty were examined using the modified fMSCT technique developed by the authors. The data obtained were compared with the results of the audiologic study. It was shown that modified fMSCT makes it possible to objectively and visually estimate the state of the stirrup prosthesis, elucidate the causes of poor outcome of the surgical treatment and identify the indicati

[The active middle ear implant for the rehabilitation of sensorineural, mixed and conductive hearing losses].

Active middle ear implants, such as the Vibrant Soundbridge, are used as an important part in the rehabilitation of sensorineural, conductive hearing, or mixed hearing loss. The attachment of the Vibrant Soundbridge at the round window and the usage of the Vibroplasty couplers strongly expanded the application of the Vibrant Soundbridge. The Vibrant Soundbridge is developed for patients who have an intolerance to hearing aids and a moderate to profound sensorineural hearing loss. The VSB also provides an optimal solution for patients with failed middle ear reconstructions or patients with atres

Evaluation of implantable actuators by means of a middle ear simulation model.

The extension of indication of implantable hearing aids to cases of conductive hearing loss pushed the development of these devices. There is now a great variety of devices available with different actuator concepts and different attachment points to the middle ear or inner ear fluid. But there is little comparative data available about the devices to provide an insight into advantages and disadvantages of different types of actuators and attachment points at the ossicular chain. This paper investigates two principle (idealized) types of actuators in respect of attachments points at the ossicu

Active Middle Ear Implant in a Patient with Neurofibromatosis Type 1 and Multiple Calvarial Defects: A Case Report.

Bony abnormalities, including sphenoid dysplasia and calvarial defects, are well recognized in patients with neurofibromatosis type 1. However, having multiple calvarial defects is rare. We present a case of a 35-year-old Japanese male patient who was referred to our hospital because of hearing loss. He was diagnosed with neurofibromatosis type 1 during early childhood. Otoscopic examination revealed a protrusion from the anterior wall of the external auditory canal that obstructed the external auditory canal. Computed tomography findings revealed multiple defects and an uneven skull surface.

Intraoperative Auditory Brainstem Response Measurements via the Vibrant Soundbridge Active Middle Ear Implant: Comparison of Two Methods.

PURPOSE: The Vibrant Soundbridge (VSB) active middle ear implant can be used to treat adults and children with hearing loss. Intraoperative measurements based on auditory brainstem response (ABR) allow the determination of the implant's performance. Two methods for stimulus presentation during ABR measurements via VSB will be compared. METHOD: ABRs were measured in patients implanted with a VSB using a standard ABR system. The stimulus was transmitted directly to the VSB using a new transmission unit (AcoustiAP, MED-EL) and for comparison via a standard audio processor coupled to a wireless s

[Round window implantation of vibrant soundbridge through a retrofacial approach].

Objective:To investigate the specific retrofacial approach for application of vibrant soundbridge (VSB) to the round window (RW) in patients with complex congenital ear malformation. Methods:The clinical information of four patients with congenital ear malformation were retrospectively analyzed. VSB were implanted unilaterally in these four patients via a retrofacial approach. During the operation, it is difficult to reach the RW via a classic posterior tympanotomy pathway, due to the mastoid portion of FN positioned anteriorly severely. So the round window niche was identified through a retro

[Clinical aspects of otosclerotic ossicular chain fixations].

Otosclerosis is a human-specific ear disease characterised by complex bone-remodelling with multifactorial aetiology. It affects the bony labyrinth capsule and consequently fixates the stapes to the oval window rim. The fixation of the ossicular chain leads to a decrease in the middle ear's acoustic impedance-fitting and amplifier function that leads to conductive hearing loss. The process is progressive and when it involves the inner ear structures, it deteriorates the sensorineural function as well. The course of the illness can be prevented or delayed if hearing reconstructive surgery is pe
